Therapeutic hypothermia and temperature management2021eng
Targeted temperature management (TTM) directly impacts energy expenditure via temperature modulation and shivering associated with temperature modulating devices. We hypothesized that resting energy expenditure (REE) can be accurately estimated utilizing data obtained from a surface gel pad temperature modulating device (TMD) and demographic factors. Baseline demographic data, along with concurrent temperature, sedation, and shivering data, and indirect calorimetry (IDC) were collected from patients undergoing TTM. The data from the IDC and temperature modulation device (TMD) were synchronized

BACKGROUND: Inducing normothermia with surface cooling temperature modulating devices (TMDs) is cumbersome and often associated with significant shivering. We tested the safety and feasibility of a novel transnasal evaporative cooling device to induce and maintain normothermia in febrile patients following ischemic and hemorrhagic stroke.
METHODS: A single-center study utilizing the CoolStat® transnasal cooling device was used to achieve core temperature reduction in mechanically ventilated stroke patients with fever (T ≥ 38.3 C) refractory to acetaminophen by inducing an evaporative cooling

Orthognathic surgery presents a unique perioperative landscape for plastic surgeons and anesthesiologists alike including blood loss and pain control, as well as postoperative complications such as shivering, nausea and vomiting, emergence agitation, and prolonged ventilation. Blood loss is a major concern due to the inherent rich vascularity of the facial skeleton and significant muscular and bone marrow dissection. Effective hemostatic strategies are critical in preventing excessive blood loss, with the 2 primary strategies being judicial hypotensive anesthesia and use of hemostatic agents.

BACKGROUND: Hypothermia increases bleeding during surgery, risk of ischemic heart disease and postoperative wound infection. Intravenous amino acid increases cell synthesis and produces heat. Our goal was evaluating of the effect of amino acid on intraoperative hypothermia under spinal anesthesia.
METHODS: This is a randomized, double-blinded clinical trial that 36 adults undergoing Hip Arthroplasty were randomly assigned into two groups of 18 each. One group received Amino Acids solution (Aminoven 10%) 500ml (240ml/h) throughout spinal anesthesia, and control group received saline solution.

In an attempt to obtain more conclusive data, especially concerning the condition in many cases of very high alcohol concentration, two groups of healthy volunteers were exposed to controlled cold surroundings in a climate chamber after i.v. infusion of 96% ethyl alcohol, 1-2 1/2 ml per kg bodyweight, supplemented with alcohol perorally; the maximum blood alcohol concentration measured was 57 mmol/l, corresponding to 2.62%. One group was not habituated to drinking; the other group was habituated to heavy drinking, but functioned well socially. During the stay in a neutral environment, the alco

1. The effects on body temperature of a number of substances injected into a lateral cerebral ventricle were examined in the unanaesthetized cat.2. Nicotine (50 and 100 mug) caused a fall in body temperature ranging from 0.95-2.1 degrees C associated with skin vasodilatation, tachypnoea and panting. These responses were prevented by the intraventricular injection of hexamethonium or mecamylamine, but not of atropine or phentolamine.3. Carbachol (5 mug) caused a rise in temperature associated with skin vasoconstriction, piloerection and severe shivering. These effects were prevented by the intr

A 65-year-old female patient with major depressive disorder suffered from clonus, shivering and impaired visual acuity after 20 mg/day of paroxetine administration. The symptoms were initially regarded as further manifestations of her somatic symptoms of depression, and paroxetine was increased to 30 mg/day resulting in frequent clonus, increased shivering, serious dysarthria, ongoing impairment in visual acuity and agitation. These symptoms subsided upon paroxetine discontinuation. Ten mg/day of paroxetine rechallenge provoked dysarthria, tremor and headache, but these symptoms improved again

BACKGROUND: Spinal anesthesia induces hemodynamic disturbances such as hypotension and bradycardia. Granisetron, a 5-HT₃ receptor antagonist primarily used for antiemesis, has been proposed to mitigate these effects.
METHODS: This meta-analysis of randomized controlled trials evaluated intravenous granisetron versus placebo/no treatment in adults undergoing elective surgery under spinal anesthesia, reporting hypotension (primary outcome), bradycardia, shivering, hemodynamic changes, and vasopressor use. A random-effects model was used to calculate relative risks (RR) or mean differences (MD)

