Primary traumatic anterior shoulder dislocation in patients 40 years of age and older.

During three consecutive ski seasons (1991 to 1994), 125 patients 40 years of age and older without previous shoulder injuries or surgery sustained a traumatic first-time anterior shoulder dislocation. At a minimum of 2 years' follow-up, patients were contacted to determine long-term outcome and to identify factors leading to prolonged morbidity or the need for surgical intervention. Fifty-two patients were available for interview. A modified Rowe shoulder score showed 32 excellent, nine good, eight fair, and three poor results. Eighteen (35%) rotator cuff tears were subsequently identified, w

[Tendon ruptures of the shoulder].

Common sports, involving raising the arms above the head, i.e., throwing, racquet games and swimming, often result in rotator cuff tendinitis. During the throwing motion, the humeral head and its overlying biceps tendon and rotator cuff must pass rapidly under the coraco-acromial arch. Damage to these structures can occur by several mechanism. First, an increase in the size of the structures passing underneath the arch may lead to impingement. This can occur either by way of hypertrophy of the musculotendinous cuff or by way of inflammation of the cuff. Second, a decreased space available unde

Effect of rotator cuff muscle imbalance on forceful internal impingement and peel-back of the superior labrum: a cadaveric study.

BACKGROUND: Throwing athletes with shoulder pain have been shown to have decreased rotator cuff muscle strength. Shoulder internal impingement and labral peel-back mechanism, as may occur during the late cocking phase of throwing motion, are thought to cause rotator cuff injury and type II superior labrum anterior and posterior lesions. Therefore, the objective of this study was to assess the effect of rotator cuff muscle force on internal impingement and the peel-back of the superior labrum by quantifying maximum external rotation, glenohumeral contact pressure, and position of the cuff inser

Editorial Commentary: Static and Dynamic Synergy May Be the Evolving Role of Biceps and Tendon Transfers in the Treatment of Irreparable Rotator Cuff Tears.

The challenge of managing massive irreparable rotator cuff tears is not solved through technical complexity alone but through achieving balance between biological restoration and mechanical reinforcement. Superior capsule reconstruction using the long head of the biceps tendon offers static and dynamic advantages, whereas tendon transfers provide additional posterior force vectors to restore external rotation. The combined "static-dynamic" strategy may represent an evolving biologically based alternative for selected patients who are not candidates for arthroplasty.

Delamination in rotator cuff tears: Explanation of etiology through anatomical dissection.

The prognostic significance of delaminated rotator cuff tears remains controversial. However, as the surgical goal is to maximize the contact area between layers, the macroscopic appearance of partial delaminated rotator cuff tears is essential. The aim of this anatomical study was to investigate the morphology of delaminated rotator cuff tears. We hypothesized that delamination zones at the intersection of the supraspinatus and infraspinatus tendon fibers are the origin of articular-side degenerative rotator cuff tears. Forty anatomical specimens were evaluated in this study. The supraspinatu

Editorial Commentary: Subtle Degenerative Tears That So Commonly Involve the Upper Third of the Subscapularis Are Oftentimes Missed.

Subtle degenerative tears that so commonly involve the upper third of the subscapularis are oftentimes missed. However, there are physical examination, imaging, and arthroscopic findings that are highly suggestive of subscapularis tearing. Positive belly-press and bear hug tests, a thinner and longer subscapularis tendon, subscapularis muscle, belly loss of bulk or fatty infiltration, narrow coracohumeral distance, and biceps pathology are indicative. Artificial intelligence-based predictive algorithms can estimate the likelihood of subscapularis tearing based on a combined analysis of these a

Shoulder arthralgia: case study of the Occupational Medicine clinic of Universidade Estadual de Campinas (Unicamp).

INTRODUCTION: Shoulder pain is ranked as the third most common musculoskeletal complaint in clinical practice. It is estimated that 65 to 70% of these occurrences are due to rotator cuff injuries. A significant number of rotator cuff syndrome cases are work related. OBJECTIVES: To evaluate the success or failure of therapeutic and administrative procedures for workers treated at an occupational medicine outpatient clinic. METHODS: This study analyzed the medical reports of 142 workers treated for shoulder pain between January 2015 and December 2019. To homogenize the information, medical rec

Subacromial Bursa: A Neglected Tissue Is Gaining More and More Attention in Clinical and Experimental Research.

