BACKGROUND: Obesity affects over one billion people globally and is a leading contributor to chronic disease. For those with clinically severe obesity, metabolic and bariatric surgery (MBS) is the most effective intervention for long-term weight loss. However, surgery is often delayed due to systemic barriers, during which time patients may experience further health decline. Low cardiorespiratory fitness is a known risk factor for perioperative complications, prompting recommendations for prehabilitation to target readiness for surgery. Despite this, few patients meet physical activity guideli

BACKGROUND: Severely obese patients have decreased cardiorespiratory fitness (CRF) and poor functional capacity. Bariatric surgery-induced weight loss improves CRF, but the determinants of this improvement are not well known. We aimed to assess the determinants of CRF before and after bariatric surgery and the impact of an exercise training program on CRF after bariatric surgery.
METHODS: Fifty-eight severely obese patients (46.1 ± 6.1 kg/m2, 78% women) were randomly assigned to either an exercise group (n = 39) or usual care (n = 19). Exercise training was conducted from the 3rd to the 6th m

AIMS: Restrictive mitral annuloplasty (RMA) can provide a durable solution for functional mitral regurgitation (MR), but might result in obstruction to antegrade mitral flow. Aim of this study was to assess the magnitude of change in mitral valve area (MVA) during exercise after RMA, to relate the change in MVA to left ventricular (LV) geometry and function, and to assess its haemodynamic and clinical impact.
METHODS AND RESULTS: Bicycle exercise echocardiography was performed in 32 patients after RMA. Echocardiographic data at rest and during exercise were compared with preoperative echocard

INTRODUCTION: Recent efforts to prehabilitate intermediately frail and frail (Fried frailty criteria ≥2) elective colorectal cancer patients did not influence clinical nor functional outcomes. The objective of this secondary analysis was to describe the subset of intermediately frail and frail prehabilitated patients who could not attain a minimum 400 m (a prognostic cut-point used in other patient populations) 6-min walking distance (6MWD) before elective surgery.
MATERIALS AND METHODS: Secondary analysis of a randomized controlled trial. Patients participated in multimodal prehabilitation a

BACKGROUND: Prehabilitation aims to increase functional capacity and physiological reserve prior to surgery, to improve post-operative outcomes. Despite growing research on prehabilitation for total hip arthroplasty (THA) and total knee arthroplasty (TKA), its acceptability and barriers and enablers to implementation in clinical settings are not well understood.
OBJECTIVE: This study, involving patients and healthcare professionals (HCPs), aimed to explore the acceptability of prehabilitation, identify factors affecting prehabilitation access, uptake and delivery, and investigate perspectives

INTRODUCTION: Colorectal cancer is a leading cause of cancer mortality in the USA and occurs most frequently in older adults. These patients are at increased risk of adverse outcomes following major cancer surgery. While prehabilitation has been shown to mitigate this risk, multiple barriers to implementation remain. Our team created a digital tool co-designed with older adults that employs an algorithm to assess patient-specific geriatric vulnerabilities and generate personalised prehabilitation programmes before surgery.
METHODS AND ANALYSIS: We have designed a multisite, unblinded randomis

PURPOSE: This study aimed to explore patient experience of participating in a multimodal prehabilitation program prior to nerve-sparing radical prostatectomy (RP).
METHODS: We used an exploratory hermeneutical phenomenological approach where qualitative interview data were analysed, building on the theory of Paul Ricoeur. Six weeks after surgery, we interviewed eight patients that had been recruited for a randomised controlled feasibility study of a multimodal prehabilitation program prior to a nerve sparing RP.
RESULTS: Prehabilitation offered patients a chance to actively contribute to the

INTRODUCTION: Cirrhosis is the main cause of morbidity and mortality globally, accounting for approximately 1.2 million deaths annually. Impaired aerobic capacity, muscle wasting and reduced muscle strength are significant complications in patients with cirrhosis. Preoperative exercise intervention "prehabilitation" has been recognised as a potential approach to optimise muscle strength, aerobic capacity and body composition as well as quality of life in patients awaiting abdominal surgery. However, there is little evidence on the effects of preoperative exercise on older adults with cirrhosis

BACKGROUND: Obesity is a growing public health issue associated with comorbidities and substantial medical costs. Although bariatric and metabolic surgery (BMS) is often recommended for individuals with severe obesity, prehabilitation may optimize their physical and psychological status. However, face-to-face delivery can limit accessibility, and evidence on home-based approaches remains scarce.
OBJECTIVE: This study protocol aims to compare the effectiveness of a home-based prehabilitation program with a face-to-face prehabilitation program on physical and psychological variables in individu

For colorectal surgery patients, prehabilitation can address common challenges in recovery, including muscle wasting, malnutrition, and emotional stress. This study aim to determine the impact of prehabilitation for patients with after colorectal surgery. We searched PubMed, Embase and Cochrane Library from inception up to April 2024. Two authors assessed the risk of bias by the Cochrane Risk of Bias tool-version 2. We computed mean difference (MD) on the basis of change values, with 95% credible interval (CI), for continuous outcomes (length of hospital stays (LOS and 6-min walk test (6MWT)),

BACKGROUND: Preoperative prehabilitation programmes for patients with colorectal cancer have been shown to reduce complications and hospital length of stay. However, supervised weekly physiotherapy is expensive and timeconsuming for both healthcare professionals and patients, leading to suboptimal implementation of prehabilitation programmes. A previous study demonstrated that offering home-based prehabilitation through an app is feasible and leads to outcomes comparable to supervised prehabilitation programmes. This study was conducted at a single hospital. To expand this promising modality n

QUESTIONS: Among patients having elective abdominal surgery, how much does preoperative physiotherapy education with breathing exercise training reduce the incidence of postoperative pulmonary complications (PPCs), hospital length of stay and 12-month mortality? How stable are the treatment effects across different PPC definitions, including pneumonia? How much do the treatment effects on PPC, hospital length of stay and mortality vary within clinically relevant subgroups?

BACKGROUND: Severe aortic stenosis (AS) is often accompanied by diastolic dysfunction. After aortic valve replacement (AVR), the left ventricle often undergoes considerable reverse remodeling. Despite this, diastolic dysfunction may persist after AVR. The aims of this study were to determine the incidence of elevated left ventricular (LV) filling pressure at rest and during exercise among patients with severe AS after AVR and to describe factors related to elevated LV filling pressure, especially its association with LV and left atrial remodeling and myocardial fibrosis.
METHODS: Thirty-seven

INTRODUCTION: Preoperative functional capacity is considered an important risk factor for cardiovascular and other complications of major non-cardiac surgery. Nonetheless, the usual approach for estimating preoperative functional capacity, namely doctors' subjective assessment, may not accurately predict postoperative morbidity or mortality. 3 possible alternatives are cardiopulmonary exercise testing; the Duke Activity Status Index, a standardised questionnaire for estimating functional capacity; and the serum concentration of N-terminal pro-B-type natriuretic peptide (NT pro-BNP), a biomarke
