Vocal Hoarseness and a Subglottic Mass: An Uncommon Diagnosis for a Common Complaint.

We report a patient with tracheopathia osteoplastica (TPO), a rare or perhaps underrecognized disorder, detected in approximately 1 in every 2000 to 5000 patients who undergo bronchoscopy. TPO is marked by proliferation of bony and cartilaginous spurs leading to airway stenosis. Multiple submucosal cartilaginous and osseous nodules can develop in the respiratory tract and may involve the entire trachea and mainstem bronchi. Symptoms may range from a completely silent condition to life-threatening respiratory failure and diagnosis is made based on radiological and bronchoscopic findings. Althou

Acute rheumatic fever in Jordanian children.

Rheumatic fever remains a significant health problem in Jordan. We retrospectively reviewed medical charts of 28 boys and 22 girls (mean age at presentation 10.5 +/- 2.6 years) with confirmed diagnosis based on modified Jones criteria at Queen Alia Heart Institute from February 1999 to February 2002. Arthritis was the commonest major manifestation (88%; 68% migratory), carditis was second commonest (48%; 8% silent carditis) and chorea was seen in 6%. None had subcutaneous nodules or erythema marginatum. The mitral valve was most commonly affected (80%); both mitral and aortic valves were affec

Lucio's Phenomenon: Report of Two Cases and Review of the Literature.

Hansen's disease is a chronic granulomatous disease of infectious origin. It has a worldwide distribution and a variety of clinical manifestations often involving the skin, nasal mucosa, and peripheral nerves. Lepromatous leprosy characterizes the condition of a large group of patients with little or no resistence to the infection. Several forms of lepromatous leprosy are recognized, including macular, nodular, and diffuse. Lucio's phenomenon is a rare but distinctive skin eruption seen in patients with diffuse lepromatous leprosy. The diffuse lesions of Lucio's phenomenon have a predilection

Cutaneous lesions and articular manifestations in multicentric reticulohistiocytosis: a diagnostic challenge.

A 56-year-old woman presented with a one-year history of polyarthralgia and skin lesions. Examination revealed polyarthritis and reddish-brown nodules on interphalangeal (IF) joints. Histopathology confirmed a diagnosis of multicentric reticulohistiocytosis (MRH) with characteristic histiocytes and multinucleated giant cells positive for PAS, PASD, CD45, and CD68. Imaging showed periarticular osteopenia and erosions. Immunosuppressive therapy with prednisolone (PDN) and methotrexate (MTX) led to significant improvement in symptoms within two months. MRH, a rare non-Langerhans cell histiocytosi

Lucio's phenomenon: report of five cases.

The different clinical forms of leprosy are mainly related to the variety of immunological responses to the infection. Several forms of lepromatous leprosy are recognized, including macular, nodular, and diffuse. Lucio's phenomenon is a rare but distinctive skin eruption seen in patients with diffuse lepromatous leprosy. The diffuse lesions of Lucio's phenomenon have a predilection for the extremities, can include nodules, and heal with atrophic stellate scars; histologically, a necrotizing vasculitis accompanied by a nonspecific inflammatory reaction may be seen. Because of its rarity and sim

Analysis of the clinical characteristics of thirteen patients with Weber-Christian panniculitis.

BACKGROUND: Weber-Christian disease (WCD) is an uncommon, idiopathic disease that is challenging to diagnose and has an unclear treatment protocol. We reviewed thirteen patients with WCD and analyzed their clinical characteristics. The purpose of this article was to improve the understanding of this rare disorder. METHODS: Thirteen cases of WCD were analyzed retrospectively regarding their clinical manifestation, laboratory results, misdiagnoses, therapy, and outcome. RESULTS: Of the thirteen patients diagnosed with WCD, the majority were female (male to female ratio, 2:11), with a mean pati

[Rheumatoid nodule in the myocardium as part of the chain in a clinical detective. Case report].

The article describes a clinical case of a patient, at first glance healthy from the cardiovascular side, whose only complaint was ventricular extrasystole during exacerbations of arthritis. A typical scenario of such patients management is the suppression of the rheumatoid arthritis activity with the expectation that the involvement of target organs will subside accordingly. A rheumatoid nodule - specific manifestation of rheumatoid arthritis in the heart, that is usually detected in postmortem examinations and rare - according to echocardiography. Despite its small size (1.5×2 mm), the granu

The role of myocardial biopsy in cardiac diagnosis.

