Jarcho-Levin syndrome: report on a long-term follow-up of an untreated patient.

Jarcho-Levin syndrome is a genetically transmitted rare entity characterized by multiple vertebral and rib anomalies. The multilevel skeletal involvement causes short stature, neck and thoracic cage deformities, and restrictive lung disease that is usually the cause of early death. The authors describe a 33-year follow-up of a patient with this syndrome who represents, to their best knowledge, the longest survival of a patient with this entity.

Morquio's disease and the hearing organ.

Histopathologic evidence offered by a case of Morquio's disease and observed in the hearing organ showed widespread collagen damage, possibly initiated by the general hemorrhageous tendency, while the family history bore evidence of a genetic source. The clinical manifestations comprise striking dwarfism, beaking of the lumbar spine, sternal bulging, flaring of the rib cage, falt vertebrae, marked osteoporosis, prominent joints, semi-crouching stance, short nose. Roentgenograms showed retarded ossification.

Determinants of rib motion in flail chest.

We have previously developed a canine model of isolated flail chest to assess the effects of this condition on the mechanics of breathing, and these studies have led to the conclusion that the respiratory displacement of the fractured ribs is primarily determined by the fall in pleural pressure (Delta Ppl) and the action of the parasternal intercostal muscles. The present studies were designed to test the validity of this conclusion. A flail was induced in six supine anesthetized animals by fracturing both dorsally and ventrally the second to fifth ribs on the right side of the chest, after wh

Bilateral cervical ribs forming pseudoarthrosis with the first ribs co-occurring with an aberrant right subclavian artery.

While anatomical variations in both aortic arch branching and rib cage development are considered relatively common, compared to various other anomalies, their coexistence seems to be underreported. We present a case report of a 70-year-old Polish woman with bilateral complete cervical ribs forming a pseudoarthrosis with the superior surface of the first ribs, and an aberrant right subclavian artery (arteria lusoria) with a retroesophageal course. We believe that the frequency of their co-occurrence may be higher than would be expected based on statistical probability derived from their preval

Skeletal abnormalities in doubly heterozygous Bmp4 and Bmp7 mice.

Analysis of the skeletal phenotypes caused by the genetic inactivation of individual Bmps, along with the study of their expression patterns, suggest possible functional redundancy of these molecules. To investigate the effect on skeleton development of the combined absence of some Bmp genes expressed in the same areas, we have intercrossed heterozygous Bmp7 mice with Bmp2 +/-, Bmp4 +/-, or Bmp5 +/- animals. Bmp2/7 and Bmp5/7 double heterozygous animals do not present with any abnormalities. In contrast, Bmp4/7 double heterozygotes develop minor defects in two restricted areas of the skeleton,

[A modified retroperitoneal approach to the kidney in patients with a highly deformed thorax: obtaining a wide operative field through subperiosteal resection of the 10th, 11th and 12th ribs].

We herein report a technique which facilitates a retroperitoneal approach to the kidney in cases of highly deformed thorax due to kyphoscoliosis. The operation consists of a lumbar oblique incision with removal of the 11th rib, combined with the additional removal of the 12th and 10th ribs. Resection of the upper two ribs was performed subperiosteally, leaving the periosteum of the deep side untouched. However, the deep side periosteum of the 12th rib was incised caudal from the pleural margin in order to facilitate exposure of the diaphragm. The retroperitoneal space was entered through the t

Thoracic cavity-to-cage ratio is a predictor of technical difficulties in minimally invasive esophagectomy.

BACKGROUND: Technical difficulties are occasionally encountered when performing conventional minimally invasive esophagectomy and robot-assisted minimally invasive esophagectomy in patients with a narrow thoracic cavity. Thoracic cavity-to-cage ratio is an indicator of thoracic cavity length. We hypothesized that the thoracic cavity-to-cage ratio could be a predictor of technical difficulties in conventional minimally invasive esophagectomy and robot-assisted minimally invasive esophagectomy. METHODS: We evaluated 340 patients who underwent minimally invasive esophagectomy for esophageal squa

Reversed Rivero-Carvallo's sign confirmed by blood flow analysis using cardiac magnetic resonance imaging in a patient with straight back syndrome.

An asymptomatic 45-year-old man was referred to our hospital for detailed evaluation of a systolic ejection murmur. The intensity of the murmur increased on deep expiration and decreased on deep inspiration, showing so-called reversed Rivero-Carvallo's sign. Using cardiac magnetic resonance imaging, we demonstrated a characteristic respiratory-induced change in peak flow velocity in the right ventricular outflow tract, which was the basic mechanism of the reversed Rivero-Carvallo's sign in a case with straight back syndrome. Concomitant anatomical changes in the entire heart in relation to the

Anetoderma Presenting as Neoplasms of Uncertain Behavior in an Elderly Female: A Case Report.

Anetoderma is a rare cutaneous disorder characterized by focal loss of dermal elastic tissue, often presenting as atrophic or herniated plaques. However, clinical presentations can vary, and atypical forms may resemble neoplastic or inflammatory lesions, complicating diagnosis. We present the case of a 78-year-old female who was evaluated for asymptomatic skin growths on the posterior neck, chest, and torso, reported to have been present for five years and slowly spreading. On physical examination, firm papules and subcutaneous nodules were noted on the left rib cage, left medial breast, and m

Respiratory sinus arrhythmia in young men and women at different chest wall configurations.

Respiratory sinus arrhythmia (RSA) is an acceleration of heart rate during inspiration and deceleration with expiration. We asked whether or not in humans some of the volume-related information necessary for RSA originated from the chest wall. Men and women, 19-20 years old, were breathing supine. Rib cage and abdomen displacement provided an index of tidal volume (VT) and RSA was computed breath-by-breath from the peak and trough of instantaneous heart rate. First, measurements were taken during breathing at rest (protocol a, 129 male and 164 female). Then, in subgroups of the original subjec

Effect of high-flow nasal cannula on thoraco-abdominal synchrony in adult critically ill patients.

