Successful Nonsurgical Resolution of a Large Periapical Lesion With Pre-Existing Sealer Extrusion and Adjacent External Inflammatory Root Resorption: A Case Report.

The primary aim of root canal treatment is to eliminate intracanal infection, achieve an apical and coronal seal, and create conditions that allow repair of the periapical tissues. In root-filled teeth, persistent apical periodontitis may be associated with extensive periapical lesions, external inflammatory root resorption affecting the treated or adjacent teeth, and extrusion of endodontic materials. These complications may lead clinicians to consider surgical approaches. However, when canals are adequately cleaned, disinfected, and hermetically obturated, favorable healing may occur followi

DETERMINATION OF CYCLIC FATIGUE OF A NICKEL-TITANIUM COXO SC PRO FILE USING A SIMULATION ENDODONTIC UNIT.

The use of nickel-titanium instruments during endodontic treatment ensures high-quality treatment of root canals. However, during instrumentation in the curved root canals of nickel-titanium instruments experience cyclic loading, which leads to their breakage. The splinter of the tool in the root canal makes it difficult for high-quality processing and obturation of the root canal, which is a very urgent problem. To carry out instrumental processing in canals with various anatomical and topographic features, the files must have phenomenal flexibility, lack of straightening of the canal, and ha

A novel root canal filling technique using a resin-based sealer without gutta-percha: a summary of two cases.

OBJECTIVES: Root canal filling is a fundamental step in non-surgical root canal treatment (RCT), required to prevent reinfection or new infection within the root canal system. Traditionally root canal filling uses a sealer in combination with gutta-percha (GP). Here, two cases are presented in which the root canals were filled using only a methacrylic resin-based sealer without GP. MATERIALS AND METHODS: Two patients (one male, one female) required RCT - one due to irreversible pulpitis and the other to facilitate a subsequent post-retained protheses. Each RCT was performed according to a sta

The Twin-Rooted Anomaly: Endodontic Management of a Rare Case of a Maxillary Central Incisor With Two Roots.

This case report describes the endodontic management of a maxillary central incisor with two roots, representing a rare anatomical variation. A 73-year-old female patient presented with a fractured tooth #21 with a carious crown. Pulp sensibility testing using cold spray elicited a sharp and prolonged painful response, consistent with a diagnosis of symptomatic irreversible pulpitis with normal apical tissues. Conventional periapical radiography suggested an unusual root morphology, which was further evaluated using cone-beam computed tomography (CBCT). CBCT imaging revealed a bifurcated root

Partial Pulpectomy with Bio-Obturation for Internal Root Resorption: A Case Report.

Internal root resorption (IRR) is an uncommon but clinically significant condition traditionally managed with complete pulpectomy and root canal therapy. Advances in vital pulp therapy and bioactive calcium silicate-based cements have enabled conservative strategies aimed at preserving residual pulp vitality. A 33-year-old woman presented with an asymptomatic, unperforated IRR in the maxillary left lateral incisor, confirmed by periapical radiography and cone-beam computed tomography. The tooth responded positively to sensibility testing and showed healthy periapical tissues. A partial pulpect

Portland cement use in dental root perforations: a long term followup.

Root canal and furcal perforations are causes of endodontic therapy failure and different materials that stimulate tissue mineralization have been proposed for perforation treatment. In the first case, a patient presented tooth 46 with unsatisfactory endodontic treatment and a periapical radiographic lesion. A radiolucent area compatible with a perforating internal resorption cavity was found in the mesial root. The granulation tissue was removed, and root canals were prepared. The intracanal medication was composed of calcium hydroxide and the perforation cavity was filled with Portland cemen

Impact of final rinse with saline or alcohol solution on root canal sealability.

Endodontic irrigation protocols are not only used to clean and disinfect the root canal system, but also to condition the canal wall dentine for subsequent root filling. In this study we tested whether a final irrigation step with saline solution or 80% ethanol improved root canal sealabilty by two popular sealers, an epoxy resin (AH Plus) and a hydraulic calcium silicate cement-based product (BioRoot RCS). Root canals in extracted single-rooted human teeth were instrumented and filled with a matched gutta-percha cone and sealer. During instrumentation and prior to root filling, sealer-specifi

Periapical status, quality of root canal fillings and estimated endodontic treatment needs in an urban German population.

The objective of this study was to determine the periapical status and the quality of root canal fillings and to estimate the endodontic treatment needs in a German population. Clinical and radiographic data and the operative procedures performed were evaluated on 323 patients coming to a dental surgery in Stuttgart, Germany, in 1993. In 182 individuals at least one tooth exhibited a root canal filling, a necrotic pulp or an irreversible pulpitis. Out of the 7897 teeth examined, 215 (2.7%) had a root canal treatment (category A), 122 being non-endodontically treated (1.5%) did not respond to t

Use of fluid filtration method to evaluate the effect of master cone size on the apical seal of severely curved root canals.

The apical third of most root canals shows some degree of curvature, which is important in cleaning/shaping and obturation during root canal treatment. The present study evaluated the effect of master cone size on the apical seal of severely curved root canals. Thirty-eight mesial roots of human mandibular first molars were prepared using the crown-down technique. All samples were mature roots with closed apices, had no carious lesions or resorption, and had a canal curvature of > 45º to 60º. Two samples were used as a negative and positive control to evaluate the fluid filtration equipment, a

Evaluation of radicular fracture resistance of maxillary premolars following non-surgical retreatment using two novel retreatment kits.

BACKGROUND: The structural integrity of teeth after endodontic retreatment depends on the amount of remaining dentin. Excessive instrumentation can weaken roots and predispose them to fracture. This study evaluated the fracture resistance of maxillary premolars following non-surgical retreatment (NS-ReT) with Retreaty and XP-endo Rise systems. MATERIALS AND METHODS: Forty-eight maxillary first premolar roots were randomly assigned to 4 groups (n = 12); G1-NC (Negative control, non-instrumented), G2-PC (Positive control, instrumented, not obturated), G3-RETY (Retreaty), and G4-XPER (XP-endo Ri
