Psychopathological and neuropsychological outcomes of deep brain stimulation for severe- treatment-resistant obsessive-compulsive disorder: An open-label case series.

OBJECTIVE: Deep brain stimulation (DBS) is considered a promising intervention for treatment-resistant obsessive-compulsive disorder (OCD). The present study describes the outcomes of the first DBS procedures for OCD in Iran. METHODS: Four women patients (age range, 25-35 years) with severe OCD meeting stringent criteria for refractoriness to treatment were selected by Psychosurgery Review Board. DBS electrodes were bilaterally implanted in the internal capsule and nucleus accumbens (NAc). Clinical and neuropsychological assessments were undertaken before and after implantation. The outcomes

Complications of capsulotomy in the treatment of psychiatric illness: A systematic review.

For more than half a century, stereotactic neurosurgical procedures have been available in the treatment of patients with severe, debilitating symptoms of treatment-resistant psychiatric conditions such as obsessive-compulsive disorder (OCD). Such surgical interventions include stimulation and lesioning techniques. Capsulotomy is a lesioning procedure targeting the internal capsule. This systematic review aims to explore the safety profile of capsulotomy in the treatment of severe medically-refractory psychiatric illness, focusing on its complications. Methodologically, a literature search was

Serendipity and Observations in Functional Neurosurgery: From James Parkinson's Stroke to Hamani's & Lozano's Flashbacks.

BACKGROUND: Serendipity and observations have a noble tradition in medicine, including neurology, and are responsible for many medical treatments (carbamazepine for tic douloureux, amantadine for Parkinson's disease, gabapentin for restless legs…). We aimed at examining the contribution of serendipity and observations to functional neurosurgery. Scholarly publications relevant to the history of functional neurosurgery for movement and psychiatric disorders were reviewed, starting from the pre-stereotactic era. The documents were scrutinized with respect to indications for surgery, surgical met

No prevention or cure of epilepsy as yet.

Approximately 20% of all epilepsy is caused by acute acquired injury such as traumatic brain injury, stroke and CNS infection. The known onset of the injury which triggers the epileptogenic process, early presentation to medical care, and a latency between the injury and the development of clinical epilepsy present an opportunity to intervene with treatment to prevent epilepsy. No such treatment exists and yet there has been remarkably little clinical research during the last 20 years to try to develop such treatment. We review possible reasons for this, possible ways to rectify the situations

Schizophrenia and neurosurgery: A dark past with hope of a brighter future.

Schizophrenia is a chronic and progressive psychiatric disease that remains difficult to manage in the 21st century. Current medical therapies have been able to give reprieve and decrease incidence of psychotic episodes. However, as the disease progresses, patients can become ever more refractory to current pharmaceutical agents and the polypharmacy that is attempted in treatment. Additionally, many of these drugs have significant adverse effects, leaving the practitioner in a difficult predicament for treating these patients. The history of neurosurgery for schizophrenia, among other psychiat

[Epilepsy and psychoses (from the viewpoint of a neurosurgical service)].

The experience of a neurosurgical unit devoted to the surgical therapy of the epilepsies is only based on very peculiar patients. However, relationship between a given epilepsy and associated psychiatric disorders are more easily understood because of more extensive explorations. Bilateral temporal epilepsies involving the limbic system on the one hand, bilateral frontal epilepsies on the other one, and P.M. status which may be paralleled, make these patients more susceptible to acute mental confusions, to acute thymic disorders, to delirious attacks. Direct relations between chronic psychosis

[Prosopagnosia. Role of the right hemisphere in visual perception. (Apropos of a case after right occipital lobectomy)].

The authors describe a case of prosopagnosia which appeared a month after right occipital lobectomy. Results obtained by means of various tests devised to analyse this perception disorder have produced the following conclusions: 1) results of pairing tests which require a high level of perceptive differentiation are successful but after abnormally long delays; 2) in spite of this success, faces--even the patient's own--are not recognized; 3) this disorder extends to affect every task requiring recognition of the individual nature of a stimulus; 4) this individuality has the peculiarity of bein

Game Changers in the Treatment of and Engagement With Persons Living With Mental Health and Substance Use Disorders: The Role of the Nurse.

