PURPOSE: Anorexia nervosa (AN) may be treated with intravenous hyperalimentation (IVH) that may be associated with catheter-related bloodstream infection (CRBSI).
DESIGN AND METHODS: Retrospective chart review was conducted to compare those who developed CRBSI were compared with those who did not.
FINDINGS: Of 34 patients, 17 episodes of AN treated with IVH were identified, of which five resulted in CRBSI. The average body mass index at admission was low at 12.2. Patients who needed physical restraint during IVH had a higher (albeit statistically nonsignificant) risk. Also, those with purgin

Hyperactive Delirium with Severe Agitation (HDSA) is characterized by disorientation and aggressive behavior, constituting an acute, life-threatening medical emergency that requires prompt intervention. It is frequently associated with substance use. In prehospital settings, management often involves physical restraint, force, or pharmacological sedation; however, the diagnostic criteria and medico-legal implications of HDSA remain insufficiently defined. We retrospectively analyzed seven fatal cases exhibiting HDSA presentations, as defined by the American College of Emergency Physicians (ACE

CONTEXT: Writer's cramp is a kind of focal hand dystonia that appears when individuals are writing. Since pharmacological treatment has not shown the desired therapeutic response, a study on immobilization of the damaged musculature was performed on two individuals with writer's cramp, using splints with the objective of reducing the handwriting abnormalities.
CASE REPORT: Two patients presenting writer's cramp who had previously undergone different therapies, including botulinum toxin, without an adequate response, participated in a body awareness program, followed by immobilization of the h

NARRATIVE REVIEW: The pathophysiology of focal hand dystonia (FHD) has not yet been completely clarified. Although there is a loss of inhibition at multiple levels of the central nervous system, maladaptive plasticity of the cerebral cortex as well as impairments in sensory and motor representations have also been reported. All of these abnormalities can be viewed as an epiphenomenon of the primary--still unknown--abnormality underlying focal dystonia. The purpose of this review is to describe the underlying constructs of novel nonpharmacologic approaches for the treatment of FHD. Alternative

Weakly electric fishes have been an important model system in behavioral neuroscience for more than 40 years. These fishes use a specialized electric organ to produce an electric field that is typically below 1 volt/cm and serves in many behaviors including social communication and prey detection. Electrical behaviors are easy to study because inexpensive and widely available tools enable continuous monitoring of the electric field of individual or groups of interacting fish. Weakly electric fish have been routinely used in tightly controlled neurophysiological experiments in which the animal

Restraint is an extreme response to an emergency situation in which there is imminent harm to self or others. Although some restrained patients become calm, others may become even more behaviorally disorganized. In some of these latter cases, the restrained patients may be victims of violence for whom the restraint procedure acts as a symbolic reminder of the past victimization. Elderly dementia sufferers may also be among these victims of violence. This paper provides a brief review of psychological trauma, research findings on psychological trauma in the elderly, and an approach to modifying

The use of the hogtie restraint (also known as hobble or prone maximal restraint) by law enforcement and prehospital personnel has come under scrutiny because of reports of sudden deaths in persons placed in this restraint position. Some contend that this body position restricts chest and abdominal movement to the point that individuals are at risk for hypoventilatory respiratory compromise and "positional" asphyxiation. We review case reports of custody deaths in subjects placed in the hogtie position, as well as related medical literature regarding positional asphyxia. We also review the cur

Thirty-eight knees in 34 patients with an average age of 22 years were diagnosed as having lateral facet syndrome (LFS), a painful compressive arthropathy of the lateral facet of the patella. This diagnosis was based on the physical findings of tenderness at the lateral patellofemoral joint line, tenderness over the vastus lateralis obliquus (VLO) tendon just above the patella, a positive medial apprehension test, and marked resistance to medial patellar displacement with the knee flexed 30 degrees. The most common complaints were patellar pain with activity, pain with prolonged knee flexion,

BACKGROUND: Agitation in older adults presents unique challenges for emergency medical services (EMS) clinicians. To safely assess, treat, and transport these patients, EMS providers must manage this agitation, which is often caused by delirium or behavioral symptoms of dementia. Strategies routinely used in younger patients, such as physical restraints and chemical sedatives, have a much higher risk of causing harm in older adults. EMS provider practices in this area are understudied. Our objective was to examine U.S. state protocols for guidance on agitation management in older adults.

BACKGROUND: Behavioral and psychological symptoms in dementia significantly contribute to caregiver burden and impose patient hospitalization. The goal of treatment of admitted patients is the rapid remission of symptoms to allow their return to home as soon as possible. Intervention requires an intrusive approach with parenteral treatment and physical restraints, with a negative emotional impact on patients and their families. Despite the large utilization of antipsychotics for behavioral and psychological symptoms, there is no antipsychotic approved by the Food and Drug Administration for ag

Animal testing remains essential in biomedical research, contributing significantly to understanding disease mechanisms and evaluating the efficacy and safety of drugs and chemical substances, with rodents among the most frequently used laboratory animals. Among the most common interventions in laboratory practice are handling and immobilizing restraint during procedures such as injections and blood sampling, which are routinely performed on conscious mice without sedation or anesthesia. Proper restraint is necessary to ensure the safety of both the animal and the handler. However, traditional

Limiting stress during interactions between captive animals and humans is important for effective husbandry. One physiological change during the vertebrate stress response is the release of glucocorticoid hormones. Here, we measured plasma corticosterone in female domesticated budgerigars (Melopsittacus undulatus) to test whether human presence, restraint, or removal and return of a newly stressed neighbor increases corticosterone. The presence of humans for 15 min at the beginning of the experiment did not significantly elevate corticosterone above baseline levels, suggesting that birds that

Delirium is common in the emergency department, and once it develops, there is a risk of self-extubation of drains and tubes, so it is critical to predict delirium before it occurs. Machine learning was used to create two prediction models in this study: one for predicting the occurrence of delirium and one for predicting self-extubation after delirium. Each model showed high discriminative performance, indicating the possibility of selecting high-risk cases. Visualization of predictors using Shapley additive explanation (SHAP), a machine learning interpretability method, showed that the predi

BACKGROUND: The reaction to stress is an adaptive response necessary for survival. When stressors are repeated, the organism adapts, although these adaptive responses can become dysregulated and result in disease, causing gastrointestinal (GI) disorders. Radiographic methods allow the non-invasive study of how a given factor affects GI transit in the same animal at different time points. These methods have never been applied to study the consequences of stress on GI motor function and their dependency on time and stimulus. Therefore, our aim was to characterize, using radiographic techniques,

BACKGROUND: Stress causes severe dysmotility in the mammalian gut. Almost all research done to date has concentrated on prevention of stress-induced altered gut motility but not on treatment. We had previously shown that intraluminal 2'FL could acutely moderate propulsive motility in isolated mouse colonic segments. Because 2'FL appeared to modulate enteric nervous system dependent motility, we wondered if the oligosaccharide could reverse the effects of prior restraint stress, ex vivo. We tested whether 2'FL could benefit the dysmotility of isolated jejunal and colonic segments from animals s
