Central regulation of heart rate and the appearance of respiratory sinus arrhythmia: new insights from mathematical modeling.

A minimal model for the neural control of heart rate (HR) has been developed with the aim of better understanding respiratory sinus arrhythmia (RSA)--a modulation of HR at the frequency of breathing. This model consists of two differential equations and is integrated into a previously-published model of gas exchange. The heart period is assumed to be affected primarily by the parasympathetic signal, with the sympathetic signal taken as a parameter in the model. We include the baroreflex, mechanical stretch-receptor feedback from the lungs, and central modulation of the cardiac vagal tone by th

Serum procalcitonin level and leukocyte antisedimentation rate as early predictors of respiratory dysfunction after oesophageal tumour resection.

INTRODUCTION: Postoperative care after oesophageal tumour resection holds a high risk of respiratory complications. We therefore aimed to determine the value of systemic inflammatory markers in predicting arterial hypoxaemia as the earliest sign of developing lung injury after oesophageal tumour resection. METHODS: In a prospective observational study, 33 consecutive patients were observed for three days (T1-T3) after admission (T0) to an intensive care unit following oesophageal tumour resection. The daily highest values of the heart rate, axillary temperature, leukocyte count and PaCO2 were

Respiratory sinus arrhythmia during anaesthesia: assessment of respiration related beat-to-beat heart rate variability analysis methods.

Beat-to-beat heart rate variability analysis is a powerful tool for the diagnosis of neuropathy. Respiration-related heart rate variability (respiratory sinus arrhythmia, RSA) reflects the function of parasympathetic nervous system during spontaneous ventilation while awake. RSA is also claimed to monitor the depth of anaesthesia. Power spectrum analysis or various averaging techniques of the heart rate variability are usually applied. The current literature, however, does not usually interpret the ground rules and limitations of the method used, and this may sometimes lead to erroneous conclu

Effect of respiratory rate and size of cannula on pressure and dead-space clearance during nasal high flow in patients with COPD and acute respiratory failure.

Nasal high flow (NHF) is an efficient oxygenation tool for the treatment of respiratory failure. The study investigated the effect of breathing pattern on positive airway pressure and dead-space clearance by NHF. The breathing cycle during NHF was characterized in 26 patients with acute respiratory failure (ARF) and stable COPD and after mechanical ventilation (post-MV) via tracheostomy where also pressure was measured in the trachea. Dead-space clearance was measured in airway models during different breathing patterns. NHF reduced the respiratory rate (RR) and TI/TE through prolonging the TE

Comparison of ROX index (SpO2/FIO2 ratio/respiratory rate) with a modified dynamic index incorporating PaO2/FIO2 ratio and heart rate to predict high flow nasal cannula outcomes among patients with acute respiratory failure: a single centre retrospective study.

BACKGROUND: High flow nasal cannula (HFNC) is increasingly being used to support patients with acute respiratory failure (ARF) and to avoid need for intubation. However, almost one third of the patients do not respond and require escalation of respiratory support. Previously, ROX index (SpO2/FIO2 [SF] ratio/respiratory rate) has been validated among pneumonia patients to facilitate early recognition of patients likely to fail HFNC and therefore, benefit from timely interventions. However, it has been postulated that incorporation of PaO2/FIO2 (PF) ratio from arterial blood gas (ABG) analysis m

Application of 24-h respiratory rate and oxygenation index variation to predict the outcome of high-flow nasal cannula therapy in patients with acute hypoxemic respiratory failure in a respiratory intensive care unit.

Background: An evaluation system is urgently needed to describe early predictors of the outcome of high-flow nasal cannula (HFNC) oxygen therapy in acute hypoxemic respiratory failure (AHRF) patients. Methods: All consecutive AHRF patients in a Respiratory Intensive Care Unit (RICU) receiving HFNC therapy between January 2019 and December 2021 were enrolled. Results: Of the 106 enrolled AHRF subjects, 57 (53.8%) succeeded in HFNC therapy and 49 (46.2%) failed. Being male (p = 0.006), initial respiratory rate oxygenation (ROX) index (p = 0.011), Acute Physiology and Chronic Health Evaluation II

Association of ventilation volumes, pressures and rates with the mechanical power of ventilation in patients without acute respiratory distress syndrome: exploring the impact of rate reduction.

INTRODUCTION: High mechanical power is associated with mortality in patients who are critically ill and require invasive ventilation. It remains uncertain which components of mechanical power - volume, pressure or rate - increase mechanical power the most. METHODS: We conducted a post hoc analysis of a database containing individual patient data from three randomised clinical trials of ventilation in patients without acute respiratory distress syndrome. The primary endpoint was mechanical power. We used linear regression; double stratification to create subgroups of participants; and mediatio

Respiratory Syncytial Virus Exceeded SARS-CoV-2 and Influenza in Lower Respiratory Infection and Mortality Rates Among Patients With Hematologic Malignancies During the 2023-2024 Respiratory Virus Season.

BACKGROUND: Respiratory viral infections (RVIs) significantly impact patients with hematologic malignancies (HMs). During the 2023-2024 respiratory viral (RV) season, we observed a decline in SARS-CoV-2-related hospitalizations in our center compared to the two previous seasons. Given the changing epidemiology of RVIs in the post-pandemic era, the low acceptance of SARS-CoV-2 and influenza vaccination, and the availability of new respiratory syncytial virus (RSV) vaccines in 2023, we aimed to compare outcomes of RSV, influenza, and SARS-CoV-2 infections in patients with HMs during the 2023-202

Rates of rewarming, heart and respiratory rates and their significance for oxygen transport during arousal from torpor in the smallest mammal, the Etruscan shrew Suncus etruscus.

