Testicular blood flows during unilateral testicular torsion were measured by Xenon-133 clearance technique. Xenon-133 was injected via the left carotid artery into the heart. A commercially available system of caesium iodide crystal mini-detectors (Oakfield Instruments, Oxon, UK) was modified so that it was suitable for dual isotopic measurement of the plasma protein accumulation index (PPA)- a measure of pulmonary endothelial permeability. Using this modified system the mean PPA x 10(-3) min-1 +/- (S.E.M.) recorded in 11 normal subjects (22 lungs) was 0.18 (0.08) and in 6 patients (9 lungs) with the adult respiratory distress syndrome was 2.88 (0.63) (P less than 0.02). Two cases of mediastinal localization of parathyroid adenoma are presented, in which technetium-thallium subtraction scintigraphy yielded a positive result. Both patients had already undergone a negative surgical neck exploration. We suggest that, in case of negative subtraction scintigraphy and negative surgical exploration in proven primary hyperparathyroidism, subtraction scintigraphy should be repeated with emphasis on the superior mediastinum, and in all cases, the use of a non-zoom, large field of few procedure is recommended for technetium-thallium subtraction scintigraphy.

The testicular scintigraphic findings of nine patients with surgically and pathologically proved torsion of the testis of over 24 hours duration are reviewed. A 22 year old man with asymptomatic hypoxemia was found to have a large right to left shunt due to a rare congenital anomaly: total drainage of the right superior vena cava into the left atrium. The anomaly was first suspected after radionuclide angiocardiography was performed using technetium-99m macroaggregated albumin and was confirmed by cardiac catheterization. Contrast echocardiographic and surgical findings are discussed. Other reports on this anomaly are reviewed.

Technetium-99m tetrofosmin is a lipophilic phosphine used for myocardial perfusion imaging.
