Clinical significance of thyrotrophin binding inhibitor immunoglobulins in patients with Graves' disease and various types of thyroiditis.

It is well known that thyrotrophin receptor antibodies are present in the sera of patients with autoimmune thyroid disease. There is now compelling evidence that the hyperthyroidism of Graves’ disease is due to antibodies to the thyrotrophin (TSH) receptor. The measurement of these antibodies is valuable in the diagnosis and monitoring of Graves’ disease and in predicting the outcome of treatment. In the present study, thyrotrophin binding inhibitor immunoglobulin (TBII) activites were measured by radioreceptor assay, according to the method of Shewring and Smith, in 30 patients with Graves’ d

New-onset Graves' disease in the postpartum period.

During routine evaluation for lactation failure, hyperthyroidism was discovered in a postpartum woman. Although postpartum thyroiditis and Hashimoto's disease are relatively more common than Graves' disease during the postnatal period, this young woman was found to have new-onset Graves' disease. Distinguishing between normal postpartum symptoms and thyroid disorders can be challenging. Utilizing history, physical examination, and laboratory testing, the provider can identify the etiology of hyperthyroidism during the postpartum period. Treatment options differ depending on the cause of the th

Clinical, laboratory, and scintigraphic manifestations of subacute and chronic thyroiditis.

The term "thyroiditis" refers to several syndromes of differing etiology. Chronic or Hashimoto's thyroiditis (HT), an autoimmune disorder that is manifested by goiter and hypothyroidism, is by far the most common of these syndromes. Subacute thyroiditis (SAT) encompasses two distinct syndromes: subacute granulomatous thyroiditis (SAGT) and subacute lymphocytic thyroiditis (SLT). SAGT is viral in origin and usually presents with neck tenderness and hyperthyroid symptoms, while SLT, which is likely to be an autoimmune entity, results in goiter and transient hyperthyroidism. SLT is often seen in

Seasonal recurrence of transient hypothyroidism in a patient with autoimmune thyroiditis.

Two postpartum and 7 spontaneous episodes of transient hypothyroidism developed during 5 years of observation in a florist with autoimmune thyroiditis and seasonal allergic rhinitis. The spontaneous episodes recurred twice a year regularly in spring and in autumn, the seasons of her allergic rhinitis. In most of these episodes as well as in the postpartum ones, hypothyroidism was preceded by transient thyrotoxicosis. In addition to the similarity in the clinical course, the changes in antimicrosomal antibody titers in pregnancy-unrelated episodes were almost identical to those in postpartum ep

Maternal screening for hypothyroidism and thyroiditis using filter paper specimens.

BACKGROUND AND OBJECTIVE: Hypothyroidism and autoimmune thyroiditis are more prevalent than previously considered in women during pregnancy and the postpartum, and are associated with adverse effects on the mother and her fetus. We determined the efficacy and accuracy of screening women for primary hypothyroidism and autoimmune thyroiditis by testing TSH and two thyroid antibodies (TAb): thyroperoxidase antibodies (TPOAb) and thyroglobulin antibodies (TgAb), in eluates of filter paper specimens collected during early pregnancy and the postpartum. METHODS: We enrolled 494 first-trimester pregn

The thyroid and autoimmunity.

Autoimmune thyroid diseases (Hashimoto thyroiditis, Graves' disease, postpartum thyroiditis, atrophic thyroiditis and drug induced thyroiditis) are prevalent disorders worldwide, especially in women (related to the millieu of sex steroids and X chromosome effects on the thyroid and the immune system). Disruption of thyroid self tolerance, usually induced by an infection, generates abnormal thyroid--immune interactions, implicating an array of cytokines and their receptors. Thyrocytes achieve antigen presenting cell properties which stimulate effector immune cells (Th1, Th2, Th17), in the conte

Autoimmune thyroid disease: immunological, pathological, and clinical aspects.

Autoimmune thyroiditis, most notably Hashimoto's thyroiditis, appears to be increasing in prevalence and is now more easily detected by sensitive laboratory tests and more invasive procedures such as fine needle aspiration. During the last decade, marked progress has been made in the understanding of these diseases. There is a greater awareness of the interaction between the humoral and cell-mediated arms of the immune system in autoimmune thyroiditis. Recent studies implicate a subpopulation of suppressor T lymphocytes which have an antigen-specific defect, resulting in their suboptimal inter

Painless thyroiditis developed in a patient with Sheehan's syndrome.

