We studied the ultrastructure and function of nasal cilia from non-atopic and atopic non-smokers, smokers, patients with chronic rhinitis and cystic fibrosis, and compared these to cilia from patients with primary ciliary dyskinesia (PCD). Atypical cilia were found in normal subjects and patients with a variety of chronic respiratory tract diseases, but the proportion was small (less than 5%). Ciliary wave form, beat frequency, and mucociliary transport were normal. In patients with PCD, the proportion of atypical cilia and ultrastructural abnormalities was significantly greater than normal su

Anthropogenic heat (AH) exerts a substantial influence on the local urban climate and intensifies the urban heat island (UHI) effect. However, the impact of AH exposure on respiratory allergy symptoms in preschool children has not been investigated. This study conducted a cross-sectional study of 39088 Chinese preschool children to investigate the associations between multisource AH exposure (building heat, industrial heat, metabolic heat, transportation heat, and total heat) and the risks of respiratory allergy symptoms (wheezing, night-cough, and rhinitis). Exposure to higher levels of AH wa

OBJECTIVE: Neonatal interfacility transport of critically ill infants with severe respiratory failure is challenging. Although high-frequency ventilation is maintstay in the neonatal intensive care unit, its use during transport has been historically limited by equipment size and complexity. The TXP-2D ventilator delivers high-frequency percussive ventilation, a hybrid modality potentially offering greater stability during transport. This study aimed to evaluate the safety, feasibility, and operational stability of the TXP-2D ventilator during neonatal transport.
METHODS: We conducted a retro

BACKGROUND: Smoking is the principle risk factor for development of chronic obstructive pulmonary disease (COPD). Multidrug resistance-associated protein 1 (MRP1) is known to protect against toxic compounds and oxidative stress, and might play a role in protection against smoke-induced disease progression. We questioned whether MRP1-mediated transport is influenced by pulmonary drugs that are commonly prescribed in COPD.
METHODS: The immortalized human bronchial epithelial cell line 16HBE14o- was used to analyze direct in vitro effects of budesonide, formoterol, ipratropium bromide and N-acet

Acute Respiratory Distress Syndrome (ARDS) is a known and severe complication of thoracic trauma. Many patients, despite appropriate ventilator and medical support, continue to worsen requiring additional cardiopulmonary support with extracorporeal membrane oxygenation (ECMO). Additionally, obesity adds a layer of complexity in the management of trauma ARDS on ECMO. We describe the first U.S. Military air transportation mission via Critical Care Air Transport (CCAT) involving the cannulation and transportation of 2 obese trauma patients requiring ECMO support. We reviewed a cohort of 2 obese p

BACKGROUND: Inhaling 35% carbon dioxide induces an emotional and symptomatic state in humans closely resembling naturally occurring panic attacks, the core symptom of panic disorder. Previous research has suggested a role of the serotonin system in the individual sensitivity to carbon dioxide. In line with this, we previously showed that a variant in the SLC6A4 gene, encoding the serotonin transporter, moderates the fear response to carbon dioxide in humans. To study the etiological basis of carbon dioxide-reactivity and panic attacks in more detail, we recently established a translational mou

Ion transport is essential for maintenance of transmembranous and transcellular electric potential, fluid transport and cellular volume. Disturbance of ion transport has been associated with cellular dysfunction, intra and extracellular edema and abnormalities of epithelial surface liquid volume. There is increasing evidence that conditions characterized by an intense local or systemic inflammatory response are associated with abnormal ion transport. This abnormal ion transport has been involved in the pathogenesis of conditions like hypovolemia due to fluid losses, hyponatremia and hypokalemi

Viral load measurement of Respiratory syncytial virus (RSV) in acute bronchiolitis depends on specimen collection, viral load quantification, and transport media. The aim of this study was to investigate viral load in three-way-comparative analyses; nasal swab versus nasal wash, quantitative real-time polymerase chain reaction (RT-PCR) versus cell tissue culture, and various transport media. A prospective cohort study of infants aged < 12 months, admitted to the Soroka Medical Center, due to acute bronchiolitis, was conducted. Two nasal swabs and two nasal wash samples (in UTM and VCM) were co

Extracorporeal membrane oxygenation (ECMO) support for severe acute respiratory failure has been increasing. Evidence suggests that higher volume centers have better outcomes, leading to a need for specialized ECMO transport teams. The inherent nature of the prehospital environment adds an additional layer of complexity; however, the experience of multiple centers has demonstrated that cannulating and transporting a patient on ECMO can be performed safely. The purpose of this review article is to discuss the state of knowledge with respect to ECMO transport with special emphasis given to how t

BACKGROUND: Chronic exposure to particulate matter (PM) is recognized as a significant contributor to respiratory health complications, including oxidative stress, inflammatory responses, and compromised epithelial barrier function. In this work, we ask whether the transport of potassium and chloride through the large-conductance calcium-activated potassium (BKCa) channel and the cystic fibrosis transmembrane conductance regulator (CFTR) channel may change PM-induced epithelial dysfunction.
METHODS: This study aimed to evaluate the impact of PM on cell variability, ROS level, inflammation, mi

Search for effective method for decreasing the mortality of children with severe thermal injuries and approaches to preventing and treating multiorgan dysfunction in severe thermal injury is a pressing problem of reanimatology. The majority of scientists consider disorders in the ventilation-perfusion function of the lungs the main factor in the pathogenesis of burn shock. Lung injury is most often the first manifestation of multiple organ dysfunction. The depth of disorders in the gas exchange function of the lungs clearly correlates with the severity of burn injury and the terms of the begin

Transport workers face various occupational hazards, however long-term effects on mortality are less understood. Transport for London (TfL) employs almost 30,000 workers across a wide range of transport-based jobs and working environments. This study aimed to characterise mortality among TfL employees, and investigate long-term health outcomes. A retrospective cohort was formed using cause of death data from the TfL pension fund for employees working between 1960 and 2010. Workers were grouped by job title and Cox proportional hazard models were used to assess all-cause, respiratory, cardiovas

We use a recently developed mathematical model that integrates a reduced representation of the brainstem respiratory neural controller together with peripheral gas exchange and transport to study numerically the dynamic response of the respiratory system to several physiological stimuli. We compare between the system responses with two major sources of delay: circulatory transport vs. neural feedback dynamics, and we show that the dynamics of the neural feedback processes dictates the dynamic response to hypoxia and hypercapnia. The source of the circulatory delay (blood velocity vs. distance

Mucosal vaccination represents an attractive strategy for preventing respiratory infections by establishing immune protection at the site of pathogen entry. However, the development of intranasal vaccines remains challenging due to limited epithelial transport, rapid mucociliary clearance, and the tolerogenic nature of the mucosal immune environment. Here, we report glycoengineered chitosan nanoparticles (NPs) functionalized with lactobionic acid, gluconic acid, and mannose as a platform to enhance mucosal vaccine delivery and immune activation. Carbohydrate functionalization preserved the muc

Pulmonary surfactant reduces surface tension at the air-liquid interface in the alveolus, thereby maintaining lung volumes during the respiratory cycle. In premature newborn infants, the lack of surfactant causes atelectasis and respiratory failure, characteristic of respiratory of distress syndrome. Surfactant is comprised of lipids and associated proteins that are required for surfactant function. Surfactant proteins B and C and a lamellar body associated transport protein, ABCA3 play critical roles in surfactant synthesis and function. Mutations in the genes encoding these proteins cause le
