Anterior dislocation of the restrained shoulder: a seat-belt injury.

The reduction in morbidity and mortality since the enforcement of seat-belt usage is well documented. Complications from the belt are also reported and the authors present anterior dislocation of the restrained shoulder, an injury not previously described. The mechanism of injury is explained and a change to the present standard of restraints is suggested.

[Serious shoulder injury after COVID-19 vaccination].

BACKGROUND: Since January 2021, over 24 million COVID-19 vaccines have been administered. Rarely vaccination in the deltoid muscle may lead to complications in the shoulder, called SIRVA (shoulder injury related to vaccine administration). General knowledge on SIRVA amongst doctors and other healthcare workers is lacking. However, due to the large amount of vaccinations which have been administered over the last year, SIRVA is seen more often. CASE REPORT: In this report, two cases of SIRVA due to septic arthritis and a shoulder abscess after administration of a COVID-19 vaccination, are desc

Abnormal Pain Response After a Compensable Shoulder Injury.

BACKGROUND: The role of psychosocial factors has been established in patients with shoulder abnormalities. However, the prevalence of exaggerated pain behaviors and their association with the characteristics of injured workers have not been well studied. PURPOSE: To examine the prevalence of abnormal pain responses (APRs) in workers with active workers' compensation claims for a shoulder injury and to examine the differences between workers with APRs versus workers without APRs. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: An analysis of electronic data files of injur

Implementation of a multicenter shoulder dystocia injury prevention program.

Although the evidence for supporting the effectiveness of many patient safety practices has increased in recent years, the ability to implement programs to positively impact clinical outcomes across multiple institutions is lagging. Shoulder dystocia simulation has been shown to reduce avoidable patient harm. Neonatal injury from shoulder dystocia contributes to a significant percentage of liability claims. We describe the development and the process of implementation of a shoulder dystocia simulation program across five academic medical centers and their affiliated hospitals united by a commo

The volleyball athlete's shoulder: biomechanical adaptations and injury associations.

In volleyball, the dominant shoulder of the athlete undergoes biomechanical and morphological adaptations; however, definitive conclusions about their exact nature, aetiology, purpose and associations with shoulder injury have not been reached. We present a systematic review of the existing literature describing biomechanical adaptations in the dominant shoulders of volleyball players and factors that may predispose to shoulder pain/injury. A thorough literature search via Medline, EMBASE and SCOPUS was conducted for original studies of volleyball players and 15 eligible articles were identifi

Treatment of shoulder sequelae in brachial plexus birth injury.

BACKGROUND: Many children with permanent brachial plexus birth injury (BPBI) develop shoulder problems, with subsequent joint deformity without treatment. We assessed the indications and outcome of shoulder operations for BPBI. PATIENTS AND METHODS: 31 BPBI patients who had undergone a shoulder operation in our hospital between March 2002 and December 2005 were included in the study. Relocation of the humeral head had been performed in 13 patients, external rotation osteotomy of the humerus in 5 patients, subscapular tendon lengthening in 5 patients, and teres major transposition in 8 patient

Measuring shoulder injury function: common scales and checklists.

The increasing shift towards patient-centred healthcare has lead to an emergence of patient-reported outcome instruments to quantify functional outcomes in orthopaedic patients. Unfortunately, selecting an instrument for use in a shoulder trauma population is often problematic because most shoulder instruments were initially designed for use with chronic shoulder pathology patients. To ensure an instrument is valid, reliable, and sensitive to clinical changes, it is important to obtain psychometric evidence of its use in the target population. Four commonly used shoulder outcome instruments ar

Recovery of brachial plexus injury after shoulder dislocation.

Brachial plexus injury is an underestimated complication from anterior dislocation of the shoulder. To our knowledge, there is limited information available about the factors that influence neurological recovery of this injury. We reviewed 15 upper extremities in 14 patients with brachial plexus injuries caused by anterior shoulder dislocation. Two-thirds of the cases had total brachial plexus palsy. With the conservative treatment, the motor recoveries of all cases are full or nearly full within 20 months except intrinsic muscle of the hand. Intrinsic muscle recovery may be better in a younge

Injury of the suprascapular nerve in shoulder surgery: an anatomic study.

