UNLABELLED: Fractures of the distal radius are common injuries. Low-energy or high-energy mechanisms may be involved. Unstable distal radius fractures present a challenge to the treating orthopaedic surgeon. External fixation is a valuable instrument for fracture reduction and stabilization. Limited open incisions, early range of motion, and treatment of complex wounds are a few of the benefits of external fixation. Fixators may be spanning or nonbridging and may be used alone or in combination with other stabilization methods to obtain and maintain distal radius fracture reduction. Augmentati

Volar fixed angle fixation may be considered as the beginning of a new era in restoring wrist function to patients with dorsally displaced distal radius fractures even in the face of comminuted or osteopenic bone. A thorough understanding of the anatomy of the wrist is a prerequisite when volarly approaching dorsally displaced distal radius fractures. The demonstration of the device theoretical and practical advantages requires an appreciation of the basics of working length, principles of plate stability, and the effect of cantilever bending. Volar fixed angle fixation successfully improves w

Internal distraction plating can be used for the treatment of highly comminuted distal radius fractures especially in elderly patients. The technique involves the use of 3.5, 2.7, or 2.5 dynamic compression plates. The instrumentation is applied in distraction dorsally from the radial diaphysis, bypassing the comminuted segment, and fixed distally to the long metacarpal. The advantages of this technique are: a) it can be used as an alternative for managing difficult fractures in the elderly population; b) it is indicated in patients with osteoporotic bone; c) complications associated with exte

Centralization for radius dysplasia purportedly offers a more normal appearance, provides length to a shortened forearm, and improves upper-extremity function. Limited objective outcome data, however, exist to substantiate its use. To better define functional status after centralization, the Jebsen-Taylor hand test and the Disabilities of the Arm, Shoulder, and Hand questionnaire (DASH) were administered to 21 patients (25 wrists) at an average of 20 years after surgery. Jebsen-Taylor scores, a measure of hand function, were significantly altered with an average total score of 48 seconds compa

We report a case of thrombocytopenia absent radius (TAR) syndrome with varus and flexion deformity and limited movement of the knee. Soft tissue release achieved only partial correction of the deformity and tended to convert a stiff knee to an unstable knee. Flexion deformity required correction by lower femoral extension osteotomy.

Thirteen patients with neglected luxation of the proximal radius who were treated with different surgical procedures at the Orthopaedic Department of the Kantonsspital in St. Gallen (Switzerland) returned for evaluation at an average of 7 years postoperatively. The luxation was traumatic in 11 cases (7 Monteggia-fractures) and in 2 cases due to a dysplasia of the forearm. In 7 patients the deformity recurred and in 1 a synostosis between radius and ulna was noticed. In 4 cases the wrist showed a relative overlength of the ulna. Proximal osteotomy of the ulna in combination with open reduction

Treatment results and complications of 404 percutaneous drill wire fixations in patients with dislocated radial fracture of typical situation will be reported. The most frequent complications were secondary impression of fracture, transient paraesthesia as well as perforation through the skin by drill wire. There was no case of osteitis or pseudo-arthrosis. Treatment results were judged excellent or good in 84% of cases, satisfactory in 11% and poor in 5%. For dislocated wrist fractures and for elderly patients osteosynthesis with percutaneous drill wire according to Willenegger is superior to

PURPOSE: Direct articular surface visualization may improve reduction accuracy in intra-articular distal radius fractures. A ligament-sparing volar radiocarpal arthrotomy has been described to complement the standard volar approach, but the articular surface area (SA) visualized has not been compared to the traditional dorsal radiocarpal arthrotomy. This study quantified and compared articular SA visualization through volar and dorsal ligament-sparing radiocarpal arthrotomies in a cadaveric model.
METHODS: Twenty-four fresh-frozen cadaveric specimens (12 matched pairs) underwent either a vola

PURPOSE: Postoperative radiocarpal traumatic arthritis (TA) after distal radius fracture (DRF) may cause persistent pain and functional impairment. Early risk stratification may help identify patients at increased risk. This study aims to identify independent predictors of radiocarpal TA after DRF and develop multifactorial predictive models. METHODS: We retrospectively analyzed 226 DRF patients grouped by radiocarpal TA status. Variables were screened using univariate and correlation analyses. Logistic regression (LR) and support vector machine (SVM) models were developed using a 70/30 strati

Distal radius fractures (DRFs) are one of the most common orthopaedic injuries seen in the Accident and Emergency (A&E) Department, accounting for 17% of all fractures. They are associated with significant morbidity, with substantial economic and societal costs due to the high incidence of these injuries. Initial management of displaced fractures consists of prompt reduction of the fracture in A&E and application of a moulded plaster cast to hold the fracture. However, there is currently no consensus on the best anaesthetic to use during the manipulation procedure. The most frequently used are

Distal radius fractures are one of the most common fractures with a growing incidence in the active and aging population. The classification of these fractures is complex due to continued improvements in our understanding of fracture patterns, their injury mechanisms, and resultant patterns of instability. This complexity has led to multiple classification systems that have not been updated with contemporary knowledge on distal radius fracture patterns and their respective treatments. We describe historical classification systems, their strengths, and limitations, and discuss gaps that may be

Background Our primary aim was to identify risk factors for subsidence development in nonoperatively treated distal radius fractures (DRFs). Secondary aims of the study included comparisons of subsidence progression over time with respect to DRF radiographic parameters. Materials and Methods A retrospective cohort study of 70 patients with DRFs treated nonoperatively and followed for a minimum of 12 weeks was performed. Of the 70 patients, 29 had subsidence and 41 did not have subsidence. Radiographic measurements assessed the radial height (RH), ulnar variance (UV), volar tilt (VT), and rad

BACKGROUND: Prolonged opioid use (more than 90 days) after injury puts the patient at risk for adverse effects. We investigated the patterns of opioid prescription after distal radius fracture and the effect of pre- and post-fracture factors on the risk for prolonged use.
METHODS: This register-based cohort study uses routinely collected health care data, including purchases of prescription opioids, in the county of Skåne, Sweden. 9369 adult patients with a radius fracture diagnosed 2015-2018 were followed for 1 year after fracture. We calculated proportions of patients with prolonged opioid

In the recent years, treatment of distal radius fractures (DRF) has advanced considerably. Surgical fixation with palmar angular stable plate has gained popularity, due to a reported lower complication rate when compared to dorsal fixation. The type of trauma or injury, surgical procedure and impaired bone quality are all contributors to complications in DRF. The main aim of this review is to summarize the most common complications and possible therapeutic solutions. In addition, strategies for minimizing these complications will be discussed.

Posttraumatic malunion or secondary dislocation can cause wrist joint incongruency. Uncorrected malalignment increases the risk of secondary degenerative changes and chronic pain. Therefore, early correction using the available fixed-angle devices, cancellous bone grafting only becomes necessary in larger bony defects. Premounting the plate through a palmar approach with regard to the desired correction angles leads to predictable results by precise correction. In case of posttraumatic growth arrest with larger discrepancy of the radius and the ulna, a two-staged procedure is advisable.

Carpal tunnel syndrome (CTS) after distal radius fractures can present in 3 forms: acute, transient, and delayed. Acute CTS requires an emergent carpal tunnel release. Many patients with transient CTS after distal radius fracture do not require surgical release of the carpal tunnel once the fracture is repaired. Prophylactic carpal tunnel release in the absence of signs and symptoms of CTS after a distal radius fracture is not indicated. For patients with delayed CTS after a distal radius fracture, all possible causes of nerve compression should be considered and addressed in standard fashion.
