Paediatric head and neck cancers in Nigeria: Implications for treatment planning in resource limited settings.

BACKGROUND: The head and neck (H/N) is a common site for childhood cancers. This study examined all cases of H/N childhood cancers diagnosed in a major teaching hospital in Nigeria over 18 years to determine patterns of broad lineage cancer groups. MATERIALS AND METHODS: Primary pediatric childhood malignancies diagnosed between 1990 and 2008 were analysed. Logistic regression models were fitted to determine significant clinical correlates of childhood cancer. RESULTS: Lymphomas were the commonest cancers (49.5%). After controlling for site and age, there was no significant difference in the

Safety, efficacy, and pharmacokinetics of rilpivirine: systematic review with an emphasis on resource-limited settings.

The vast majority of people living with human immunodeficiency virus (HIV)/acquired immune deficiency syndrome reside in the developing world, in settings characterized by limited health budgets, critical shortages of doctors, limited laboratory monitoring, a substantial burden of HIV in children, and high rates of coinfection, in particular tuberculosis. Therefore, the extent to which new antiretrovirals will contribute to improvements in the management of HIV globally will depend to a large extent on their affordability, ease of use, low toxicity profile, availability as pediatric formulatio

Achieving excellence in paediatric cardiac care in resource limited and resource plentiful settings and building successful care networks across different countries.

BACKGROUND: The delivery of paediatric cardiac care across the world occurs in settings with significant variability in available resources. Irrespective of the resources locally available, we must always strive to improve the quality of care we provide to our patients and simultaneously deliver such care in the most efficient and cost-effective manner. The development of cardiac networks is used widely to achieve these aims. METHODS: This paper reports three talks presented during the 56th meeting of the Association for European Paediatric and Congenital Cardiology held in Dublin in April 20

The application of focused assessment with sonography for trauma in resource-limited settings: a scoping review.

BACKGROUND: Trauma represents a leading cause of mortality and disability in resource-limited settings. However, access to advanced imaging modalities is severely constrained, limiting timely and accurate diagnosis as well as clinical decision-making. Focused assessment with sonography for trauma (FAST) offers a portable, non-ionizing, and cost-effective bedside imaging solution with significant potential for clinical application. It enables rapid identification of life-threatening hemoperitoneum or hemothorax, thereby guiding immediate surgical intervention or preventing unnecessary patient t

Best practices in sample management & pre-analytical quality control: overcoming challenges in resource-limited laboratory settings.

BACKGROUND: Pre-analytical errors remain the leading source of laboratory diagnostic failures worldwide, accounting for approximately 60-75% of errors across the total testing process. These challenges are particularly pronounced in resource-limited settings (RLS), where inadequate infrastructure, unreliable transport systems, workforce shortages, and weak quality management systems compromise specimen integrity and diagnostic accuracy. Despite growing recognition of these barriers, evidence on practical, context-appropriate interventions remains disjointed. OBJECTIVES: This study synthesizes

Diagnostic approach to episodic ataxia types 1 and 2: a proposed algorithm for limited resource-settings.

BACKGROUND: Episodic ataxias (EA) comprise a heterogeneous group of genetic conditions with spells of gait difficulty and imbalance, for which the main causes are EA1 (KCNA1 gene) and EA2 (CACNA1A gene). While EA1 may respond to some antiepileptics and EA2 responds to acetazolamide, no guideline exists to inform decision-making in settings where genetic testing is unavailable. OBJECTIVES: We sought to determine distinguishing clinical features between EA1 and EA2 and propose an algorithm based on our findings. METHODS: Systematized literature review to identify individuals with confirmed pat

Development of a low-cost external fixation system: an off-label solution for resource-limited settings.

PURPOSE: External fixators are essential for the surgical management of long-bone fractures, particularly in resource-limited settings where access to commercial systems is often restricted by cost and logistics. This study aimed to develop a low-cost external fixation concept using widely available construction materials and evaluate its biomechanical performance under axial loading. METHODS: An external fixator composed of M10 threaded rods, washers, and nuts was developed. Five configurations were tested: SP (Steinmann pins), KT (threaded K-wires), KS (smooth K-wires), and BKS (bilaterally

TB LAMP assay, a beneficial tool for the diagnosis of Tubercular meningitis in resource-limited settings.

INTRODUCTION: Tubercular meningitis (TBM) is a serious public health problem in developing countries as it leads to significant mortality and residual neurological sequelae. The estimated mortality due to TBM in India is 1.5 per 100,000 population. In resource-limited settings, only the Ziehl-Neelsen (ZN) stain, which has very little sensitivity, is available. The World Health Organization recommended the Loop Mediated Isothermal Amplification (TB LAMP) assay for pulmonary tuberculosis only. We evaluated this test for tubercular meningitis as well. METHODOLOGY: In a cross-sectional study of 2

A single low-dose rituximab infusion in severe chronic refractory myasthenia gravis in resource-limited settings.

The benefits of multi-dose rituximab cycles in patients with refractory anti-muscle-specific kinase antibody myasthenia gravis (MuSK+MG) are well reported, although less consistently in anti-acetylcholine receptor antibody MG (AChR+MG). Responsivity data to single low-dose rituximab infusions for refractory autoimmune myasthenia, are limited. Here, observational outcomes using MG grading scores and prednisone doses, before and after at least six months of a single-dose infusion of rituximab, were audited in previously treatment-refractory MG patients in a resource-limited setting.
