Restrictive fluid resuscitation versus liberal fluid resuscitation in patients with septic shock: comparison of outcomes.

OBJECTIVE: To compare the prognosis of restrictive fluid resuscitation (RFR) versus liberal fluid resuscitation (LFR) in patients with septic shock. METHODS: A retrospective analysis was conducted using clinical data from 82 septic shock patients treated in the Intensive Care Unit of Aviation General Hospital from January 2021 to December 2023. Patients were divided into two groups: the LFR group (n=41) and the RFR group (n=41), based on the resuscitation strategy used. RESULTS: Both groups demonstrated significant reductions in heart rate (HR) and significant increases in mean arterial pres

[History of Resuscitation: 3. Development of Resuscitation in the Mid-18 Century-3: Artificial Respiration].

In the mid-18th century, resuscitation attempts started of "apparently dead" people as a result of drowning or other causes. In this article, I describe development of artificial ventilation. It was already rec- ognized in the mid-18th century that early initiation of artificial ventilation was the main factor for successful resuscitation. The oldest remaining record of mouth-to- mouth resuscitation performed was on November 11th 1732, by William Tossach, and John Fothergill in Edin- burgh, and this encouraged lay people to do this pro- cedure. Monro Secundus, Kite and Cullen attempted to vent

Opinions of Brazilian resuscitation instructors regarding resuscitation in the delivery room of extremely preterm newborns.

OBJECTIVE: To describe the opinions of pediatricians who teach resuscitation in Brazil on initiating and limiting the delivery room resuscitation of extremely preterm infants. METHOD: Cross-sectional study with electronic questionnaire (Dec/2011-Sep/2013) sent to pediatricians who are instructors of the Neonatal Resuscitation Program of the Brazilian Society of Pediatrics, containing three hypothetical clinical cases: (1) decision to start the delivery room resuscitation; (2) limitation of neonatal intensive care after delivery room resuscitation; (3) limitation of advanced resuscitation in t

Emergency medicine updates: Cardiopulmonary resuscitation.

INTRODUCTION: Cardiac arrest is the loss of functional cardiac activity; emergency clinicians are integral in the management of this condition. OBJECTIVE: This paper evaluates key evidence-based updates concerning cardiopulmonary resuscitation (CPR). DISCUSSION: Cardiac arrest includes shockable rhythms (i.e., pulseless ventricular tachycardia and ventricular fibrillation) and non-shockable rhythms (i.e., asystole and pulseless electrical activity). The goal of cardiac arrest management is to achieve survival with a good neurologic outcome, in part by restoring systemic perfusion and obtaini

Fluid Resuscitation in Septic Patients.

Sepsis is a life-threatening organ failure caused by a dysregulated response to infection. Fluid resuscitation and vasopressors are used to maintain systolic blood pressure and organ perfusion. Fluid resuscitation can be done with liberal or restricted fluids as well as colloids or crystalloid fluids. This review analyses the evidence for the use of liberal or restrictive fluids and colloids or crystalloids for the management of sepsis. A methodical search was conducted across PubMed, Cochrane Library, and ScienceDirect, and the Preferred Reporting Items for Systematic Reviews and Meta-Analyse

Resuscitation Strategies for Maximizing Survival.

There is no single resuscitation strategy that will uniformly improve cardiac arrest outcomes. Traditional vital signs cannot be relied on in cardiac arrest, and the use of continuous capnography, regional cerebral tissue oxygenation, and continuous arterial monitoring are options for use early defibrillation are critical elements of resuscitation. Cardio-cerebral perfusion may be improved with the use of active compression-decompression CPR, an impedance threshold device, and head-up CPR. In refractory shockable arrest, if ECPR is not an option, consider changing defibrillator pad placement a

Special consideration in neonatal resuscitation.

Delivery room resuscitation of neonates is performed according to evidence-based neonatal resuscitation guidelines. Neonatal resuscitation guidelines focus on the resuscitation of newborns suffering from perinatal asphyxia. Special considerations are needed when resuscitating newborns in locations other than the delivery room and for newborns with congenital anomalies. In this review, we examine the resuscitation of newborns at home and in the emergency department and highlight special considerations for resuscitating newborns with specific congenital anomalies. In addition, we explore the res

Cardiopulmonary resuscitation in special circumstances.