The game changers in mental health and substance use disorder treatment have been shaped by historical sea changes marked by transformative advancements that have significantly enhanced patient care. Breakthroughs in psychotherapy, psychosurgery and psychopharmacology have each contributed to reshaping clinical practices, especially the revolutionary discovery of psychotropics. More recently, the rapid development of artificial intelligence has begun to address long-standing barriers in access to care for individuals experiencing mental health and substance use disorders. Transitioning from pa

LSD treatment in Scandinavia: emphasizing indications and short-term treatment outcomes of 151 patients in Denmark.

BACKGROUND: New research has suggested the clinical use of lysergic acid diethylamide (LSD) and psilocybin in selected patient populations. However, concerns about the clinical use of LSD were advanced in a large Danish follow-up study that assessed 151 LSD-treated psychiatric patients approximately 25 years after their treatment in the 1960s. AIMS: The purpose of the present study was to give a retrospective account of the short-term outcome of LSD treatment in these 151 Danish psychiatric patients. METHODS: The LSD case material in the Danish State Archives consists of medical case records

Treatment of the neuroleptic-nonresponsive schizophrenic patient.

The treatment and management of neuroleptic-resistant schizophrenic patients, who comprise 5 to 25 percent of all patients with that diagnosis, are major problems for psychiatry. In addition, another large group of schizophrenic patients, perhaps 5 to 20 percent, are intolerant of therapeutic dosages of neuroleptic drugs because of extrapyramidal symptoms, including akathisia, parkinsonism, and tardive dyskinesia. Because about 60 percent of neuroleptic-resistant schizophrenic patients respond to clozapine and a large percentage of neuroleptic-intolerant patients are able to tolerate clozapine

[Psychobiology of compulsive disease].

The predominant psychobiological hypotheses regarding the pathophysiology of obsessive-compulsive disorder (OCD) are that a selective basal ganglia dysfunction and a dysregulation of one or several central serotonin (5-HT) subsystem are related to at least some aspects of the syndrome. Recent neuroanatomical, -pharmacological, and -ethological studies indicate a complex perceptual and cognitive role for the basal ganglia, particularly the striatum and the pallidum, in addition to the well-established motor functions. Obsessive-compulsive symptoms in syndromes with extrapyramidal-motor dysfunct

[Safety and neuroethical consideration of deep brain stimulation as a psychiatric treatment].

Deep brain stimulation (DBS) is a surgical treatment involving the implantation of a medical device, which sends electrical impulses to specific parts of the brain. DBS in select brain regions has provided remarkable therapeutic benefits for treatment-resistant movement and affective disorders such as Parkinson's disease. DBS directly alters brain activity in a controlled manner, and its effects are reversible. Lately, DBS treatment has been used for the treatment of various psychiatric disorders, such as obsessive compulsive disorder, Tourette syndrome, and severe depression. However, DBS the

Cerebral lateralization: relationship of language and ideomotor praxis.

OBJECTIVE: To determine the relationship of language lateralization and hand preference to praxis performance following left and right hemispheric amobarbital-induced inactivations. BACKGROUND: Patients who are aphasic from left cerebral dysfunction also frequently exhibit ideomotor apraxia in which they make temporal, spatial, and postural errors of learned skilled movements. However, hemispheric lateralization of the systems mediating ideomotor praxis in patients with atypical cerebral language dominance (i.e., bilateral or right hemispheric language function) remains uncertain. METHODS: S

Hemispherectomy--a modification.

Hemispherectomy is probably the best operation for treating epilepsy. It was abandoned because of the delayed haemorrhagic complications that developed some years after the operation. A hypothesis is described to account for the mechanism of such complications and modifications made to the operative technique to prevent such complications. This modified hemispherectomy has been carried out on four patients since 1980.
