Open Rotator Cuff Tear Repair Using Deltopectoral Approach.

BACKGROUND: The goal of this study was to evaluate the outcome of the open repair of rotator cuff tears via the deltopectoral approach in patients unable to afford arthroscopic repair costs. METHODS: We evaluated 80 consecutive patients who were treated for full-thickness rotator cuff tears by open repair through the deltopectoral approach. There were 48 men and 32 women at a mean age of 60.1 years (range, 35-80 years). Preoperative and postoperative clinical assessments were performed with the Constant score, American Shoulder and Elbow Surgeons (ASES) score, modified University of Californi

[Therapy of degenerative and posttraumatic rotator cuff lesions].

The prevalence of rotator cuff lesions is age-dependent and up to 19-32 % for full-thickness ruptures and 13-32 % for partial-thickness lesions respectively. The therapy of partial-thickness ruptures should be considered in accordance with the articular, bursal or intratendinous location of the lesion. The therapy of full-thickness ruptures should be applied in accordance with topography and area of defect, retraction, atrophy and fatty infiltration. These parameters are considered to be important prognostic factors for the intraoperative repairability and the success of the surgery. Symptomat

Human dermal allograft for massive rotator cuff tears.

Previously published studies reported variable results using various suture techniques and reconstruction options for massive rotator cuff tears. Therefore, the current authors retrospectively studied 21 consecutive patients/shoulders with massive rotator cuff tears treated from January 2005 to October 2011 with a human dermal allograft through a mini-open approach. Mean patient age was 58 years (range, 33-72 years). Mean follow-up was 29 months (range, 18-52 months). Ten patients underwent revision repair for a failed rotator cuff repair. The authors measured the tendon gap (mean, 1.7 cm) and

The role of acromioplasty for rotator cuff problems.

Acromioplasty is a well-described technique used throughout the wide spectrum of treatment options for shoulder impingement and rotator cuff pathology. Several randomized prospective studies have described clinical outcomes that are statistically similar when comparing patients undergoing rotator cuff repair either with or without concomitant acromioplasty. This article reviews the current evidence for use of acromioplasty in patients with subacromial impingement syndrome and during arthroscopic rotator cuff repair. Despite recently published studies, more long-term data, especially with regar

Human evolution and tears of the rotator cuff.

PURPOSE: Humans differ from other great ape species in their propensity to develop tears of the rotator cuff. The aim of this study was to compare the anatomical risk factors for subacromial impingement and rotator cuff tears amongst the great apes and to determine which features may be accentuated in humans and therefore play a more significant role in disease aetiology. METHODS: Orthogonal digital photographs of 22 human, 17 gorilla, 13 chimpanzee and 12 orangutan dry bone scapula specimens oriented in the glenoid plane were taken. Anatomical measurements were preformed using a calibrated d

Subdeltoid lipoma arborescens combined with rotator cuff tears.

Lipoma arborescens, a rare benign intra-articular lesion, is characterized by lipomatous proliferation of the synovium in which the subsynovial tissue is replaced by mature adipocytes. Subdeltoid bursa is a rare location for lipoma arborescens, and only a few cases have been reported in the literature. This article reports 2 cases of subdeltoid lipoma arborescens combined with rotator cuff tears, and the possible relationships between subdeltoid lipoma arborescens and rotator cuff tears are discussed. The intra-articular villous proliferations on fat-suppressed T2-weighted magnetic resonance i

Biologically based strategies to augment rotator cuff tears.

Lesions of the rotator cuff (RC) are among the most frequent tendon injuries. In spite of the developments in both open and arthroscopic surgery, RC repair still very often fails. In order to reduce the failure rate after surgery, several experimental in vitro and in vivo therapy methods have been developed for biological improvement of the reinsertion. This article provides an overview of the current evidence for augmentation of RC reconstruction with growth factors. Furthermore, potential future therapeutic approaches are discussed. We performed a comprehensive search of the PubMed database

Arthroscopic repair techniques for massive rotator cuff tears.

Patients with massive rotator cuff tears present with pain, weakness, and loss of function. Candidates for arthroscopic repair include symptomatic, young, active patients; those with an acute tear or tears with early changes of atrophy; and patients willing to comply with recovery and rehabilitation processes after surgery. As massive rotator cuff tears extend, the glenohumeral articulation is destabilized, allowing superior migration. Repair of the force couples and reinforcement of the anterosuperior rotator cuff cable can restore functional elevation via the deltoid. Muscle changes, includi

A mouse model of massive rotator cuff tears.

BACKGROUND: Rotator cuff tears are the most common tendon injury seen in orthopaedic patients. Muscle atrophy and fatty infiltration in rotator cuff muscles are considered among the key factors responsible for the failure of attempted repair of a massive rotator cuff tear. However, the pathophysiology of rotator cuff muscle atrophy and fatty infiltration remains largely unknown, partly because of the lack of appropriate small animal models. The goal of this study was to develop a mouse model of muscle atrophy and fatty infiltration after a rotator cuff tear. We also sought to study the role of

Suture number determines strength of rotator cuff repair.