Fifty-two shoulders in 26 cadavers were dissected to evaluate the safe zone for avoiding injury of the suprascapular nerve during open surgical procedures and arthroscopic Bankart repairs requiring blind drilling. The course of the suprascapular nerve was given as the shortest distance between the suprascapular nerve and the glenoid rim. A Kirschner wire was inserted from the anterior glenoid rim toward the suprascapular nerve. The insertion angle toward the suprascapular nerve at the base of the scapular spine in the transverse plane averaged 37.0 degrees and in the sagittal plane averaged 17

Etiologies of shoulder pain in cervical spinal cord injury.

A protocol including physical examination, plain radiography, and shoulder arthrography was designed to study prospectively the causes of shoulder pain in patients with cervical spinal cord injury. Twenty-four patients (30 shoulders) were studied and subdivided into acute and chronic groups. The causes of shoulder pain in the acute group of 11 patients (15 shoulders) included capsular contracture or capsulitis or both in 6 shoulders; rotator cuff tears in 4; anterior instability in 2; and rotator cuff impingement, osteoarthritis with osteonecrosis, and osteoarthritis in 1 each. Of 13 patients

[Shoulder girdle injury in childhood. Operation or conservative procedure?].

The fracture of the clavicle is a very frequent injury to children and young adults. Using a figure-of-eight bandage for treatment leads to good functional result and acceptable cosmetical appearance. Indications for an open reduction are rare: they are confined to brachial plexus lesions, injuries to the subclavian vessels, open fractures or fractures of the outer third of the clavicle combined with an acromioclavicular separation. Fractures of the scapula merely require immobilization and subsequent physiotherapy to yield satisfactory results. Distorsions and subluxations of the joints of th

Shoulder dystocia recognition: differences in neonatal risks for injury.

Shoulder dystocia (SD) is an event whose current diagnostic approach is based on subjective criteria alone. Since the risk for immediate neonatal morbidity is critically linked to recognition and appropriate management of this obstetric emergency, we hypothesized that infants having injuries consistent with SD are frequently delivered without the intrapartum identification of this condition. A retrospective analysis of 26,033 vaginal births from January 1979 to April 1987 identified 162 maternal cases in which SD was diagnosed during delivery (incidence, 0.62%). Within this subset of patients,

Osteonecrosis: an overuse injury of the shoulder in paraplegia: case report.

The shoulder of the wheelchair dependent paraplegic is subject to overuse injury with subsequent pain. The major overuse syndromes observed include soft tissue injuries and secondary degenerative arthritis. This report presents a case in which bilateral osteonecrosis of the humeral heads was found to be the source of pain in the shoulders of an active paraplegic without any evidence of disease or medical treatment associated with the development of osteonecrosis. Osteonecrosis should be entertained in the differential diagnosis of overuse injuries of the shoulder in paraplegia.

Early and Emergency Department Management of Infraclavicular Brachial Plexus Injuries Secondary to Shoulder Trauma.

Infraclavicular brachial plexus injury is an uncommon but potentially disabling complication of shoulder trauma, particularly glenohumeral dislocation and proximal humerus fracture or fracture-dislocation. Reported frequencies of neurological deficit vary widely because of heterogeneous definitions and ascertainment methods. Systematic reviews consistently identify the axillary nerve as the most commonly affected nerve following dislocation; however, multi-nerve deficits indicating cord-level involvement are clinically significant because recovery may be delayed or incomplete. Emergency care p

Shoulder dystocia related fetal neurological injuries: the predisposing roles of forceps and ventouse extractions.

On the basis of 333 documented cases of permanent perinatal neurological damage, associated with arrest of the shoulders at birth, the authors conducted a retrospective study in order to evaluate the predisposing role, if any, of the utilization of extraction instruments. The investigation revealed that 35% of all injuries occurred in neonates delivered by forceps, ventouse or sequential ventouse-forceps procedures. This frequency was several-fold higher than the prevailing instrument use in the practices of American obstetricians during the same years. A high rate of forceps and ventouse extr