BACKGROUND: High-flow nasal cannula (HFNC) creates positive oropharyngeal airway pressure and improves oxygenation. It remains unclear, however, whether HFNC improves thoraco-abdominal synchrony in patients with mild to moderate respiratory failure. Using respiratory inductive plethysmography, we investigated the effects of HFNC on thoraco-abdominal synchrony. METHODS: We studied 40 adult subjects requiring oxygen therapy in the ICU. Low-flow oxygen (up to 8 L/min) was administered via oronasal mask for 30 min, followed by HFNC at 30-50 L/min. Respiratory inductive plethysmography transducer

Breathing pattern in patients with Parkinson's disease.

The improvement in motor performance resulting from levodopa administration in patients with Parkinson's disease (PD) provides the opportunity to investigate ventilatory changes brought about by the disease. The aim of this study has been to investigate these changes in order to specify the mechanisms of the impairment in breathing in PD. Breathing patterns at rest were investigated in 11 patients with idiopathic PD both before (OFF) and after (ON) administration of levodopa at a dose improving their motor performance by at least 30%. Airflow (Fleisch head mounted on a mask), rib cage and abdo

Respiratory muscle activity during rapid eye movement (REM) sleep in patients with chronic obstructive pulmonary disease.

BACKGROUND: In patients with chronic obstructive pulmonary disease (COPD) periods of hypopnoea occur during rapid eye movement (REM) sleep, but the mechanisms involved are not clear. METHODS: Ten patients with stable COPD were studied during nocturnal sleep. Detailed measurements were made of surface electromyographic (EMG) activity of several respiratory muscle groups and the accompanying chest wall motion using magnetometers. RESULTS: Hypopnoea occurred in association with eye movements during phasic rapid eye movement (pREM) sleep. During pREM sleep there were reductions in EMG activity o

The respiratory muscles in multicore myopathy.

Multicore myopathy is a rare congenital myopathy that can cause progressive weakness, but it has not been recognized previously to have respiratory consequences. This study describes two patients who developed respiratory failure because of respiratory muscle weakness. Both patients had low vital capacities without evidence of airway obstruction, and CO2 retention. Physical examination found that neck accessory muscles and abdominal muscles were very weak. In inspiration the abdomen expanded, but the rib cage contracted. Detailed studies were done in one patient with magnetometers, esophageal

Posterior intercostal nerve block for pain relief after cholecystectomy. Anatomical basis and efficacy.

The human intercostal space has been studied by excision of the posterior part of the rib cage at autopsy, followed by fixation, decalcification, section and staining. Injection of India ink was used to simulate local anaesthetic. At a point 7 cm from the midline, the distance from the posterior aspect of the rib to the pleura averaged 8 mm. The intercostal nerve usually comprises a number of small bundles without any enclosing fascial sheath. The bundles lie in a triangular space bounded by the rib, the posterior intercostal membrane and the intercostalis intima muscle. The first two appear i

Prevalence of Fractures and Diagnostic Accuracy of Emergency X-ray in Older Adults Sustaining a Low-Energy Fall: A Retrospective Study.

BACKGROUND: Plain radiography (XR) series are standard of care for detection of fall-related fractures in older patients with low-energy falls (LEF) in the emergency department (ED). We have investigated the prevalence of fractures and diagnostic accuracy of XR imaging in the ED. METHODS: 2839 patients with LEF, who were presented to two urban level I trauma centers in 2016 and received XR and computed tomography (CT), were consecutively included in this retrospective cohort study. The primary endpoint was the prevalence of fractures of the vertebral column, rib cage, pelvic ring, and proxima

Defective neural tube closure and anteroposterior patterning in mice lacking the LIM protein LMO4 or its interacting partner Deaf-1.

LMO4 belongs to a family of transcriptional regulators that comprises two zinc-binding LIM domains. LIM-only (LMO) proteins appear to function as docking sites for other factors, leading to the assembly of multiprotein complexes. The transcription factor Deaf-1/NUDR has been identified as one partner protein of LMO4. We have disrupted the Lmo4 and Deaf-1 genes in mice to define their biological function in vivo. All Lmo4 mutants died shortly after birth and showed defects within the presphenoid bone, with 50% of mice also exhibiting exencephaly. Homeotic transformations were observed in Lmo4-n

Overall contribution of chest wall hyperinflation to breathlessness in asthma.

BACKGROUND: Studies suggest that the increased volume of both abdominal and rib cage compartments of the chest wall contribute to dyspnea during methacholine-induced airway narrowing. MATERIAL: Eight male patients with asthma aged 34 +/- 13 years (mean +/- SD) before and during methacholine challenge. METHODS: The volume of the chest wall (Vcw), volume of the abdomen (Vab), and volume of the rib cage (Vrc) were measured by using a three-dimensional optoelectronic plethysmography. RESULTS: During methacholine challenge, the increase in end-expiratory Vcw (Vcw,ee) [0.55 +/- 0.23 L, p < 0.001]

Effect of progressive hypoxia on breathing during sleep.

We examined the effects of progressive hypoxia on breathing during wakefulness and quiet (NREM) sleep in 17 healthy young adults (11 males and 6 females). Ventilation was determined from quantitative measurements of abdominal and rib cage excursions using magnetometers or inductive plethysmography. Hypoxia was induced by blending N2 into the inspiratory line of a loose-fitting mask while O2 saturation was monitored with an ear oximeter. No attempt was made to maintain isocapnia. Ventilatory responses to hypoxia were depressed in 2 male subjects by sleep, but were unchanged or increased in the
