Laparoscopic versus open radical nephrectomy for xanthogranulomatous pyelonephritis: Contemporary outcomes analysis.

BACKGROUND AND PURPOSE: Early experience with laparoscopic nephrectomy for xanthogranulomatous pyelonephritis (XGP) was associated with high conversion and complication rates. We describe our institution's experience with this procedure compared with a contemporary cohort of patients with XGP treated by open nephrectomy. PATIENTS AND METHODS: Retrospective review of surgical pathology reports from radical nephrectomies at our institution between 1995 and 2005 yielded a diagnosis of XGP in 12 patients, 6 of whom had undergone laparoscopic surgery. Each patient's medical records were reviewed,

[Xanthogranulomatous pyelonephritis. Diagnostic and therapeutic problems. Report of three paediatric cases].

Xanthogranulomatous pyelonephritis (XGP) is an unusual and rare form of chronic renal suppuration in children, which appear to have a multifactorial pathogenesis. It is usually diffuse, but can remain localized to one part of the kidney, resulting in a misleading pseudoneoplastic appearance raising a problem of differential diagnosis with the other renal masses, particularly Wilms tumour. The authors present a retrospective analysis of three cases of XGP observed in the Monastir department of paediatric surgery. The preoperative diagnosis was pyonephrosis with perirenal abscess in two cases an

Xanthogranulomatous pyelonephritis infected with the Providencia stuartii: a case report and literature review.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is a rare and severe chronic inflammatory disease of the renal parenchyma, which is most commonly associated with super-infections by bacteria such as E. coli, Proteus mirabilis, and occasionally Pseudomonas species. CASE PRESENTATION: Herein, we present a rare case of a patient with XGP infected with Providencia stuartii. Initially, the patient refused nephrectomy and underwent holmium laser lithotripsy and right ureteral stenting, followed by meropenem treatment of 7 days. Relapse occurred in the third month after discharge from the hospi

Massive Renal Replacement Lipomatosis With Foci of Xanthogranulomatous Pyelonephritis in a Horseshoe Kidney.

Renal replacement lipomatosis (RRL) is a rare condition that occurs at the end of the spectrum of renal tissue replacement by fat. Xanthogranulomatous pyelonephritis (XGP) is a granulomatous inflammation characterized by destruction of renal parenchyma and replacement by lipid-laden macrophages. We present the case of a 75-year-old man who complained of severe anemia 34 years after right nephrolithotomy. Computed tomography revealed a huge low-density mass with renal parenchyma atrophy on the right side of horseshoe kidney. Right nephrectomy was performed. Pathological diagnosis was RRL with X

Early Progression of Xanthogranulomatous Pyelonephritis in Children Might Be Dependent on Vimentin Expression.

BACKGROUND Xanthogranulomatous pyelonephritis (XP) is an extremely rare, severe, atypical form of chronic renal parenchymal inflammation accompanied by hydronephrosis and/or urolithiasis. The pathomechanism of XP is not yet fully understood. Microscopically, XP is indicated by the presence of multinucleated giant cells and lipid-laden macrophages, as well as inflammatory infiltration and intensive renal fibrosis. The lipid accumulation in kidney parenchyma may be secondary to the altered flow of low-density lipoprotein (LDL)-derived cholesterol particles inside the affected cells. Physiologica

An unusual case of xanthogranulomatous pyelonephritis in an immunosuppressed patient with systemic lupus erythematosus.

Xanthogranulomatous pyelonephritis (XGP) is invariably associated with urinary tract obstruction, infection and nephrolithiasis. We report an unusual case of XGP occurring in an immunosuppressed patient in the absence of either urinary obstruction or nephrolithiasis, presenting as a diagnostic dilemma, and highlighting the possible role of glucocorticoid immunosuppression towards its pathogenesis.

Xanthogranulomatous pyelonephritis in adults: clinical and radiological findings in diffuse and focal forms.

AIM: To describe the clinical and radiological features of focal and diffuse xanthogranulomatous pyelonephritis (XGP) in adults. MATERIALS AND METHODS: A retrospective review of the clinical data, laboratory findings, imaging features, and surgical treatment of 13 cases of histologically proven XGP diagnosed between January 1993 and December 2005 was undertaken. There were 10 women and three men with a mean age of 55.2 years (range 30-87 years). All patients underwent both sonography and computed tomography (CT) of the kidneys. Magnetic resonance imaging (MRI) was performed in two patients.

