[Hypotension after catheter ablation].

A 80-year-old female patient with symptomatic atrial tachycardia develops cold sweat and hypotension in the recovery room after pulmonary vein isolation. The procedure included an anterior line and an endo- and epicardial mitral isthmus line. Echocardiography excluded a hemodynamically relevant pericardial effusion. The patient's symptoms deteriorated over time. A rare cause was found.

General Anesthesia vs Monitored Anesthesia Care With Local Anesthetic Infiltration (MAC-VL) for Ankle Fracture Open Reduction and Internal Fixation: Impact on Recovery Metrics and Safety.

BACKGROUND: Monitored anesthesia care (MAC) combined with regional anesthetic techniques is commonly used for open reduction and internal fixation (ORIF) of unstable ankle fractures. When compared to general anesthesia (GA), MAC with regional anesthesia has been associated with reduced systemic adverse effects and shorter recovery times, although regional anesthesia carries additional costs and potential complications of its own. A new technique involving the infiltration of high-volume, low-concentration (VL) local anesthetic was therefore developed. When used in conjunction with MAC, we refe

Auriculotherapy for Prolonged Postoperative Pain Management Following Rotator Cuff Surgery: A Randomized, Placebo-Controlled Study.

INTRODUCTION: Pain after rotator cuff surgery is prolonged. This has led to the need for opioids and the risk of developing opioid use disorder. This study was designed to investigate the role that active auriculotherapy treatment (AT) may play in reducing opioid consumption following rotator cuff surgery. METHODS: Fifty patients who underwent a primary ambulatory unilateral rotator cuff surgery participated in this randomized, placebo-controlled study. Each patient was randomized to either the AT group or the placebo group (P group). The primary endpoint was overall opioid consumption (oral

Detection of Saccharopolyspora rectivirgula by quantitative real-time PCR.

The thermophilic actinomycete species Saccharopolyspora rectivirgula has been associated with the exogen allergic alveolitis (EAA). EAA is caused by the inhalation of high amounts of airborne spores that can be found for example in environments of agricultural production, compost facilities, mushroom cultivation rooms, or rooms with technical air moistening. Because of the medical relevance of S. rectivirgula, a reliable detection system is needed. Therefore, a quantitative real-time polymerase chain reaction (qPCR) primer system was designed, targeting the 16S rRNA gene of the type strain S.

The effects of acute intermittent hypercapnia on cardiovascular and ventilatory control.

Acute intermittent blood-gas perturbations elicit cardiorespiratory neuroplasticity, with intermittent hypercapnic hypoxia resulting in well-characterized and persistent increases in ventilation, sympathetic activity, and blood pressure. However, the effects of intermittent hypercapnia (IHc) independent of hypoxia remain unknown in humans. We investigated the long-lasting effects of acute IHc on ventilation, blood pressure, and limb vascular conductance. Twenty-four healthy participants (age: 23 ± 3 yr; body mass index: 22 ± 2 kg/m2) underwent a 10-min baseline, 40 min of either IHc (n = 17; 6

The effect of nasal midazolam premedication on parents-child separation and recovery time in dental procedures under general anaesthesia.

AIM: For many children medical and dental procedures, unfamiliar dental staff and treatment places are disturbing and stressful. Stress in children often makes them uncooperative. General anaesthesia is indicated for anxious uncooperative children or those who are disabled, immature or too young to undergo dental treatment by other means. Moreover parents' separation while entering the operative room is a traumatic experience for children. Thus premedication such as midazolam is recommended to decrease child's stress. In these situations the increased recovery time was considered as one of the

Impact of Sugammadex Versus Neostigmine/Glycopyrrolate on Perioperative Efficiency.

PURPOSE: Neuromuscular blockade in the operating room necessitates the utilization of reversal agents to accelerate postoperative recovery and sustain operating room patient throughput. Cholinesterase inhibitors represent the historical standard of care for neuromuscular blockade reversal within anesthesia practice. Sugammadex, a synthetic gamma-cyclodextrin, was introduced to the market with evidence of more rapid and predictable reversal of neuromuscular blockade compared to alternative agents. Higher medication acquisition costs have limited more extensive use of sugammadex compared to that

The scapular manipulation method for reducing anterior shoulder dislocations.

Anterior dislocation of the shoulder is the most common dislocation encountered by the emergency room medical officer. Many of the present methods employed, such as Kocher's method, the Hippocratic method and distraction method, necessitate the use of both intravenous pethidine and midazolam, which prolongs the recovery time of the patient in the emergency room, as well as carrying the risk of respiratory depression. Also, such methods are associated with complications such as fractures of the humerus and injury to the axillary vessels and brachial plexus. In view of the above disadvantages, a

Synergistic Effect of MOF-Derived Porous In2O3 and High-Entropy Alloy for Enhanced Room-Temperature Ammonia Sensing.

High-performance room-temperature ammonia (NH3) sensors are highly desirable for environmental monitoring and noninvasive chronic kidney disease diagnosis via breath analysis. Structurally advantageous, metal-organic framework (MOF)-derived metal oxides combined with low-cost, highly catalytic, nonprecious high-entropy alloy (HEA) nanoparticles show great potential for room-temperature gas sensing, yet such composites are rarely explored. Herein, FeCoNiMnCu HEA was rationally coupled with MOF-derived porous In2O3 to fabricate a HEA-modified In2O3 (HEA-IO) composite for room-temperature NH3 sen

Postoperative urinary retention: Part II--A retrospective study.

Postoperative urinary retention, a potential postoperative complication, can lead to discomfort and contribute to atony of bladder wall, need for catheterization and, sometimes, prolonged hospital stays. A retrospective study was conducted to analyze variables related to postoperative urinary retention. Data was collected by a chart review of 100 postoperative cases in a large Veterans Administration Medical Center. All patients were male, had a mean age of 61.1 years, and had received spinal or general anesthesia. Patients with known urologic problems, paraplegic patients, quadruplegic patien

Thermal Recovery of Colloidal Quantum Dot Ensembles Following Photoinduced Dimming.

Colloidal CdSe quantum dot (QD) core ensembles were photodimmed and allowed to recover in the dark using ambient thermal energy at a range of temperatures. Nonlinear thermal recovery is well described by a stretched exponential function, and further analysis yields an underlying probability distribution of rate constants. Casting the rate constants as a collection of first-order activated processes provides an activation barrier probability distribution with significant density at room-temperature thermal energy that peaks at 200 meV before decaying to zero. This treatment for the recovery tra

Maternal temperature in emergency caesarean section (MATES): an observational multicentre study.

BACKGROUND: Temperature regulation in women undergoing emergency caesarean section is a complex topic about which there is a paucity of evidence-based recommendations. The adverse effects of inadvertent peri-operative hypothermia are well described. Hyperthermia is also associated with adverse neonatal outcomes, an increased risk of obstetric intervention and increased treatment for suspected sepsis. We conducted a multi-centre observational cohort study to identify the prevalence of hypothermia and hyperthermia during emergency caesarean section. S: Participants undergoing emergency caesarean
