Apical root resorption in patients treated with comprehensive orthodontics.

External apical root resorption (EARR) is a common--but seldom extreme--consequence of orthodontic treatment. Incisors are most at risk, perhaps because of their single roots and because they typically are moved farther than other teeth. We followed a cohort of patients (n = 153) treated with comprehensive orthodontics. EARR was scored on the upper incisors with a qualitative five-grade ordinal scale. There was no EARR at the start of treatment, but most (> 80%) exhibited slight-to-moderate EARR by the end of treatment (i.e., a loss of 1-2 mm). Cases treated with premolar extractions experienc

Predicting and preventing root resorption: Part I. Diagnostic factors.

The purpose of this study was to determine whether it would be possible to identify pretreatment factors that will allow the clinician to predict the incidence, location, and severity of root resorption before the commencement of orthodontic treatment. The records of 868 patients who were treated with full, fixed edgewise appliances were obtained from 6 private offices; full-mouth periapical radiographs were used to accurately assess apical root resorption from first molar to first molar in both arches. The results showed that resorption occurs primarily in the maxillary anterior teeth, averag

Predicting and preventing root resorption: Part II. Treatment factors.

The main objective of this study was to determine which treatment factors are most clearly identified with external apical root resorption that is detectable on periapical radiographs at the end of orthodontic treatment. The records of 868 patients who completed fixed, edgewise treatment from experienced clinicians in private practice were examined. The horizontal and vertical displacement of the root apex of the maxillary central incisor was measured on cephalometric radiographs. Patients who underwent first premolar extraction therapy had more resorption than those patients who had no extrac

Deciduous canine and permanent lateral incisor differential root resorption.

When a permanent maxillary canine erupts apical to the permanent lateral incisor and the deciduous canine, resorption typically takes place only on the deciduous canine root. An understanding of this differential resorption could provide insight into the reasons for excessive iatrogenic root resorption during orthodontic tooth movement. The purpose of the present study was to examine the response of roots of permanent lateral incisors and deciduous canines to simulated resorption, and to acid and enzyme attack, reflecting the physiologic environment of an erupting permanent canine. Transition and determinants of orthodontic root resorption-repair sequence.

When severe root resorption is observed during orthodontic treatment the clinician needs to know how the active process can be stopped. It is generally assumed that if active force is discontinued, the root resorption will end. The determinants of the resorption/repair sequence are, however, not well understood. The aim of the present study was to register and analyse determinants that may influence the extent of resorbed root surface, as well as the transition of a process of active root resorption into a process of tissue deposition/repair in the resorption lacunae. Using an experimental mod

Root resorption and pulpal changes due to intrusive force.

Intrusion has been regarded as a controversial topic in the literature. It is apparent from many studies that excessive forces applied in orthodontic treatment might lead to undesirable results such as circulatory disturbances, pulp degeneration, calcification and even necrosis. This study was performed to observe the reaction of pulp and roots following tooth intrusion. Material consisted of four first premolar teeth of two adult patients. One premolar of each patient was intruded orthodontically and the other premolars were taken as controls. The palatal roots of the experimental teeth were

Root resorption after orthodontic treatment: Part 2. Literature review.

All permanent teeth may show microscopic amounts of root resorption that are clinically insignificant and radiographically undetected. Root resorption of permanent teeth is a probable consequence of orthodontic treatment and active tooth movement. The incidence of reported root resorption during orthodontic treatment varies widely among investigators. Usually, extensive resorption does not affect the functional capacity or the effective life of the tooth. Most studies agree that the root resorption process ceases once the active treatment is terminated. Root resorption of the deciduous dentiti

Microscopical aspects of root resorption of human deciduous teeth.

In order to obtain detailed information on the tissue changes which occur during physiological root resorption, 52 human deciduous teeth at various stages of resorption were studied under light microscopy. The early stage of root resorption was defined as resorption of not more than one third of the root length; the late stage was defined as resorption of more than one third. A close topographical interrelationship was found among che sites of pressure of the permanent tooth, the extent of root resorption and the types of tissue changes. Linear resorption (which reflects suspension or marked s

[Histological study on root resorption of upper permanent incisor].

In the permanent tooth, physiological root resorption does not occur, but inflammatory resorption occurs due to the orthodontic force, ectopic eruption of neighboring tooth and others. In this study, the morphological and histological investigation of the root resorption of the upper permanent incisors caused by the ectopic eruption of the canine was carried out. The left central incisor and lateral incisor from a 12-year-old female were examined. The roots were resorbed almost completely, and in part the resorption extended into the enamel. The results were as follows: 1. In the resorbed dent

Orthodontically Induced External Root Resorption: A Finite Element Analysis.

Background/Objectives: This finite element analysis (FEA) assessed stress distribution in the tooth and dentin within an intact periodontium under 4 N of force and five orthodontic movements (intrusion, extrusion, rotation, tipping, and translation), using four failure criteria commonly used in numerical dental studies. Secondly, differences between brittle- and ductile-like failure criteria were found, and the most accurate criterion was determined. Additionally, movements more prone to inducing external orthodontic root resorption were assessed. Methods: Using nine 3D models of the second lo

Denosumab and invasive cervical root resorption: a case report.

This case report describes an adverse side effect from long-term denosumab therapy in a patient with metastatic breast cancer. The patient presented with extensive invasive cervical root resorption affecting most of her dentition. There were no other identified risk factors. As the treatment of breast cancer evolves leading to improved survival, and the number of cancer survivors increases, it is expected that dental practitioners will see a growing number of patients who are on long-term denosumab. Comprehensive dental examinations including radiographs are warranted to identify unusual or un

TGF-β and Physiological Root Resorption of Deciduous Teeth.

The present study was performed to examine how transforming growth factor β (TGF-β) in root-surrounding tissues on deciduous teeth regulates the differentiation induction into odontoclasts during physiological root resorption. We prepared root-surrounding tissues with (R) or without (N) physiological root resorption scraped off at three regions (R1-R3 or N1-N3) from the cervical area to the apical area of the tooth and measured both TGF-β and the tartrate-resistant acid phosphatase (TRAP) activities. The TGF-β activity level was increased in N1-N3, whereas the TRAP activity was increased in R2
