A Quantitative Approach for the Objective Assessment of Coupling Efficiency for an Active Middle Ear Implant by Recording Auditory Steady-state Responses.

OBJECTIVE: The coupling efficiency of a semi-implantable active middle ear implant with an electromagnetically driven floating mass transducer coupled to a middle ear ossicle or the round window can only be quantified postoperatively in cooperative patients by measuring behavioral vibroplasty in situ thresholds in comparison with bone conduction thresholds. The objective of the study was to develop a method to objectively determine the vibroplasty in situ thresholds by determining calibration factors from the relation between the objective and behavioral vibroplasty in situ thresholds. Influence of backside loading on the floating mass transducer: An in vitro experimental study.

HYPOTHESIS: The vibration of the floating mass transducer (FMT) of a single active middle-ear implant (AMEI) is distinctly influenced by the properties of the material coupled to its back side. BACKGROUND: In round window vibroplasty, the FMT needs to be padded against the surrounding bone opposite from the round window membrane. This represents one factor influencing its performance as a round window driver. Therefore, we examined the effects of different materials linked to the back side of an FMT on its vibration range. METHODS: The back side of an FMT was glued to a silicone cylinder 1.0

The Codacs™ direct acoustic cochlear implant actuator: exploring alternative stimulation sites and their stimulation efficiency.

This work assesses the efficiency of the Codacs system actuator (Cochlear Ltd., Sydney Australia) in different inner ear stimulation modalities. Originally the actuator was intended for direct perilymph stimulation after stapedotomy using a piston prosthesis. A possible alternative application is the stimulation of middle ear structures or the round window (RW). Here the perilymph stimulation with a K-piston through a stapes footplate (SFP) fenestration (N = 10) as well as stimulation of the stapes head (SH) with a Bell prosthesis (N = 9), SFP stimulation with an Omega/Aerial prosthesis (N = 8

Intratympanic gentamicin for the treatment of Meniere's disease and other forms of peripheral vertigo.

Titration therapy with intratympanic gentamicin offers class A or B control of vertigo in 87% (range, 75%-100%) of patients with unilateral Ménière's disease. The risk of additional hearing loss is about 21% (range,0-37%). Vertigo may recur, however, in nearly one third of patients over time. These recurrences can also be treated by intratympanic gentamicin with a similar risk of hearing loss. The salient effect of intratympanic gentamicin is probably the reduction of vestibular function through damage to hair cells, but a complete ablation of function does not seem to be necessary for vertigo

Investigations on the critical perilymphatic pressure value causing round window membrane rupture in anesthetized cats.

Increasing perilymphatic fluid pressure was found to be an important factor in the etiology of round window membrane rupture. The critical pressure, causing membrane rupture was determined in anesthetized cats. Its value was found to be in the range 10--30 mmHg (x+/-S.E.M.=23.4+/-17.1). This value was compared with the pressure increases caused by thoracic and abdominal compression, cervical strangulation, forced Trendelenburg position, coughing and sneezing.

Perilymphatic fistulas: can we predict the diagnosis?

This study intends to identify factors that could better predict the diagnosis of perilymphatic fistula (PLF) since exploration surgery is currently the only confirmatory method. This retrospective chart review in a tertiary care center is based on all 71 available patient files operated for a suspicion of PLF between 1983 and 2012. History of predisposing factors, clinical findings and investigations were documented pre- and postoperatively. Patients were divided according to intraoperative findings into two groups: group I (fistula negative) and group II (fistula positive). In addition, grou

Evaluation of Intracochlear Trauma Caused by Insertion of Cochlear Implant Electrode Arrays through Different Quadrants of the Round Window.

HYPOTHESIS: This study aimed to evaluate whether there is a difference in the degree of intracochlear trauma when the cochlear implant electrode arrays is inserted through different quadrants of the round window membrane. BACKGROUND: The benefits of residual hearing preservation in cochlear implant recipients have promoted the development of atraumatic surgeries. Minimal trauma during electrode insertion is crucial for residual hearing preservation. METHODS: In total, 25 fresh human temporal bones were subjected to mastoidectomy and posterior tympanotomy. The cochlear implant electrode array

Retrofacial approach of cochlear implantation in inner ear malformation with aberrant facial nerve: a case report.

