A common causative organism in osteomyelitis in sickle cell disease is Salmonella. Septic arthritis and muscle infection due to Salmonella are much less common. We present a case of a 28-year-old woman with sickle cell disease who presented with left shoulder and elbow pain for two days. Physical examination revealed swelling of the left upper arm. The patient was initially treated for a sickle cell pain crisis. On hospital day 4, the patient developed a fever. She empirically started intravenous vancomycin and cefepime before her blood culture showed Salmonella. Subsequently, the antibiotic w

Pyomyositis is the bacterial infection of skeletal muscle, usually accompanied by abscesses. The main etiologic agent is Staphylococcus aureus. There are rare cases attributed to Streptococcus pneumoniae. This paper presents an autopsy of a four year old child with multiple congenital intramuscular hemangiomas that developed pneumococcal pyomyositis associated with meningitis. The authors propose the hypothesis that patients with hemangiomas, mainly the intramuscular type, may also represent a risk group for pyomyositis. The possibility of respiratory/meningeal co-infection, might also be cons

The authors present three case reports of primary pyomyositis, a severe but rare disorder involving the muscles around the hip. In three boys, with an average age of 16 years, the disease developed suddenly in association with strenuous sporting activities. The boys had fever, pain and restricted motion at hip joints, haemoculture tests positive for Staphylococcus aureus and the presence of inflammatory markers. Magnetic resonance findings showed infiltrates and abscesses in the muscles around the hip. X-ray and computed tomography (CT) examination of the pelvis revealed bone irregularities ne

Pyomyositis is a suppurative infection of the skeletal muscle; it mainly occurs in immunocompromised patients or, exceptionally, in immunocompetent patients in tropical or other areas. We present a 24-year-old immunocompetent lady with bilateral thigh myalgia and fever. Upon investigation, extensive multifocal bilateral fluid collections involving the extensor muscles of both thighs were demonstrated. Pus aspirate from the involved muscles proved the presence of Staphylococcus aureus. Incision and drainage of the involved muscles were performed with successful and complete recovery.

INTRODUCTION: There has been a recent increase in the incidence of myositis worldwide. To this date, myositis has been described almost exclusively in children and adolescents. In most cases the causative agents are Gram-positive bacteria. When it involves muscles around the hip, other differential diagnoses such as septic arthritis and transient synovitis need to be excluded amongst others.
CASE PRESENTATION: We present the case of a 16 year old Caucasian male who suffered from pyomyositis of his left obturator internus muscle. He was pyrexial at 41 degrees C with rigors and severe hip pain,

CASE REPORT: A 25-year-old man presented with inverse hypopyon due to dense inflammatory exudation in the left eye. He was an ascetic with a history of long pilgrimages barefoot. Drainage of the pyogenic material with superficial exploration revealed localized scleral thinning at 2'o clock. Two days later, a thigh abscess was noted, which on ultrasonography showed hypoechoic areas with increased muscle mass suggestive of tropical pyomyositis. The culture from anterior chamber aspirate grew Staphylococcus aureus, as did 200 mL of pus drained from the thigh. His ocular condition improved signifi

Fifteen patients, nine males and six females, diagnosed with pyomyositis from 1988 to 1994, and followed for an average of 69.8 months, were reviewed. Excluding two children, the average age was 56.6 years. Eleven adults (73.3%) had underlying diseases. The lesions were multiple in five patients (33.3%) and a total of twenty-four muscle abscesses, including eleven extrapelvic and thirteen intrapelvic, were identified. When comparing extra- and intrapelvic pyomyositis, intrapelvic pyomyositis presents a diagnostic challenge requiring a high index of suspicion. Distinct clinical features such as

Four cases of non-tropical pyomyositis in adults are presented and 72 cases reported in the literature are reviewed. The diagnosis is often overlooked or delayed because most physicians are not familiar with the entity. Local signs of inflammation fever, leukocytosis and an elevated erythrocyte sedimentation rate are common features. Staphylococcus aureus is the most common pathogen and the thigh muscles are the most common site involved. Computerized tomography is used to establish the diagnosis and surgical incision and drainage in combination with antibiotic therapy is successful in the maj