We investigated the process of rewarming from torpor with respect to respiratory and circulatory oxygen transport properties in the smallest mammal, the Etruscan shrew Suncus etruscus. In seven adult Etruscan shrews with a mean body mass of 2.4g, torpor was induced by deprivation of food and a cold environment. During arousal from torpor at an ambient temperature of 22 degrees C, the shrews actively rewarmed from the lowest mean (+/- S.D.) body temperature (Tb) of 12.1 +/- 1.2 degrees C to 20 degrees C at a rate of 0.43 +/- 0.14 degree C min-1, from 20 to 24 degrees C at a rate of 0.8 degree C

For a clinical application of optical triangulation to assess respiratory rate using an RGB camera and a line laser.

This paper presents a non-contact and unrestrained respiration monitoring system based on the optical triangulation technique. The proposed system consists of a red-green-blue (RGB) camera and a line laser installed to face the frontal thorax of a human body. The underlying idea of the work is that the camera and line laser are mounted in opposite directions, unlike other research. By applying the proposed image processing algorithm to the camera image, laser coordinates are extracted and converted to world coordinates using the optical triangulation method. These converted world coordinates r

The surge in heart rate and blood pressure at respiratory event termination is dampened in children with down syndrome.

BACKGROUND: Children with Down syndrome (DS) have a high prevalence of sleep disordered breathing (SDB) and altered cardiovascular autonomic control. We aimed to analyze the effect of DS on the surge in heart rate (HR) and pulse transit time (PTT, an inverse surrogate measure of blood pressure change) at respiratory event termination. METHODS: 44 children (3-19 y) with DS and 44 typically developing (TD) children matched for SDB severity, age and sex underwent overnight polysomnography. Multilevel modelling determined the effect of DS on HR and PTT changes between a 10s pre-event to the latte

Improved survival rates in patients with H1N1 acute respiratory failure in Korea between 2009 and 2016.

There was a pandemic of influenza A (H1N1) in 2009; in Korea, there was also an H1N1 epidemic in 2016. We aim to investigate whether survival had improved in the setting of recent advances in intensive care unit (ICU) management. We conducted a retrospective analysis of acute respiratory failure patients with H1N1 influenza pneumonia in 2016 and 2009 respectively at two tertiary referral hospitals in Korea. A total of 28 patients were treated in 2016, and 34 in 2009. There was no significant difference in SOFA scores on ICU admission day. In-hospital mortality was significantly lower in patien

Rates of respiratory virus-associated hospitalization in children aged <5 years in rural northern India.

OBJECTIVES: Though respiratory viruses are thought to cause substantial morbidity globally in children aged <5 years, the incidence of severe respiratory virus infections in children is unknown in India where 20% of the world's children live. METHODS: During August 2009-July 2011, prospective population-based surveillance was conducted for hospitalizations of children aged <5 years in a rural community in Haryana State. Clinical data and respiratory specimens were collected. Swabs were tested by RT-PCR for influenza and parainfluenza viruses, respiratory syncytial virus (RSV), human metapneum

Low rates of antimicrobial resistance in respiratory pathogens from a pediatric population in north-eastern Italy.

BACKGROUND: Streptococcus pneumoniae, Haemophilus influenzae and Streptococcus pyogenes are the main agents of bacterial upper respiratory tract infections in infants and children. In the past decades, the proportion of patients infected with drug-resistant organisms has increased; surveying at a regional level is important for understanding the size of the problem and elaborating therapeutic guidelines based on local epidemiology. METHODS: Since 1997, all isolates of Streptococcus pneumoniae, Haemophilus influenzae and Streptococcus pyogenes have been surveyed for antimicrobial resistance.

Respiratory-physiology modeling of therapeutic hydrogen inhalation: defining the fraction of inspired hydrogen (FiH2) and flow-rate requirements.

BACKGROUND: Molecular hydrogen (H2 gas) has emerged as a promising therapeutic agent with reported benefits across oxidative stress, inflammation, cardiovascular, and neurodegenerative conditions. Among administration routes, inhalation provides direct systemic delivery but has been hindered by a lack of individualization, methodological inconsistencies, misinterpretation of concentration, and safety concerns. We propose that fraction of inspired hydrogen (FiH2), defined at the airway opening rather than by source gas concentration (FH2) or flow rate, should be considered the standardized metr

Long-wave infrared imaging for respiratory rate measurement in a patient with amyotrophic lateral sclerosis: A case report.

Respiratory monitoring is important in amyotrophic lateral sclerosis patients as progressive respiratory muscle weakness often leads to respiratory failure. We performed single-case feasibility evaluation of long-wave infrared imaging for noncontact respiratory rate estimation in a hospitalized amyotrophic lateral sclerosis patient and compared it with red-green-blue camera-based optical flow analysis and electrocardiogram-derived respiration. Respiratory rate was assessed using four methods: (a) manual counting as the ground truth; (b) long-wave infrared; (c) red-green-blue; and (d) single-le
