Open skull fractures and sepsis: Evidence towards a targeted antibiotic prophylactic recommendation.

BACKGROUND AND OBJECTIVES: Although patients with weapon-induced open skull fractures benefit from antibiotic prophylaxis, there is limited data on microbiological factors associated with such injuries. This study considers the most prominent bacteria cultured, alongside their antibiotic resistance profiles and factors relating to wound infection for insights into a targeted clinical management recommendation in patients suffering from assault-related open skull fractures. METHODS: Patients with weapon-induced traumatic head injuries and infective complications were analysed (n = 108) regardi

Elevated skull fractures: an institutional experience and individual participant data meta-analysis.

Elevated skull fracture (ESF) is a rare but potentially life-threatening type of skull fracture. The literature on this topic is relatively sparse. Herein, we conducted a meta-analysis of all the patients reported in the literature with ESFs with respect to their clinical management to better inform practice. On 20th of January 2023, we conducted a systematic search of literature to find all published cases of ESF. We also conducted a retrospective review of ESF cases from our institution. The data collection and analysis were conducted in accordance with the Preferred Reporting Items for Syst

The Usefulness of Vascularized Flap Craniotomy for Open-Wound Depressed Skull Fractures.

BACKGROUND AND IMPORTANCE: Free bone flap craniotomy for open-wound depressed skull fractures poses a high risk of infection due to free bone fragments. This risk escalates in the presence of compound fractures with accompanying dural injuries. The primary objectives in managing depressed skull fractures include infection prevention and the correction of cosmetic deformities. This report describes the efficacy of vascularized flap craniotomy, which preserves blood flow to bone fragments, in reducing infection risks. CLINICAL PRESENTATION: In this study, we present a 54-year-old man with a hea

Extra-axial haemorrhages in young children with skull fractures: abuse or accident?

OBJECTIVE: Infant and toddler subdural haemorrhages (SDH) are often considered indicative of abuse or major trauma. However, accidental impact events, such as falls, cause contact extra-axial haemorrhages (EAHs). The current study sought to determine frequency and clinical behaviour of EAHs with infant and toddler accidental and abusive skull fractures. PATIENTS AND METHODS: Children aged <4 years with accidental skull fractures and abusive fractures identified by CT at two paediatric tertiary care centres. Clinical data were abstracted by child abuse paediatricians and images were reviewed b

Basilar Skull Fractures and Their Complications in Patients With Traumatic Brain Injury.

OBJECTIVE: Since traumatic brain injury is more common in young people, who are the main workforce and builders of society, it is important to consider the effects caused by brain injury on them. In this study, we investigated the clinical manifestations, complications, and prognosis of patients with basilar skull fractures. METHODS: This cross-sectional study was conducted from March 2021 to March 2022 at the Kashani Hospital, Esfahan, Iran. Patients with basilar skull fractures were included in this study by census for one year. Recorded patient information was divided into two parts: demog

Outcome of management of elevated skull fractures in Enugu, South-East Nigeria.

INTRODUCTION: Elevated skull fractures, previously thought of as a very rare variety of fractures, are no longer very uncommon. They are expectedly gradually finding a slowly growing list of references in neurosurgical literature. They are mostly posttraumatic compound fractures due to the mechanism of injury. Outcome of operative neurosurgical care is generally rewarding. MATERIALS AND METHODS: A 4-year retrospective study of case notes, operation registers, and radiology records of patients diagnosed with elevated skull fractures who had neurosurgical care at the University of Nigeria Teach

Management of skull fractures in children less than 1 year of age.

BACKGROUND: Management of skull fracture (SF) in pediatric patients varies from observation in the emergency department (ED) to floor admission. Since 2010, a protocol for admitting children with SF specifically to the trauma service was implemented at our institution. The purpose of our study was to review the management of children with SF younger than 1 year of age. METHODS: Retrospective chart review of all patients between 0 and 1year of age seen in our ED for a SF was done from 2010 to 2013. RESULTS: A total of 180 patients with a mean age of 4.5months (1day-12months) were identified.

A QI Initiative to Reduce Hospitalization for Children With Isolated Skull Fractures.

BACKGROUND AND OBJECTIVE: Although children with isolated skull fractures rarely require acute interventions, most are hospitalized. Our aim was to safely decrease the hospitalization rate for children with isolated skull fractures. METHODS: We designed and executed this multifaceted quality improvement (QI) initiative between January 2008 and July 2015 to reduce hospitalization rates for children ≤21 years old with isolated skull fractures at a single tertiary care pediatric institution. We defined an isolated skull fracture as a skull fracture without intracranial injury. The QI interventio

Healed Depressed Parasagittal Skull Fractures-A Feature of Archaic Australian Aboriginal Remains.