The plasma concentration of vitamin C of 14 patients with tropical pyomyositis without clinical evidence of scurvy and that of II age and sex-matched controls was determined in order to ascertain whether or not there was a deficiency of the vitamin in this condition. The results show that the mean plasma concentrations of vitamin C of patients were slightly higher (15.9 +/- 6.4 g/l) than those of controls (12.0 +/- 4.5 g/l). The differences, however, were not statistically significant (P greater than 0.25). It is therefore concluded that lack of vitamin C does not play a part in the aetiology

Spontaneous pyomyositis, or so-called tropical pyomyositis, developed in the hamstring muscles of a 62-year-old man with rheumatoid arthritis and Felty's syndrome. The diagnosis was confirmed by computerized tomography, and the appropriate muscle compartment was surgically drained.

Background : Pyomyositis is a subacute bacterial infection of the skeletal muscles. Its most common features are fever, muscle swelling, and focal pain. There have been insufficient data regarding pyomyositis in healthy infants. Case report : A one-month-old boy presented with an impairment of his left arm movement. He was well-nourished and not under any apparent distress. His vital signs were within the normal limits but neurological examination revealed left forearm paralysis. Physical examination showed no abnormal findings in the region from the left shoulder joint to the fingertips. Cons

BACKGROUND: Pyomyositis is typically caused by gram-positive bacteria, especially Staphylococcus aureus. Few cases of Escherichia coli pyomyositis have been reported, including only 1 involving a patient with a hematologic malignancy.
METHODS: The clinical microbiology database at The M. D. Anderson Cancer Center (Houston, TX) was reviewed for the period January 2003 through December 2007 to identify cases of E. coli pyomyositis. Clinical characteristics, laboratory and radiologic findings, treatment, and outcomes were recorded. Available isolates underwent phylogenetic group determination, v

Necrotising fasciitis (NF) is a severe, rapidly progressing infection of the muscular fascia and subcutaneous tissue. Monomicrobial NF from hypermucoviscous Klebsiella pneumoniae (hvKP-NF) is rare but highly virulent, most common in East Asia. This report reviews a unique case of hvKP-NF in a US male in his 40s with undiagnosed diabetes mellitus, presenting with severe sepsis, neck swelling and calf pain. Imaging revealed pyomyositis in the sternocleidomastoid, and debridement cultures confirmed hvKP. Despite unremarkable examination findings, persistent pain led to a diagnosis of necrotising

BACKGROUND: Pyomyositis generally occurs in otherwise healthy young men. Because this condition is unusual among otherwise healthy women in temperate climates, we present the following case.
CASE SUMMARY: An otherwise healthy 43-year-old woman presented with bilateral pain in her lower extremities and fever. Magnetic resonance imaging (MRI) findings were indicative of myositis with a possible abscess. We initiated empirical antibiotic therapy with ceftriaxone. However, the swelling and pain in her legs persisted even after 7 d of treatment. Contrast MRI revealed multiple pockets of pus in the

Myositis tropicans or pyomyositis is a muscle inflammation resulting from a bacterial infection of skeletal muscle (commonly caused by Staphylococcus aureus) that usually leads to hematogenous muscle seeding. The present study was designed to estimate the role of ZnO-NPs and a physiotherapeutic program in the management of induced biceps femoris atrophy in rats through histological, biochemical, and radiological examinations at different time intervals. At the beginning, several bacterial strains were evaluated through a proteolytic enzyme activity assay and the highest activity was recorded w

Primary pyomyositis is a rare bacterial infection of skeletal muscle occurring without a contiguous source of infection. Involvement of the sternocleidomastoid muscle (SCM) is exceptionally uncommon in temperate regions and may result in serious complications if diagnosis is delayed. We report a case of primary pyomyositis of the left SCM in a previously healthy 19-year-old male who presented with rapidly progressive neck pain and swelling nine days after minor blunt neck trauma. Contrast-enhanced computed tomography demonstrated an intramuscular SCM abscess with marked muscle oedema. Due to w