Cochlear implantation is regarded as a safe surgery for young children with minimal complications. However, inner ear malformations and aberrant course of facial nerves may impede electrode insertion via the round window approach and increase the risk of iatrogenic facial nerve injury. We report a case of cochlear incomplete partition in a patient with anomalous facial nerve anatomy. The anterior and inferior displacement of the facial nerve obscured the round window. A retrofacial approach was used to expose the round window and the electrode was inserted successfully. No surgical complicatio

Instrumental perforation of the round window. Animal experiments using cochleography and ERA.

The round window membrane of the inner ear of the guinea pig was perforated under Ketanest anaesthesia. A very rapid and almost total loss of cochlear microphonics, auditory nerve action potential and brain-stem response developed. Replacement of the perilymph with Ringer's solution and surgical closure of the membrane damage had no effect on auditory nerve action potential or brain-stem response. Further decline in the amplitude of cochlear microphonics was halted, however. When perfusion of the round window membrane was carried out whilst the bulla was full with Ringer's solution, no essenti

Case report: Perilymphatic fistula from a round window microfissure.

A microfissure near the round window niche is an anatomical structure that communicates between middle ear and the ampulla of the posterior semicircular canal. It has been suggested that the microfissure can cause inner ear symptoms; however, the etiology has not yet been confirmed clinically. We report, to our knowledge, the first case of microfissure with complaint of hearing loss and vertigo and improvement in hearing after surgical sealing of the microfissure. A 50-year-old man complained of hearing disturbance, tinnitus with flowing-water sound in the left ear, and a floating sensation up

[Implantation of a vibratory mass transducer on the round window: a report of two cases].

The Vibrant Soundbridge (VSB, Med-El, Austria) is a semiimplantable hearing aid usually attached to the long process of the incus to vibrate the ossicular chain in patients with moderate to severe mixed hearing loss. We implanted a VSB vibratory transducer on the round window membrane of the left ear in two cases not treated effectively by tympanoplastic surgeries. Pure tone audiography did not differ significantly in the two cases pre- or postoperatively, indicating that the small mass transducer does not adversely affect middle-ear vibration. Postoperative hearing thresholds with VSBs were s

Imaging of congenital middle ear deafness.

INTRODUCTION: Conductive hearing loss, in the presence of a normal external ear, is usually due to an interference with the normal transmission of sound from the tympanic membrane across the ossicular chain to the oval window. If conductive hearing loss occurs in a young child, congenital middle ear pathology is the most likely cause. MATERIALS AND METHODS: High resolution computed tomography of the middle ear is the diagnostic modality of choice. Details of the imaging technique are outlined in this article. Eight cases of congenital middle ear deafness imaged in the radiological departments

Optimal Head Position Following Intratympanic Injections of Steroids.

BACKGROUND: Intratympanic drug applications may be performed for a number of reasons, including the treatment of Meniere's disease, sudden hearing loss, and tinnitus. In this study, the aim was to determine the optimal head position for steroid solutions to be absorbed through the round window after intratympanic injections. METHODS: The authors retrospectively reviewed the best head position where the level of steroid solution reaches the round window area behind the eardrum in video recordings of 20 Meniere's disease patients undergoing intratympanic steroid injections. The patients were po

Congenital perilymphatic fistula: computed tomography appearance of middle ear and inner ear anomalies.

Congenital perilymphatic fistula is an abnormal communication between the inner ear and middle ear. Inner ear anomalies have been described on computed tomography scans. Middle ear anomalies have been found at surgery; the most frequent are anomalies of the stapes and round window. This retrospective study describes the appearance of the inner and middle ear on computed tomography scans, and of the middle ear at surgery, in 10 patients (15 ears) in whom perilymphatic fistula was found at surgery. Twelve of 15 stapes were abnormal at surgery; 4 of these 12 (33%) could be seen on computed tomogr

Histologic variants in otosclerosis.

A study of 164 temporal bones with otosclerosis revealed stapes fixation in 123. Large lesions (4 mm or larger) were characterized by involvement of the cochlear endosteum, increased cellular activity, and bone resorption. Anatomical sites of predilection were: 1. anterior to the oval window, 2. margins of the round window, and 3. apical-medial wall of the cochlea. Two of the five ears with cochlear otosclerosis were from a patient with severe bilateral hearing loss. The size, cellular activity, and location of otosclerotic lesions showed no correlation with the magnitude of sensorineural hear
