Non-inferiority randomized controlled trial comparing CricOid pressure and para-laryngeal pressure in parturients undergoing cesarean delivery: NiCOP trial.

BACKGROUND: Cricoid pressure has been surrounded with controversies regarding its effectiveness. Application of ultrasound-guided para-laryngeal (PL) force has been shown to occlude the esophagus effectively compared with cricoid pressure (CP) in awake patients. We hypothesized that there would be no meaningful difference in the change in antero-posterior esophageal diameter from with application of cricoid or para-laryngeal pressure in parturients undergoing cesarean delivery under general anesthesia. METHODS: In this prospective, randomized, non-inferiority trial, 40 parturients scheduled f

Acute poisoning due to ingestion of Datura stramonium - a case report.

Datura stramonium (DS) is a widespread annual plant, containing atropine, hyoscyamine, and scopolamine, which can produce poisoning with a severe anticholinergic syndrome. Teenagers ingest the roots, seeds or the entire plant to obtain its hallucinogenic and euphoric effects. We presented the case of a 22 year old male who was admitted to the Emergency Room in a coma after consuming Datura stramonium, 2 hours earlier. The patient presented with fever, tachycardia with right bundle branch block, and urinary retention. Rapid sequence induction and intubation was performed immediately, with sedat

Neuro-anesthesiology in pregnancy.

Management of the pregnant patient requiring neurosurgery poses multiple challenges, juxtaposing pregnancy-specific considerations with that accompanying the safe provision of intracranial or spine surgery. There are no specific evidence-based recommendations, and case-by-case interdisciplinary discussions will guide informed decision-making about the timing of delivery vis-à-vis neurosurgery, the performance of cesarean delivery immediately before neurosurgery, consequences of neurosurgery on subsequent delivery, or even the optimal anesthetic modality for neurosurgery and/or cesarean deliver

Procedural Challenges During Intubation in Patients With Oropharyngeal Masses: A Prospective Observational Study.

BACKGROUND: In patients who undergo surgery for oropharyngeal masses, intubation is almost always successful. However, technical aspects of airway management, including bag mask ventilation and oxygenation, may still be difficult. Although rates of airway difficulty and intubation success in these patients have been studied, these data may not reflect difficulty with individual components of the intubation process. We hypothesized that rates of complications with individual elements of the intubation process would not be reflected in the rate of eventual intubation success. To test our hypothe

Impact of paralytic choice on postintubation sedation and analgesia in the emergency department.

PURPOSE: We describe the timing of first-dose sedation and analgesia after rapid sequence intubation (RSI) in patients induced with etomidate and paralyzed with rocuronium or succinylcholine. METHODS: This was a retrospective study of adult patients undergoing RSI in 3 emergency departments (EDs). We evaluated the time to administration of analgesia and sedation using a Cox proportional hazard model controlling for choice of paralytic, post-RSI hypotension (nadir systolic blood pressure of less than 100 mm Hg in the first hour), bedside presence of an ED pharmacist, and practice site. We also

Anesthesia for Cesarean section in a patient with paraplegia resulting from tumour metastases to spinal cord.

PURPOSE: Spinal cord injured patients present multiple unique challenges to the anesthesiologist. These include choice of muscle relaxant and management of autonomic hyperreflexia. We report the anesthetic management for Cesarean delivery in a patient who was paraplegic due to spinal canal metastases. Preeclampsia and fever complicated this case. CLINICAL FEATURES: The patient presented at 29 wk gestation with progressive paraplegia at the T10 level due to metastatic osteosarcoma. She had a decompressive laminectomy without improvement in her paralysis. She subsequently developed preeclampsia

The "BURP" maneuver worsens the glottic view when applied in combination with cricoid pressure.

PURPOSE: The purpose of this study was to determine if the application of a BURP maneuver to the cricoid cartilage would combine the benefits of both the BURP and the Sellick maneuvers, resulting in an improved glottic view and offer the potential of protection against passive gastric regurgitation. METHODS: This was a double-blind, prospective, randomized, crossover trial. Forty-three patients scheduled for elective surgery participated in this study. General anesthesia was induced using fentanyl, propofol and rocuronium. In a random sequence for each case and blinded to the laryngoscopist,

Resolution of status epilepticus after ketamine administration.