PURPOSE: This retrospective cohort study aimed to investigate the association between intraoperative dexmedetomidine infusion and emergence agitation (EA), and recovery profiles after lung surgery in adult patients. It was hypothesized that dexmedetomidine was associated with reduced EA and improved recovery profiles.
PATIENTS AND METHODS: A single-center chart review was conducted on elective lung surgeries in adults between January and December 2016. The primary outcome was the incidence of EA in postanesthesia care units (PACUs). The secondary outcomes included rescue analgesia, shivering,

Birds primarily rely on fat for energy during fasting and to fuel energetically demanding activities. Proteins are catabolized supplemental to fat, the function of which in birds remains poorly understood. It has been proposed that birds may increase the catabolism of body protein under dehydrating conditions as a means to maintain water balance, because catabolism of wet protein yields more total metabolic and bound water (0.155·H(2)O(-1)·kJ(-1)) than wet lipids (0.029 g·H(2)O(-1)·kJ(-1)). On the other hand, protein sparing should be important to maintain function of muscles and organs. We us

BACKGROUND/OBJECTIVES: Brown adipose tissue (BAT) activation has important metabolic health implications, yet the relationship between habitual dietary intake and BAT activity in humans remains to be fully understood.
METHODS: We compared dietary intake among adult men with (BATpositive, age: 34.8 ± 5.4 years, BMI: 28.2 ± 5.3 kg/m2, n = 12) and without (BATnegative, age: 39.1 ± 4.1 years, BMI: 31.1 ± 6.7 kg/m2, n = 11) cold-induced BAT activation. Activation of BAT was measured immediately following 2 h of cold exposure using 18F fluorodeoxyglucose positron emission tomography and computed to

Thermoregulation is an essential yet incompletely understood homeostatic process in mammals. UCP1-mediated thermogenesis, while efficient, is dispensable, suggesting the existence of alternative mechanisms. Using a pharmacogenetic approach, we show that the adipose tissue futile lipid cycling (FLC) contributes to UCP1-independent thermogenesis, with DGATs being involved in the regulation of FLC. The loss of DGAT-driven FLC-mediated thermogenesis is compensated for by the hierarchical recruitment of alternative mechanisms such as shivering and enhanced lipid catabolism mediated by AMPK activati

BACKGROUND: Perioperative hypothermia is common during liposuction and can lead to delayed recovery, shivering, and patient discomfort. Although electric warming (EW) systems are effective, their use may be limited in certain settings.
OBJECTIVES: The authors of this study aim to evaluate the effectiveness of reflective blankets (RBs) in maintaining core temperature during liposuction compared with EW and no warming.
METHODS: In this randomized controlled trial, 90 patients undergoing elective liposuction under general anesthesia were assigned to 3 groups (n = 30): RB, EW, and control (no wa

Post-operative nausea and vomiting (PONV) is an event of multifactorial origin with an incidence of 30% in the general population. Opioids such as fentanyl are being used as adjuvant to local anesthetic for its antiemetic effect. In this context, with this study we aimed to evaluate the impact of spinal fentanyl as an adjuvant on the incidence of PONV compared with a placebo, and shivering. A systematic search of randomized controlled trials that evaluated the use of spinal fentanyl in the prevention of PONV and shivering was conducted in different databases, of which 32 studies met the inclus

PURPOSE: The objective of this study was to compare the efficacy of a low-cost heat-preserving method in preventing intraoperative hypothermia with that of forced-air warming in a resource-limited setting.
METHODS: In this randomized controlled non-inferiority trial, we recruited children younger than 12 years scheduled for cranial neurosurgery in a large East-African hospital. Patients were block-randomized by age to intraoperative warming measures using Hibler's method (intervention) or warm air (comparator). Hibler's group patients were circumferentially wrapped in transparent plastic shee

Rigors are not a recognized characteristic of miliary tuberculosis. We report two patients presenting with persistent rigors, thought to be suggestive of acute pyogenic infection, who were subsequently found to have miliary tuberculosis. In both cases, there was significant diagnostic delay. Miliary tuberculosis should therefore be included in the differential diagnosis of any patient presenting with unexplained rigors.

BACKGROUND: New Zealand's (NZ) complete absence of community transmission of influenza and respiratory syncytial virus (RSV) after May 2020, likely due to COVID-19 elimination measures, provided a rare opportunity to assess the impact of border restrictions on common respiratory viral infections over the ensuing 2 years.
METHODS: We collected the data from multiple surveillance systems, including hospital-based severe acute respiratory infection surveillance, SHIVERS-II, -III and -IV community cohorts for acute respiratory infection (ARI) surveillance, HealthStat sentinel general practice (GP