We experienced an unusual combination of painless thyroiditis and Sheehan's syndrome in a single patient. A 29-year-old postpartum woman was referred to us for suspected Sheehan's syndrome. Endocrine tests confirmed the diagnosis, but, unexpectedly, her thyroid hormone levels in the plasma were elevated. Based on the data from various examinations on her thyroid including an extremely low uptake of radioactive iodine and spontaneous alterations of the thyroid function without any therapy for the thyroid, we diagnosed the patient as suffering from Sheehan's syndrome and painless thyroiditis sim

Impact of Thyroid Autoantibodies on Pregnancy Outcomes in Euthyroid Women: A Prospective Cohort Study.

Background Thyroid autoantibodies, including thyroid peroxidase antibodies (TPO-Ab) and thyroglobulin antibodies (TgAb), are commonly found in euthyroid women and have been linked to adverse pregnancy outcomes. However, their role in predicting complications in euthyroid pregnant women remains poorly defined. Objective This study aimed to evaluate the association between elevated TPO-Ab and TgAb levels and the risk of miscarriage, preterm birth, and postpartum thyroiditis in euthyroid women. Methods A prospective cohort study was conducted with 150 euthyroid pregnant women, with blood samples

[100 years of Hashimoto thyroiditis, still an intriguing disease].

INTRODUCTION: In 1912 Japanese physician Hashimoto Hakaru described 4 patients with chronic thyroid disease. The histopathology findings exactly 100 years ago described lymphocyte infiltration, fibrosis, parenchymal atrophy and eosinophilic changes of some acinar cells. Those findings are typical for the autoimmune thyroid disease named by the author Hashimoto thyroiditis or lymphocytic thyroiditis. Hashimoto thyroiditis: The pathophysiology of thyroid autoimmunity during the past decades was described in details. Many thyroid antigens were identified (thyroid - stimulating hormone or TSH, thy

Recurrent Episodes of Thyrotoxicosis in a Man following Pregnancies of his Spouse with Hashimoto's Thyroiditis.

Over an 8-year period, a male patient presented three times to an endocrinologist with strikingly similar presentations, including palpitations, anxiety, and tremors. Each of his presentations occurred following either the birth of one of his two children or his wife's late termination of pregnancy. This patient's illness followed the typical time course of silent thyroiditis: hyperthyroidism, followed by euthyroidism, a late hypothyroid phase, and then a complete resolution of symptoms and normalization of thyroid function tests over a period of several months. We discuss the curious clinical

Thyroiditis and SARS-CoV-2 pandemic: a review.

The world is dealing with the Covid-19 pandemic due to the coronavirus SARS-CoV-2. Amongst the extra-pulmonary manifestations presented by Covid-19 patients, thyroiditis form part of the spectrum of visceral involvement linked to SARS-CoV-2. In this review, we will describe the various documented clinical forms of thyroiditis (inflammatory thyroiditis, subacute or de Quervain's thyroiditis, chronic lymphocytic thyroiditis or Hashimoto's disease, painless (silent) postpartum thyroiditis) to facilitate their diagnosis in more or less symptomatic Covid-19 patients and to provide guidance for pati

Demystifying autoimmune thyroid disease. Which disorders require treatment?

The term "autoimmune thyroid disease" encompasses all of the autoimmune thyroid conditions, including Hashimoto's thyroiditis, Graves' disease, most cases of silent thyroiditis, and postpartum thyroiditis. Extrathyroidal manifestations (e.g., ophthalmopathy, dermopathy) can occur in Graves' disease and, less commonly, Hashimoto's thyroiditis. Spontaneous hypothyroidism is common in patients with Hashimoto's thyroiditis, and when it develops, life-long therapy with levothyroxine is needed. In the United States, most adult patients with Graves' disease are initially or eventually treated with ra

Thyroiditis: Evaluation and Treatment.