BACKGROUND: Status Epilepticus is the most common non-traumatic neurologic emergency in childhood. Current algorithms prioritize the use of benzodiazepines as first line treatment followed by Levetiracetam or Valproic Acid, possibly Fosphenytoin and eventually high dose Propofol and intubation. CASE REPORT: A 9-month old girl was brought to the emergency department with a continuous seizure involving the right upper and lower extremity for 45 min prior to arrival. Patient received a dose of rectal Diazepam, intramuscular Midazolam, 2 doses of Lorazepam, Levetiracetam, Fosphenytoin and 2 addit

Alfentanil versus fentanyl for emergency department rapid sequence induction with ketamine: A-FAKT, a pilot randomized trial.

BACKGROUND: Fentanyl is often administered during rapid sequence induction of anesthesia (RSI) in the emergency department (ED) to ameliorate the hypertensive response that may occur. Due to its more rapid onset, the use of alfentanil may be more consistent with both the onset time of the sedative and the commencement of laryngoscopy. As such, we compared the effect of alfentanil and fentanyl on post-induction hemodynamic changes when administered as part of a standardized induction regimen including ketamine and rocuronium in ED RSI. METHODS: This was a double-blind pilot randomized controll

Use of the intubating laryngeal mask airway in emergency pre-hospital difficult intubation.

AIM OF THE STUDY: While several techniques are used for the management of difficult intubation (DI) in planned conditions in the operating theatre, they are not always suitable or usable in pre-hospital emergencies. We decided to assess the intubating laryngeal mask airway (ILMA) after failure of tracheal intubation (TI) under direct laryngoscopy. MATERIAL AND METHODS: After emergency physicians of the mobile intensive care unit were trained (theory and training on manikin) in using the ILMA (Fastrach), prospective data were collected after each use from March 2002 to December 2005. Data incl

Airway management practices among emergency physicians: An observational study.

OBJECTIVES: Emergency airway management is an integral part of patient stabilization. It is an essential skill for an emergency physician to master. There is a paucity of literature on airway management from low-to-middle-income countries like India where emergency medicine (EM) specialty is still in its infancy. We conducted this study to identify the existing airway management practices among emergency physicians in our tertiary care center. This study could pave the way for national airway registries. METHODS: This prospective, observational study was conducted in the emergency department

Standard gastroscope-guided endotracheal intubation in the lateral position for life-threatening variceal bleeding: a report of technical feasibility and airway safety.

BACKGROUND: Patients with acute upper gastrointestinal hemorrhage (UGIH) often present with airway contamination due to hematemesis and gastric reflux. Conventional endotracheal intubation (ETI) in the supine position faces challenges such as limited visualization and high aspiration risk. While securing the airway and controlling bleeding are both critical during resuscitation, simultaneous achievement of these goals remains technically difficult. CASE PRESENTATION: A 56-year-old woman with esophageal variceal rupture presented with hemorrhagic shock. After left-lateral positioning and rapid

Physiologically Difficult Airways in Emergency Medicine: A Narrative Review of Recognition, Resuscitation, and Management Strategies.

The focus of managing critically ill patients' airways has traditionally centered around anatomically complicated issues, however an increasing amount of literature indicates that peri-intubation complications are likely to be related to the patient's physiologic state at the time of intubation. Physiologic instability may lead to severe hypoxemia, cardiovascular collapse, or cardiac arrest even when the anatomy of the airway is uncomplicated. Shock states, severe metabolic acidosis, right heart failure, or an insufficient oxygen reserve may contribute to these unstable conditions but they oft

Development and Testing of a Novel Anaesthesia Induction/Ventilation Protocol for Patients With Cardiogenic Shock Complicating Acute Myocardial Infarction.

BACKGROUND: Cardiogenic shock is a life-threatening condition and patients might require rapid sequence induction (RSI) and mechanical ventilation. In this study, we evaluated a new RSI/mechanical ventilation protocol in patients with acute myocardial infarction complicated by cardiogenic shock. METHODS: We included consecutive adult patients who were transferred to the emergency department. The RSI protocol included 5 phases: preoxygenation, pretreatment, induction/paralysis, intubation, and mechanical ventilation (PPIIM). A posteriori, we selected historical patients managed with standard R
