A critical link between heart failure self-care and intrathoracic impedance.

BACKGROUND: Effective self-care is regarded as essential to the management of heart failure (HF). The influence of self-care on HF decompensation, however, is not well understood. Accordingly, we examined the relationship between self-care and fluid accumulation accompanying worsening HF as indexed by decreasing intrathoracic impedance (Z). METHODS: Z data were collected from 58 HF patients with OptiVol enabled devices (Medtronic Inc, Minneapolis, Minnesota). Heart failure self-care was measured with the European Heart Failure Self-care Behaviour Scale. Regression modeling was used to describ

Do self-reported 'integrated' continence services provide high-quality continence care?

INTRODUCTION: systematic collection of clinical outcome data remains the most difficult task in the measurement of clinical effectiveness. However, the examination of the relationship between organisational and clinical process of care may provide a surrogate measure of quality in care. METHODS: data from the 2006 National Audit of Continence Care for Older People were used to examine whether there was an association between organisational structure and standard of continence care for older people. 'Quality' scores were produced and the relationship between scores was examined. RESULTS: ther

Beyond Compliance: The Role of Self-Efficacy in Foot Care and Self-Management Among Patients With Type 1 Diabetes.

Background: Effective self-management behaviors are crucial for diabetes management. This study examines the mediating role of self-efficacy in the relationship between compliance dimensions and self-management activities in patients with Type 1 diabetes. Methods: The current study explores a baseline analysis from a randomized controlled trial but the intervention's effectiveness is not reported here; the analysis focuses on elucidating potential mediating factors at baseline. The study investigated the relationships between seven compliance dimensions (treatment effort, intention, adaptabili

Assessing the feasibility of integration of self-care for filarial lymphoedema into existing community leprosy self-help groups in Nepal.

BACKGROUND: Lymphatic filariasis (LF) and leprosy are disabling infectious diseases endemic in Nepal. LF infection can lead to lymphoedema and hydrocoele, while secondary effects of leprosy infection include impairments to hands, eyes and feet. The disabling effects of both conditions can be managed through self-care and the supportive effects of self-help groups (SHGs). A network of SHGs exists for people affected by leprosy in four districts in Nepal's Central Development Region, however no such service exists for people affected by LF. The aim of this study was to determine the feasibility

Still I rise: The need for self-validation and self-care in the midst of adversities faced by Black women with HIV.

OBJECTIVES: Psychosocial factors of trauma and abuse, racial discrimination, HIV stigma, and gender-related stressors (e.g., prioritizing others' needs) have been associated with antiretroviral treatment (ART) nonadherence and poor viral suppression among Black women living with HIV (BWLWH). To inform the development of an intervention addressing these psychosocial factors to improve ART adherence, the authors sought the insight of BWLWH. METHOD: Qualitative semistructured interviews were conducted with 30 BWLWH to gather information on their experiences with trauma, racism, HIV stigma, gende

The impact of initiatives in education, self-management training, and computer-assisted self-care on outcomes in diabetes disease management.

The purpose of this work is to elucidate the advantages and disadvantages arising from three distinct diabetes disease management initiatives in a managed care setting. The initiatives included (1) education alone, (2) education with self-management training, and (3) education with computer-assisted self-care. Outcomes of interest were changes in glycated hemoglobin (HbA1c), body weight, and costs of care in each cohort of care recipients. A total of 978 health plan members with diabetes within a mixed model HMO were included in the initiatives for improving blood glucose control. HbA1c was me

Power Within: a meta-analysis of the roles of self-efficacy in type 2 diabetes self-care and glycaemic control.

Self-efficacy is a critical psychological factor in managing type 2 diabetes (T2D), yet findings across populations and measurement tools remain inconsistent. This meta-analysis investigates the associations between self-efficacy, self-care behaviours and glycaemic control (HbA1c), accounting for diabetes duration, geographic region and the type of self-efficacy scale used. A systematic search of six databases from 2007 to 2024 identified studies examining self-efficacy in relation to self-care and HbA1c in adults with T2D. Random effects models were used to pool data from 62 studies (n=19 231

Exploring home care nurses' perceived competence and self-efficacy in palliative care delivery: A cross-sectional study.

BACKGROUND: Integration of home-based palliative care (PC) enables patients to receive care at home, fosters family involvement, and reduces healthcare costs. Despite its benefits, nurses report challenges in delivering competent PC, and limited research has explored how home care nurses perceive their own competence and self-efficacy within this context. OBJECTIVES: The study aimed to explore Ontario nurses' perceived competence and self-efficacy in home-based PC delivery. It also examined the relationship between both constructs, perceived competence and self-efficacy. METHODS: A cross-sec

The use of self-expanding silicone stents in esophageal cancer care: optimal pre-, peri-, and postoperative care.

BACKGROUND: Preoperative nutritional supplementation, management of esophageal leaks, and postoperative anastomotic strictures still remain common problems in the management of esophageal cancer. Jejunal feeding tubes, total parenteral nutrition (TPN) with nasogastric suction, and repeated esophageal dilations remain the most common treatments, respectively. The aim of this study was to evaluate the use of removable silicone stents in (1) the preoperative nutritional optimization during neoadjuvant therapy, (2) the management of perioperative anastomotic leak, and (3) the management of postope

The roles of diabetes-specific self-compassion, affect balance, and self-care self-efficacy in psychological distress among Hong Kong patients with Type 2 diabetes.

OBJECTIVE: Patients with Type 2 diabetes (T2D) often blame themselves for their conditions and not reaching their self-management goals. Guided by the self-regulation resource model, self-compassion facilitates adjustments to chronic diseases through increasing affective and cognitive resources like positive affect and self-care self-efficacy. However, the contributions of positive and negative self-compassion (PSC and NSC) in psychological adjustment among patients with T2D have not been examined. Studies also suggest that patients with T2D experience positive changes from their illness journ
