BACKGROUND: Synovial fluid proteins had been applied as diagnostic biomarkers for periprosthetic joint infection (PJI) in recent research papers. Thus, this meta-analysis aimed to estimate the diagnostic efficiency of synovial fluid α-defensin and leukocyte esterase (LE) for PJI.
METHODS: We conducted our systematic review by searching the keywords in online databases such as PubMed, Embase, Cochrane, Elsevier, Springer, and Web of Science from the time of database inception to October 2018. Inclusion criteria were as follows: patients who have undergone knee, hip, or shoulder joint replaceme

BACKGROUD: Daptomycin has been used in bone and joint infections (BJI) and prosthesis joint infections (PJI) considering spectrum of activity and biofilm penetration. However, the current experience is based on case reports, case series, cohorts, and international surveys. The aim of this systematic review was to evaluate studies about daptomycin treatment efficacy in BJI/PJI compared to other antibiotic regimens.
METHODS: PubMed, LILACS, Scielo and Web of Science databases were searched for articles about daptomycin and treatment of BJI and PJI from inception to March 2018. Inclusion criteri

BACKGROUND: Unexpected microbial signals are frequently encountered in patients with orthopaedic implants who do not satisfy validated criteria for confirmed infection. The central interpretive problem is not limited to periprosthetic joint infection (PJI); it also arises in fracture-fixation and spinal instrumentation, and it involves conventional culture, sonication, histopathology, broad-range polymerase chain reaction (PCR), 16 S rRNA sequencing, and metagenomic next-generation sequencing (mNGS).
METHODS: We performed a structured narrative review of literature identified in PubMed, Embas

With the increase in the number of orthopedic surgeries, such as knee and hip replacement, the risk of peri-implant infection also increases. According to various registries, the number of such interventions continues to grow, making the problem of infections particularly relevant. Morphological assessment of peri-implant tissues, primarily the peri-implant membrane, has become routine practice and is used as one of the diagnostic criteria for infection. This review summarizes the main morphological changes in the peri-implant membrane formed at the bone/implant interface, presents the pathoge

BACKGROUND: Periprosthetic joint infections (PJIs) are severe complications of arthroplasty in which biofilm formation on implant surfaces compromises microbiological diagnosis and antimicrobial efficacy. Although staphylococci remain the predominant pathogens, Gram-negative bacilli have increasingly been associated with diagnostic failure and unfavorable clinical outcomes. This study aimed to evaluate the formation, maturation, and structural organization of Gram-positive and Gram-negative bacterial biofilms and to investigate the effects of a standardized sonication protocol on biofilm disru

UNLABELLED: Implant-associated infections, including periprosthetic joint infections (PJI) and fracture-related infections (FRI), are severe complications following orthopedic surgery. Conventional tissue cultures often yield false-negative results, especially after antibiotic initiation or in chronic biofilm-associated infections. Sonication of explanted implants enhances bacterial recovery, but heterogeneous protocols limit comparability. This scoping review maps sonication protocols currently reported in the literature. A comprehensive literature search across three databases (PubMed, Embas

OBJECTIVES: To describe diagnostic delay and treatment burden in Brucella periprosthetic joint infection (PJI) and propose a stratified evidence-informed clinical framework.
METHODS: We comprehensively reviewed published cases of Brucella PJI and summarized data from 80 patients to describe epidemiology, clinical features, diagnosis, treatment, and reported outcomes.
RESULTS: Of 80 patients, 59.72% were not diagnosed before initial treatment. Low culture yield and failure to meet conventional PJI criteria appeared to contribute to delayed recognition. Fifty-five percent received ≥3 treatment

Since its first description in the late nineteenth century, the epidemiology of infective endocarditis (IE) has changed considerably. Once primarily affecting younger individuals with structural heart disease, IE is now increasingly encountered in older patients with multiple comorbidities and frequent healthcare exposure. Population ageing, end-stage renal disease (ESRD), immunosuppression, and injection drug use (IDU) have broadened the pool of susceptible hosts. At the same time, the increasing use of prosthetic valves (PVs), cardiac implantable electronic devices (CIEDs), and transcatheter

Periprosthetic joint infection (PJI) is one of the most challenging complications following total joint arthroplasty, contributing to high morbidity, increased health care costs, and frequent implant failures. The current diagnostic methods, including synovial fluid analysis and microbiological cultures, are limited by their sensitivity and specificity, particularly in culture-negative infections. As a result, there is a growing need for novel diagnostic technologies that can provide rapid and reliable detection of PJI. This review explores emerging diagnostic strategies, including real-time i

Despite limited evidence for a preoperative dental screening examination and needs-based focal rehabilitation, as well as the need for antibiotic prophylaxis prior to invasive dental procedures, oral health is and remains a relevant topic in arthroplasty. The aspect of oral health should not be neglected in the future in connection with periprosthetic infections and should continue to be the subject of intensive research. The frequency of conspicuous dental findings in middle and old age underlines the importance of this problem. A dental examination prior to arthroplasty implantation can gene

The concept of Debridement, Antibiotics and Implant Retention (DAIR) is well known in periprosthetic joint infections. Extrapolating this concept to fracture related infections is mired in controversies. Characteristics of the metal implant, duration of infection, state of fracture healing, microbiological profile etc. appear to play a role in the decision making process whether or not to keep, adjust , exchange or remove (infected) metalwork. More than likely it is the quality of source control by meticulous debridement having a major impact whether a DAIR approach to FRI could result in a su

Accurate diagnosis of orthopedic infection is crucial in guiding both antimicrobial therapy and surgical management in order to optimize patient outcomes. A variety of microbiological and nonmicrobiological methods are used to establish the presence of a musculoskeletal infection. In this minireview, we examine traditional culture-based and newer molecular methodologies for pathogen detection, as well as systemic and localized assays to assess host response to maximize diagnostic yield.

Fungal periprosthetic joint infection (PJI) is a devastating complication because it can be difficult to diagnose, manage, and eradicate. Fungal PJI treatment requires a systematic approach. Increased awareness is essential when patients with painful arthroplasties present with immunosuppression, significant comorbidities, multiple surgeries, and history of drug use. Every suspected fungal PJI should be promptly diagnosed using readily available serum and synovial fluid markers. Surgical management involves débridement, antibiotics, and implant retention, one-stage exchange arthroplasty, prost

Periprosthetic joint infection (PJI) remains one of the most serious complications of arthroplasty. Since the publication of major guidelines, several aspects of the prevention of PJI have been refined, although high-quality evidence remains limited in many areas. The aim of this annotation was to provide an updated overview of current preventive strategies, and to summarize the latest recommendations in perioperative practice.

Microorganisms, including bacteria, fungi, and viruses, that reside on and within the human body are collectively known as the human microbiome. Dysbiosis, or disruption in the microbiome, has been implicated in several disease processes, including asthma, obesity, autoimmune diseases, and numerous other conditions. While the Human Microbiome Project and the generation of descriptive studies it inspired established correlations between characteristic patterns in the composition of the microbiome and specific disease phenotypes, current research has begun to focus on elucidating the causal role