[Xanthogranulomatous pyelonephritis presenting a subcutaneous mass in the lower abdomen: a case report].

A 72-year-old female presented with the complaint of left lower abdominal swelling in May, 2005. Laboratory analysis revealed elevated white blood cell counts and C-reactive protein. Abdominal computed tomography showed left ureteral calculi, left hydronephrosis and a mass extending through the perinephric space, psoas major muscle into the left flank and lower abdomen. Echo-guided needle mass biopsy was performed. Histopathological findings revealed xanthogranulomatous changes. Under the diagnosis of diffuse xanthogranulomatous pyelonephritis extended into psoas muscle and subcutaneous tissue

[Xanthogranulomatous pyelonephritis: conservative therapy in the para-nephritic stage. Apropos of a case].

Xantogranulomatous pyelonephritis is a rare disease, occuring most frequently in middle-aged woman. The patients usually present with recurrent fever, dysuria, renal colic or aspecific findings. We report a case of focal PXG (paranephric stage) treated with conservative surgery. Generally, treatment consists of nephrectomy for diffuse or advanced stage diseases, excision of the diseased renal segment for localized and low/high stage.

[Pseudotumor focal xanthogranulomatous pyelonephritis in adults. Role of imaging technics in the diagnosis].

Based on 8 personal cases, pseudo-tumoral xanthogranulomatous pyelonephritis is reviewed with emphasis on diagnosis. The condition is a particular form of chronic renal suppuration of histologic definition (combined lesions of chronic pyelonephritis and xanthogranulomatous foam cells). Two forms are recognized: one diffuse, fairly frequent form corresponding to a pyonephrosis, and a pseudo tumoral focal form, the only type discussed in this report, which raises the problem of diagnosis of a renal mass that requires the application of all currently available exploration means to define its true

Xanthogranulomatous pyelonephritis-presentation as acute renal failure in a child with a solitary kidney.

A 4-year-old girl, who had been in perfect health, presented with the acute onset of renal failure and abdominal mass. Investigations revealed a solitary left kidney which on histology had the typical features of xanthogranulomatous pyelonephritis. The child had no previous history suggestive of chronic renal disease and had no radiologic evidence of renal osteodystrophy. She had no improvement in renal function and has gone on to hemodialysis and transplantation. The unique mode of presentation and the unusual features of this case are discussed.

Co-existent massive renal replacement lipomatosis and xanthogranulomatous pyelonephritis--a case report.

Renal replacement lipomatosis is a benign condition that may assume major clinical significance by producing pyelocalyceal deformities that may be mistaken for true renal masses. While the number of reported cases of renal replacement lipomatosis has been relatively small, this entity's prevalence is probably underestimated. We report a case of co-existent massive renal replacement lipomatosis and xanthogranulomatous pyelonephritis.

Xanthogranulomatous Pyelonephritis in a Horseshoe Kidney: Effective Conservative Therapy in an Immunocompromised Patient.

Xanthogranulomatous pyelonephritis (XGPN) is a rare, chronic granulomatous inflammation that leads to diffuse destruction of the renal parenchyma. This destructive process may be so profound as to infiltrate surrounding tissues and viscera. Although nephrectomy remains the most common option for managing XGPN in adults, we report a case of XGPN in a horseshoe kidney of a 27-year-old woman on active immunosuppressive therapy for inflammatory bowel disease, namely ulcerative colitis. This case was successfully treated conservatively with antimicrobials, with complete resolution of the renal mass

Xanthogranulomatous pyelonephritis caused by an unusual pathogen Raoultella ornithinolytica: a case report.

Xanthogranulomatous pyelonephritis (XGP) is a rare and severe chronic inflammatory condition of the kidney. XGP's exact etiology remains unclear, although it is frequently associated with chronic urinary obstruction and infections within the urinary tract. Superinfections such as Escherichiacoli, Proteusmirabilis, and occasionally Pseudomonas species typically cause XGP. Documented cases of Raoultellaornithinolytica (R.ornithinolytica) infections in humans, particularly within the urinary tract, are limited. However, advancements in diagnostic techniques are expected to increase the detection