With the advent of cardiac catheterization, cardioangiography, and selective coronary arteriography, specific types of cardiac disease can be recognized and clearly defined. This is appropriate because myocardial biopsy alone rarely plays a major role in cardiac diagnosis. Excluding Aschoff's nodules in patients with rheumatic valve disease, the light microscopic findings in patients with rheumatic heart disease, congenital heart disease, pericardial disease, hypertensive and arteriosclerotic heart disease are similar and nonspecific. In these, interstitial fibrosis and/or myocardial hypertrop

PAPILLARY THYROID CARCINOMA FOLLOWING ADALIMUMAB TREATMENT IN ANKYLOSING SPONDYLITIS: A CAUSAL OR COINCIDENTAL ASSOCIATION?

The introduction of tumor necrosis factor-alpha (TNF-α) targeting drugs has given new opportunities in the treatment of various inflammatory rheumatic diseases and has been the most important development in the treatment of ankylosing spondylitis (AS). However, the increasing use and longer follow-up periods of treatment also pose risks of developing various adverse effects, ranging from common ones, including infections, to uncommon thyroid neoplasms. Adalimumab (ADA), a fully human monoclonal antibody targeted against TNF-α, is indicated for AS. We describe the case of a patient with AS who

Brief Report: Association of Quantitative and Topographic Assessment of Heberden's Nodes With Knee Osteoarthritis: Data From the Osteoarthritis Initiative.

OBJECTIVE: To determine whether the presence, number, and topography (digit location and symmetry) of Heberden's nodes are associated with the incidence and progression of radiographic osteoarthritis (OA) of the knee. METHODS: We analyzed 8,023 knees (with 8 years of follow-up) from the Osteoarthritis Initiative. Cox regression was performed on Heberden's node presence, total number, location, and symmetry (using 2 symmetry index models) obtained at baseline physical examination as well as self-report of Heberden's node presence for evaluation of association with radiographic knee OA incidenc

[Visceral manifestations of rheumatoid polyarthritis].

The commonest extra-articular symptoms of rheumatoid arthritis include nodules, vasculitis, neuro-muscular involvement and keratoconjunctivitis sicca with dry mouth, characteristic of Sjögren's syndrome. However, laboratory tests demonstrate that there is a high percentage of pericarditis and involvement of the lungs, liver and pancreas, which are clinically silent. Personal statistics on extra-articular symptoms in 110 cases of classic rheumatoid arthritis are presented.

[Superior oblique tendon trochlear passage syndrome : Causes, motility findings and treatment of acquired Jaensch-Brown syndrome].

CLINICAL FEATURES: Acquired Jaensch-Brown syndrome is characterized by a mechanical limitation of elevation in adduction, with orthophoria in downward gaze. It was first described by Jaensch in 1928 after orbital trauma in his case and has the same motility pattern as congenital Brown's syndrome. For this reason, in 1973 Brown differentiated between the "true" and "simulated" cases. Further clinical findings of the different etiological factors must be considered in order to differentiate between the two groups. ORIGIN: The cause is an acquired restriction of the free passage of the superior

[Pulmonary sarcoidosis developing during treatment with etanercept].

INTRODUCTION: TNF blockers are widely used to treat inflammatory rheumatic diseases and also in the treatment of extrapulmonary sarcoidosis. TNFα plays a major role in the development and persistence of sarcoid granulomata. However, recent studies have reported the involvement of anti-TNF therapies in the development of granulomatosis associated with the clinical and radiological features of sarcoidosis. CASE REPORT: A 54-years-old man with ankylosing spondylitis was treated with etanercept for two years. He was admitted with symptoms of bronchitis associated with radiological evidence of bil

Cutaneous Manifestations of Rheumatoid Arthritis: Diagnosis and Treatment.

Rheumatoid arthritis (RA) is a chronic, systemic autoimmune disorder characterized by inflammatory arthritis and periarticular structural damage. Available evidence suggests that RA results from complex interactions between genetic susceptibility (e.g., HLA-DRB1), environmental factors (e.g., smoking), and immune dysregulation. Alongside joint-related symptoms, individuals with RA may also experience a wide array of skin issues, including the development of nodules, neutrophilic dermatoses, vasculitis, and vasculopathy. Treatment strategies for these manifestations vary but routinely involve c