The skeletal remains of eight Australian Aboriginals with healed depressed skull fractures were examined. Male:female ratio 5:3; age range 20-60 yrs. Burial dates by 14 C dating in three cases were 500 years BP (n = 2) and 1300 BP. There were 13 healed depressed skull fractures manifested by shallow indentations of cortical bone and thinning of diploe, with no significant disturbance of the inner skull tables. Nine (69%) were located within 35 mm of the sagittal suture/midline. These lesions represent another acquired feature that might be helpful in suggesting that a skull is from a tribal Ab

Versatility of the pedicled nasoseptal flap in the complicated basal skull fractures.

With the advancement of transnasal endoscopic technique, endoscopic repair of basal skull fractures has considerably substituted former external approaches. The endoscopically feasible pedicled flap, named nasoseptal flap has been extending its range of application, since it was introduced for the reconstruction of the defect after resection of skull base tumors. We introduce two patients with complicated basal skull fractures at different sites who were successfully treated by the transnasal endoscopic approach using nasoseptal flap.

Brain trauma in head injuries presenting with and without concurrent skull fractures.

Head injuries and skull fractures may be problematic in cause and manner of death. Over a 10-year period, 54 cases showing head injuries were studied. Of these, 34 had skull fractures and 20 had no skull fractures. Virtually all decedents with skull fractures had brain injuries. The most common injury in both groups was motor vehicle accidents (MVA), in which 50% had skull fractures. In cases of skull fracture, brain lacerations, hemorrhages, and cerebral edema were common. Of 20 decedents with head injury but no skull fracture, most were accidents, and all but 3 cases had brain injury, althou

Surgical repair of skull fractures in four horses using cuttable bone plates.

Three horses with severely comminuted, open facial bone fractures and one horse with a comminuted, open orbital rim fracture were referred for treatment. Severe facial bone asymmetry and epistaxis were apparent in all cases and subcutaneous emphysema was present in two, however physical and neurological examinations were otherwise normal. Radiography and endoscopy were of some use in assessing the degree of damage, although the true extent of the damage was more apparent at surgery. Surgical reduction of the fractures was recommended to maximise cosmetic and functional outcome. After surgical

Is routine computed tomographic (CT) scanning necessary in suspected basal skull fractures?

The aim of this prospective observational study was to assess the yield of routine fine-cut computed tomographic (CT) scans in patients with suspected basal skull injuries. Over an 8 month period in 1994, 500 consecutive head-injured patients were examined for clinical signs of basal skull fracture and underwent fine-cut (5 mm) CT scans through the skull base in addition to standard (10 mm) cuts through the vault. Clinical signs were present in 144 patients (144/500, 28.8 per cent) of which 75 (75/144, 52 per cent) had corresponding CT evidence of fracture. A further 22 patients (22/500, 4.4 p

[Depression fractures of the skull in newborn infants. Treatment with vacuum extractor].

Depressed skull fracture in newborns, usually called "ping-pong" lesion, is rare. The fractures may interfere with the function and growth of the brain and may establish an epileptogenic focus. Therefore early elevation is usually necessary. Traditionally, these lesions have been considered to be indication for neurosurgical elevation. During recent years, non-surgical elevation by means of breast pump or vacuum extractor have been reported. This procedure is technically feasible and less traumatic to the infant, and should be attempted before considering operative intervention. If the fractur

Presentation, Evaluation, and Outcomes of Infants Under 3 Months With Skull Fractures.

BACKGROUND: Head injuries in infants are common but challenging to assess due to minor mechanisms potentially causing significant injury, nonspecific symptoms, and the risk of nonaccidental trauma (NAT). STUDY OBJECTIVES: This study evaluates clinical presentations, assessments, and outcomes in infants with skull fractures, with and without intracranial hemorrhage (ICH). METHODS: A retrospective chart review was conducted at a tertiary care children's hospital, identifying 291 infants (mean age: 6 weeks) with skull fractures via computed tomography imaging and ICD9/ICD10 codes. RESULTS: Mos

Single-Stage Titanium Mesh Cranioplasty for the Treatment of Depressed Skull Fractures.

BACKGROUND: Historically, depressed skull fractures that warranted surgery were treated in 2 stages: the first stage involved debridement and craniectomy, followed by the second stage of delayed cranioplasty. More recently, single-stage autologous cranioplasty has been proven to be safe. However, there is a paucity of literature regarding single-stage titanium mesh cranioplasty when autologous repair is not possible. METHODS: A retrospective review identified 22 patients who underwent single-stage titanium mesh cranioplasty for the acute treatment of comminuted depressed skull fractures. Frac

Association between cervical spine and skull-base fractures and blunt cerebrovascular injury.

INTRODUCTION: Blunt cerebrovascular injuries (BCVI) are associated with high morbidity and mortality and can lead to neurological deficits. The established criteria for patients undergoing CT angiography (CTA) for BCVI are broad, and can expose patients to radiation unnecessarily. This study aimed to examine the prevalence of BCVI in patients on CTA and determine presentations associated with the highest rates of BCVI. MATERIALS AND METHODS: With IRB approval, patients were selected for CTA screening for BCVI according to a predefined set of criteria at our hospital between 2007 and 2010. Pat
