Incorporating Community Case Management in Risk-Based Surveillance for Malaria Elimination in the Dominican Republic.

As countries strive for malaria elimination, it is crucial to gather sufficient evidence to confirm the absence of transmission. Routine surveillance data often lack the sensitivity to detect community transmission at low levels. In the Dominican Republic, community health workers (CHWs) have been deployed in malaria foci to perform active case detection. This study aimed to assess the added value of CHWs in enhancing the health system's malaria detection capabilities. Freedom from infection (FFI) is a statistical framework designed to demonstrate the absence of malaria by using routinely coll

Monitoring epidemiologic surveillance data using hidden Markov models.

The analysis of routinely collected surveillance data is an important challenge in public health practice. We present a method based on a hidden Markov model for monitoring such time series. The model characterizes the sequence of measurements by assuming that its probability density function depends on the state of an underlying Markov chain. The parameter vector includes distribution parameters and transition probabilities between the states. Maximum likelihood estimates are obtained with a modified EM algorithm. Extensions are provided to take into account trend and seasonality in the data.

Healthcare Big Data in Hong Kong: Development and Implementation of Artificial Intelligence-Enhanced Predictive Models for Risk Stratification.

Routinely collected electronic health records (EHRs) data contain a vast amount of valuable information for conducting epidemiological studies. With the right tools, we can gain insights into disease processes and development, identify the best treatment and develop accurate models for predicting outcomes. Our recent systematic review has found that the number of big data studies from Hong Kong has rapidly increased since 2015, with an increasingly common application of artificial intelligence (AI). The advantages of big data are that i) the models developed are highly generalisable to the pop

Clinical information has low sensitivity for postmortem diagnosis of heart valve disease.

BACKGROUND: Accuracy of routinely collected information concerning cause of death is essential for public health and health systems planning. Since clinical examination has relatively low sensitivity for detection of valvular heart disease (VHD), mortality data based on clinical information alone might routinely underestimate the number of deaths due to VHD. METHODS: We compared autopsy findings against premortem clinical information for 8198 consecutive adult postmortems (mean age 69.1 years, 61.3% men), performed in a single UK tertiary referral centre with on-site cardiac surgical faciliti

Guiding principles for pragmatic randomised trials of tuberculosis treatments.

Tuberculosis remains a major global health challenge, despite recent advances in drug and regimen development. Pragmatic randomised trials are needed to evaluate the effectiveness, safety, and tolerability of new tuberculosis treatments under routine care conditions to appropriately inform practice guidelines and facilitate uptake. In this Review, we propose guiding principles for pragmatic tuberculosis treatment trials. Using the PRECIS-2 framework, we address eligibility, recruitment, setting, organisation of care, adherence support, outcomes, follow-up, primary analysis, data collection, an

HealthCare Complexity Index (HCIn): A Clinical Decision-Making Tool for Primary Care.

BACKGROUND: Population ageing, multimorbidity, and chronic conditions have increased the demand for person-centred care in Primary Health Care. Current stratification systems rely on diagnoses or resource utilization and fail to capture the multidimensional nature of care complexity, which also depends on functional, social, environmental factors. OBJECTIVE: To develop a HealthCare Complexity Index using routinely collected electronic health record (EHR) data and the Person-Centred Care Knowledge Model. METHODOLOGY: A retrospective, cross-sectional observational study was conducted in Primar

Strengths and limitations of non-survey-based data sources for assessing adult vaccination coverage in the United States.

INTRODUCTION: Non-survey-based data sources (e.g. electronic health records, administrative claims) have been used to estimate vaccination coverage among US adults. However, these data sources were not collected for research or surveillance purposes and may have substantial limitations. The objectives of this narrative review were to: 1) identify published studies that used non-survey-based data sources to estimate adult vaccination coverage for one or more routinely recommended vaccines; and 2) summarize the strengths and limitations of these data sources for coverage assessments. Commentary: Enhancing Health Data for Systems Change to Prevent Homelessness in Canada.

Housing is a critical social determinant of health, yet until recently, hospitals did not routinely collect data on homelessness. National homelessness coding mandates have brought attention to an often poorly represented issue, but largely reaffirm existing evidence - homelessness is a growing public health issue. Governments and health systems should not just document the ongoing crisis but also leverage administrative data to prevent homelessness. Policies aimed at early detection, cross-sectoral data integration and affordable housing are needed to address the upstream drivers of homelessn

Nowcasting methods to improve the performance of respiratory sentinel surveillance: lessons from the COVID-19 pandemic.

Respiratory diseases, including influenza and coronaviruses, pose recurrent global threats. This study delves into the respiratory surveillance systems, focusing on the effectiveness of SARI sentinel surveillance for total and severe cases incidence estimation. Leveraging data from the COVID-19 pandemic in Chile, we examined 2020-2023 data (a 159-week period) comparing census surveillance results of confirmed cases and hospitalizations, with sentinel surveillance. Our analyses revealed a consistent underestimation of total cases and an overestimation of severe cases of sentinel surveillance. T

Graduate Student Literature Review: Detecting health disorders using data from automatic milking systems and associated technologies.