The subacromial bursa has long been demolded as friction-reducing tissue, which is often linked to shoulder pain and, therefore, partially removed during shoulder surgery. Currently, the discovery of the stem cell potential of resident bursa-derived cells shed a new light on the subacromial bursa. In the meanwhile, this neglected tissue is gaining more attention as to how it can augment the regenerative properties of adjacent tissues such as rotator cuff tendons. Specifically, the tight fibrovascular network, a high growth factor content, and the large progenitor potential of bursa-derived cel

Therapeutic potential of exosomes in rotator cuff tendon healing.

Rotator cuff tears are common musculoskeletal injuries that can cause significant pain and disability. While the clinical results of rotator cuff repair can be good, failure of tendon healing remains a significant problem. Molecular mechanisms underlying structural failure following surgical repair remain unclear. Histologically, enhanced inflammation, disorganization of the collagen fibers, calcification, apoptosis and tissue necrosis affect the normal healing process. Mesenchymal stem cells (MSCs) have the ability to provide improved healing following rotator cuff repair via the release of m

A Novel Arthroscopic Medial Knot-Tying Suture-Bridge Repair with Rip-Stop Technique for Rotator Cuff Tears.

Rotator cuff tears remain one of the most common injuries encountered by shoulder surgeons. Although arthroscopic suture-bridge repair has demonstrated superior biomechanical properties and achieved promising clinical results, tendon retears remain a persistent challenge. To address this issue, we have introduced several refinements to optimize the existing repair techniques. One such refinement is the incorporation of a medial knot in the suture-bridge repair, which can decrease gap formation and enable a watertight repair, thereby improving tendon-bone healing. Another critical modification

Graft Treatment for Rotator Cuff Tendon-Bone Interface Augmentation: Status and Prospects-A Narrative Review.

Sutures and suture anchors are commonly used in rotator cuff repairs as they offer cost-effective and mechanically robust solutions for tendon-bone interface (TBI) healing. However, in large to massive rotator cuff tears, where substantial tendon loss and limited intrinsic healing potential are present, conventional repair techniques alone often fail to restore the native biomechanics and structural integrity. Consequently, retear rates in these cases remain unacceptably high. This review summarizes recent advances in graft-based augmentation strategies aimed at improving outcomes in these cha

Editorial Commentary: Interposition Graft Bridging Reconstruction: Good Mid-Term Outcomes for Massive Irreparable Rotator Cuff Tears.

Massive irreparable rotator cuff tears without glenohumeral arthritis are a common cause of shoulder pain and disability. Many surgical treatment options have been proposed, including debridement, partial repair, tendon transfer, superior capsule reconstruction, balloon spacer placement, bursal acromial reconstruction, and reverse shoulder arthroplasty. Interposition graft bridging reconstruction, as evidenced by the mid-term results of the current study, may also be considered, at least for now. However, let's see if this procedure will truly stand the test of time because all orthopaedic sur

Editorial Commentary: The Long Head of the Biceps Brachii Tendon-The Shoulder Surgeon's Conundrum.

The long head of the biceps tendon can be a significant pain generator in the setting of rotator cuff tearing. Surgeons who encounter long head of biceps tendon pathology at the time of rotator cuff repair can be confident in a high likelihood of statistically significant clinical improvement with concomitant long head of biceps surgery. Patients can be reliably counseled preoperatively that if they have both clinically apparent long head of biceps tendon pathology and rotator cuff tearing, and if they undergo long head of biceps tendon surgery with rotator cuff repair, they can expect compara

Outcomes of arthroscopic versus open rotator cuff repair: a systematic review of the literature.

Full-thickness rotator cuff tears are common. When symptomatic, they can affect quality of life. Surgical repair might improve patients' overall health. We systematically reviewed postoperative outcomes in 10 studies comparing mini-open repair and all-arthroscopic repair techniques. Data regarding patient demographics, rotator cuff pathology, postoperative rehabilitation protocols, American Shoulder and Elbow Surgeons (ASES) scores, University of California Los Angeles (UCLA) scores, pain scores, and incidence of recurrent defects were extracted. There were no statistically significant differe