BACKGROUND: Failure of surgical repair of a rotator cuff tear continues to be a clinical problem. For other tendon repairs, increasing the number of sutures improves both biomechanical performance and clinical outcomes. Several investigators have shown biomechanical advantages of double-row techniques but have used many more sutures than were used with the single-row techniques with which the double-row techniques were compared. The purposes of our study were to establish whether using a greater number of sutures would improve the biomechanical properties of a rotator cuff repair model, and wh

Humeral head osteonecrosis following arthroscopic rotator cuff repair.

Humeral head chondrolysis and osteonecrosis of the glenoid have been reported; however, there is no report to date about humeral head osteonecrosis following arthroscopic shoulder procedure. We report a case of osteonecrosis of the humeral head following arthroscopic rotator cuff repair what we believe is probably secondary to disruption of its blood supply after placement of multiple metallic suture anchors. The surgical records were also reviewed in an attempt to identify the cause of the humeral head osteonecrosis.

HYPOTHESIS: This study was conducted to identify preoperative factors correlating with limited motion after rotator cuff repair (RCR) and to evaluate the affect of loss of motion on outcome. We hypothesized that patients with preoperative ROM loss, diabetes, and workman's compensation claims would exhibit postoperative ROM loss at 3 months. MATERIAL AND METHODS: Preoperative and postoperative evaluations, including outcomes assessment and physical examination parameters, were reviewed for 345 patients who underwent RCR. Heterotopic ossification after minimally invasive rotator cuff repair.

Heterotopic ossification is a common phenomenon after spinal cord injury, head injury, neurologic disorders, burns and other trauma, and joint arthroplasty. Periarticular ossifications after shoulder surgery have been known to occur since the 19th century, at an incidence of up to 27%. After arthroscopic and minimally invasive shoulder surgical procedures were introduced and came into broad use, reports about heterotopic ossification became very rare. We describe here a case of heterotopic bone formation in the subdeltoid fascia after arthroscopic subacromial decompression, acromioclavicular j

Effect of rotator cuff pathology on shoulder rhythm.

The purpose of this study was to test the hypothesis that shoulder rhythm is affected by rotator cuff pathology during arm elevation. We divided 42 subjects into 3 groups: those with full-thickness rotator cuff tears (RCTs) (n = 14), those with tendinopathy (n = 13), and control subjects (n = 15). Shoulder kinematics was recorded while subjects performed elevation in the sagittal and scapular planes. Euler angles were computed for scapular and humeral elevation. Data were divided into 3 equal phases. Lines were fit and the slope determined for each phase. Significant differences in slopes ( P

Arthroscopic treatment of ruptures of the rotator cuff.

566 patients with shoulder disease were treated at the Clinic of Orthopaedics at the University Hospital Olomouc. Of these, 181 were operated for ruptures of the rotator cuff (RC). They were categorised according to the Gschwend classification, age and gender(1). Laterality of the operated limb was also evaluated. Patients with ruptures larger than 4 cm were treated by open operation. 84 shoulders with ruptures classified as Gschwend I, II and IIIa -52 males and 32 females-were treated arthroscopically. The age of the operated patients ranged from 40-60 years. In both genders, the right should

Arthroscopic evaluation and management of rotator cuff tears.

The development of arthroscopic techniques has provided the shoulder surgeon with new opportunities to improve the chances of salvaging these difficult rotator cuff tears while generally avoiding the potential disastrous consequences attendant to the open procedures. The arthroscope can be used to review and document the status of the joint and cuff, assess the tear pattern, debride damaged tissues, and smooth the acromion. In addition, a trained shoulder arthroscopist can mobilize the available tissues and repair any viable tendon to bone, without damage to the overlying muscles, through smal

Glenohumeral articular cartilage changes in rotator cuff disease.

This study aims to clarify the natural history of glenohumeral articular cartilage changes in rotator cuff disease. We assessed 33 cadaveric shoulder joints, examining the rotator cuffs for macroscopic tears and the articular surfaces for degenerative changes. Ten specimens had tears of the rotator cuff. Articular degenerative changes were noted in 23 joints. Articular cartilage degeneration was almost twice as frequent in the group with rotator cuff tears (10/10 shoulders) as in those without tears (13/23 shoulders). This difference was statistically significant. This strong correlation betwe

Rotator cuff alterations resulting from humeral head fractures.

Most complex humeral head fractures are associated with a longitudinal tear of the rotator cuff. Reconstruction of this tear is frequently omitted from surgical reduction and fixation procedures. In 48 patients treated by surgery or by conservative means, we investigated the relationship between rotator cuff integrity and the functional results. The rotator cuff was assessed by ultrasonography, the functional results by the Constant Score. Common signs of rotator cuff alterations were partial or complete atrophy. In all patients affected, a longitudinal defect located at the rotator interval w

Mini-open rotator cuff repair: an updated perspective.

The mini-open rotator cuff or arthroscopically assisted surgical procedure for repair of the rotator cuff has become a popular procedure with proven clinical results. The development of this procedure has followed a natural progression toward more minimally invasive means to accomplish rotator cuff repair. For many, it represents a middle ground between the traditional formal open repair and the newer complete arthroscopic rotator cuff repairs. As arthroscopic rotator cuff repair becomes more refined and accepted, the mini-open repair can also represent an excellent transitional technique to s