This review synthesizes a range of research findings regarding behavioral and production responses to health disorders and subsequent illness detection for herds using automatic (robotic) milking systems (AMS). We discuss the effects of health disorders on cow behavior and production, specifically those variables that are routinely recorded by AMS and associated technologies. This information is used to inform the resultant use of behavior and production variables and to summarize and critique current illness detection studies. For conventional and AMS herds separately, we examined research fr

Characteristics and outcomes of people in suicidal crisis at two emergency departments: a service evaluation.

Emergency departments (EDs) provide critical opportunities for nurses to support suicide prevention. This article details a service evaluation that was undertaken to explore the characteristics and outcomes of people in suicidal crisis at two EDs in the East of England during June 2023. Data routinely collected by the ED mental health liaison team were combined with a retrospective case note review of the local NHS mental health trust's electronic patient records. Attendees had a mean age of 35 years and seven months, and were often diagnosed with depression or emotionally unstable personality

Early sepsis prediction in the emergency department using machine learning.

INTRODUCTION: Early recognition of sepsis is critical for timely intervention. Early intervention (eg. with antibiotics) can reduce the risk on organ injury and eventually death. Commonly used clinical scores, such as qSOFA and NEWS, have limited predictive performance to predict organ failure and mortality in patients with early sepsis. Moreover, they require to be estimated by a human operator and lack the capacity to use diverse input sources. Machine learning (ML) models may improve risk stratification by integrating a broader range of parameters. METHODS: This study is a post-hoc analysi

The Liverpool Heart And bRain Project (L-HARP): Protocol for an Observational Cohort Study of Cardiovascular Risk and Outcomes Following Stroke.

Further research is needed to refine risk prediction models for adverse cardiovascular outcomes following stroke in contemporary clinical practice, such as incident atrial fibrillation (AF), recurrent stroke, and cognitive impairment and dementia. The aims of this study are to prospectively investigate cardiovascular outcomes and risk factors for incident cardiovascular disease in a post-stroke cohort, and to externally validate, refine and expand current risk prediction models for cardiovascular and cardiovascular-related outcomes. The study sample size was based on the development of post-st

Exploration of gender differences of Chlamydia trachomatis infection amongst young people reveals limitations of using sexual histories to assess risk in high-prevalence areas.

In England, chlamydia positivity in young men occurs at a slightly older age group (20-24 years) than positivity among young women (16-20 years) but total rates of infection among the population aged under 25 years are similar. Where there is variation, explanations often focus on individual sexual risk behaviours. We aimed to explore the extent to which variations in chlamydia positivity could be explained by reasons for attendance and sexual behaviour at a sexual health clinic in a high-prevalence area of England. Data routinely collected during clinic appointments were extracted retrospecti

Application of behavior data to predictive exploratory models of metritis self-cure and treatment failure in dairy cows.

The objective was to evaluate the performance of exploratory models containing routinely available on-farm data, behavior data, and the combination of both to predict metritis self-cure (SC) and treatment failure (TF). Holstein cows (n = 1,061) were fitted with a collar-mounted automated-health monitoring device (AHMD) from -21 ± 3 to 60 ± 3 d relative to calving to monitor rumination time and activity. Cows were examined for diagnosis of metritis at 4 ± 1, 7 ± 1, and 9 ± 1 d in milk (DIM). Cows diagnosed with metritis (n = 132), characterized by watery, fetid, reddish/brownish vaginal dischar

Joint modeling of multiple longitudinal biomarkers and survival outcomes via threshold regression: variability as a predictor.

Longitudinal biomarker data and health outcomes are routinely collected in many studies to assess how biomarker trajectories predict health outcomes. Existing methods primarily focus on mean biomarker profiles, treating variability as a nuisance. However, excess variability may indicate system dysregulations that may be associated with poor outcomes. In this paper, we address the long-standing problem of using variability information of multiple longitudinal biomarkers in time-to-event analyses by formulating and studying a Bayesian joint model. We first model multiple longitudinal biomarkers,

Subtyping Alzheimer's disease and Parkinson's disease using longitudinal electronic health records.

Neurodegenerative diseases such as Alzheimer's disease (AD) and Parkinson's disease (PD) are clinically heterogeneous, hampering the success of nonselective treatment strategies. Here we apply a transformer-based unsupervised clustering framework to longitudinal electronic health record data from over 100,000 patients across two UK cohorts, Clinical Practice Research Datalink Aurum and UK Biobank, to identify, validate and characterize subtypes of AD and PD. We uncover five reproducible subtypes for each condition, characterized by distinct comorbidity patterns, symptom trajectories, outcomes