Thyroiditis is a general term for inflammation of the thyroid gland. The most common forms of thyroiditis encountered by family physicians include Hashimoto, postpartum, and subacute. Most forms of thyroiditis result in a triphasic disease pattern of thyroid dysfunction. Patients will have an initial phase of hyperthyroidism (thyrotoxicosis) attributed to the release of preformed thyroid hormone from damaged thyroid cells. This is followed by hypothyroidism, when the thyroid stores are depleted, and then eventual restoration of normal thyroid function. Some patients may develop permanent hypot

[Inflammatory diseases of the thyroid gland. Epidemiology, symptoms and morphology].

Inflammation of the thyroid gland makes up approximately 20% of all thyroid diseases. According to the clinical course, thyroiditis has been subdivided into acute, subacute, and chronic forms. Recent classifications are based on the fact that the majority of thyroiditis cases have an autoimmune background. The most common form of this disease is autoimmune thyroiditis (AIT), with or without subclinical/manifest hypothyroidism; less common forms of autoimmune thyroiditis comprise subacute granulomatous (de Quervain's) thyroiditis, postpartum thyroiditis, silent ("painless") thyroiditis, and inv

Effect of maternal thyroid autoantibodies and post-partum thyroiditis on the fetus and neonate.

Thirty-five pregnant women with thyroid antibodies were followed during pregnancy and 12 month post partum. Twenty antibody-negative women served as controls and none of these developed postpartum thyroiditis in contrast to 12 of 35 antibody-positive women. Umbilical cord blood was collected at birth for measurements of thyroid antibodies, and blood samples for measurements of thyroid hormones were obtained at 5 and 30 days of age in the infants together with a clinical examination. There were no differences between the infants of antibody-positive and -negative mothers or the infants of those

Recurrent transient hyperthyroidism and hypothyroidism associated with pregnancy.

We describe a female patient with insulin-dependent diabetes mellitus and autoimmune thyroiditis. Transient hyperthyroidism developed in the postpartum period in two successive pregnancies, followed by temporary hypothyroidism with eventual full recovery. The clinical course was consistent with "silent thyroiditis" and the circumstantial evidence suggested that alterations in the antibody titers due to pregnancy may have been responsible for these episodes. This case study serves to emphasize that hyperthyroidism and hypothyroidism in the postpartum period may be transient, and this fact shoul

[Pitfalls in cytologic diagnosis of thyroid disease associated with pregnancy].

The activation of the hypothalamo-pituitary thyroidal axis during pregnancy and puerperal period may provoke an unusual morphological response that is hard to distinguish from malignancy. Moreover, autoimmune thyroiditis (postpartum thyroiditis--PPT) becomes manifest or aggravated in some predisposed women prone to contract thyroiditis with the termination of the physiological gestation immunosuppression. In five cytologically investigated women (some of whom were examined repeatedly) during the postpartum period less typical goitre and lymphoplasmacytic thyroiditis were observed. In two of th

The prevalence of elevated serum C-reactive protein levels in inflammatory and noninflammatory thyroid disease.

C-reactive protein (CRP) levels have not been routinely used to diagnose thyroid disease, although many thyroid conditions involve inflammation. This study was intended to determine whether CRP levels could differentiate between inflammatory and noninflammatory thyroid conditions, especially between type II inflammatory amiodarone-induced thyrotoxicosis (AIT) and type I iodine-induced AIT. Serum high-sensitivity CRP levels were measured in 100 euthyroid controls (7 taking amiodarone) and 353 patients with one of the following thyroid conditions: AIT, subacute thyroiditis, toxic diffuse goiter,

Thyroiditis: differential diagnosis and management.

Thyroiditis is a group of inflammatory thyroid disorders. Patients with chronic lymphocytic thyroiditis (also referred to as Hashimoto's thyroiditis) present with hypothyroidism, goiter, or both. Measurement of serum thyroid autoantibodies and thyroglobulin confirms the diagnosis. Subacute granulomatous thyroiditis (sometimes referred to as de Quervain's disease) is a self-limited but painful disorder of the thyroid. Physical examination, elevated erythrocyte sedimentation rate, elevated thyroglobulin level and depressed radioactive iodine uptake (RAIU) confirm the diagnosis. Subacute lymphocy