Cardiopulmonary resuscitation prioritises treatment for cardiac arrests from a primary cardiac cause, which make up the majority of treated cardiac arrests. Early chest compressions and, when indicated, a defibrillation shock from a bystander give the best chance of survival with a good neurological status. Cardiac arrest can also be caused by special circumstances, such as asphyxia, trauma, pulmonary embolism, accidental hypothermia, anaphylaxis, or COVID-19, and during pregnancy or perioperatively. Cardiac arrests in these circumstances represent an increasing proportion of all treated cardi

Updates in Cardiac Arrest Resuscitation.

There are approximately 350,000 out-of-hospital cardiac arrests and 200,000 in-hospital cardiac arrests annually in the United States, with survival rates of approximately 5% to 10% and 24%, respectively. The critical factors that have an impact on cardiac arrest survival include prompt recognition and activation of prehospital care, early cardiopulmonary resuscitation, and rapid defibrillation. Advanced life support protocols are continually refined to optimize intracardiac arrest management and improve survival with favorable neurologic outcome. This article focuses on current treatment reco

Enhanced monitoring during neonatal resuscitation.

Immediately after birth through spontaneous breaths, infants' clear lung liquid replacing it with air, and gradually establishing a functional residual capacity to achieve gas exchange. Most infants start breathing independently after birth and ~3% of infants who require positive pressure ventilation. When newborns fail to start breathing the current neonatal resuscitation guidelines recommend initiatingpositive pressure ventilationusing a face mask and a ventilation device. Adequate ventilation is the cornerstone of successful neonatal resuscitation; therefore, it is mandatory that anybody in

Decompensated Cirrhosis and Fluid Resuscitation.

The critically ill patient with decompensated cirrhosis has a unique physiology and alterations in albumin that need to be understood to properly resuscitate them and minimize morbidity and mortality. Little data exist on specific resuscitation of the patient with cirrhosis compared with those patients without liver disease. The effectiveness of albumin administration compared with saline administration in common settings, such as large-volume paracentesis, can be extrapolated to the care of the general surgical patient but further studies in this area are warranted. This article enhances the

Epinephrine Use during Newborn Resuscitation.

Epinephrine use in the delivery room for resuscitation of the newborn is associated with significant morbidity and mortality. Evidence for optimal dose, timing, and route of administration of epinephrine during neonatal resuscitation comes largely from extrapolated adult or animal literature. In this review, we provide the current recommendations for use of epinephrine during neonatal resuscitation and also the evidence behind these recommendations. In addition, we review the current proposed mechanism of action of epinephrine during neonatal resuscitation, review its adverse effects, and iden

Cardiopulmonary resuscitation in hospitalized infants.

BACKGROUND: Hospitalized infants requiring cardiopulmonary resuscitation (CPR) represent a high-risk group. Recent data on risk factors for mortality following CPR in this population are lacking. AIMS: We hypothesized that infant demographic characteristics, diagnoses, and levels of cardiopulmonary support at the time of CPR requirement would be associated with survival to hospital discharge following CPR. STUDY DESIGN: Retrospective cohort study. SUBJECTS: All infants receiving CPR on day of life 2 to 120 admitted to 348 Pediatrix Medical Group neonatal intensive care units from 1997 to 20

Sustained inflation during neonatal resuscitation.

PURPOSE OF REVIEW: Sustained inflation performed shortly after birth to help clear lung fluid and establish functional residual capacity in preterm infants is gaining popularity, but definitive evidence for its effectiveness is lacking. Although there is a sound physiologic basis for this approach, and much preclinical experimental evidence of effectiveness, the results of recent animal studies and clinical trials have been inconsistent. RECENT FINDINGS: The most recent data from a multicenter randomized trial suggest a modest benefit of sustained inflation in reducing the need for mechanical

Rotational thromboelastometry-guided trauma resuscitation.

PURPOSE OF REVIEW: Haemorrhage from major trauma is a significant cause of death worldwide. The UK Defence Medical Service (UK-DMS) has had significant experience in managing severely injured and shocked trauma casualties over the last decade. This has led to the integration of rotational thromboelastometry (ROTEM) into damage control resuscitation delivered at Camp Bastion Field Hospital in Afghanistan. This review aims to describe the rationale for its use and how its use has evolved by UK-DMS. RECENT FINDINGS: Although there is reasonable evidence showing its benefit in cardiac and liver s
