Meniere's disease: clinical course, auditory findings, and hearing aid fitting.

Results of a five-year longitudinal study of Meniere's disease in 95 patients is presented. Tests utilized in diagnosis and in determining specific etiology are described. Clinical treatment for patients with Meniere's disease symptoms is discussed. Auditory test results for 101 ears are analyzed. The fluctuating nature of the hearing loss, the poor discrimination for speech, and the limited tolerance for amplification because of the narrow dynamic range present significant problems in utilization of hearing aids for patients with Meniere's disease. A practical approach is suggested for the fi

A preliminary validation of a dynamic speech motor assessment for Swedish-speaking children with childhood apraxia of speech.

OBJECTIVE: Children with speech sound disorders (SSDs) have difficulties affecting different levels of speech production. For treatment to be beneficial, it is important to differentiate between Childhood Apraxia of Speech (CAS) - a motor speech disorder with deficits in speech praxis - and other SSDs (nonCAS-SSD). We have previously developed a motor speech examination Dynamisk motorisk talbedömning (DYMTA). We aimed to evaluate DYMTAs reliability and validity in a small-scale sample to estimate DYMTAs usability in diagnostic settings. METHODS: Speech, language, and oral motor abilities were

Effect of phonological training in French children with SLI: perspectives on voicing identification, discrimination and categorical perception.

The aim of the present study was to investigate the effect of auditory training on voicing perception in French children with specific language impairment (SLI). We used an adaptive discrimination training that was centred across the French phonological boundary (0 ms voice onset time--VOT). One group of nine children with SLI attended eighteen twenty-minute training sessions with feedback, and a control group of nine children with SLI did not receive any training. Identification, discrimination and categorical perception were evaluated before, during and after training as well as one month fo

Relationships among rapid digit naming, phonological processing, motor automaticity, and speech perception in poor, average, and good readers and spellers.

In this article, we explore the relationship between rapid automatized naming (RAN) and other cognitive processes among below-average, average, and above-average readers and spellers. Nonsense word reading, phonological awareness, RAN, automaticity of balance, speech perception, and verbal short-term and working memory were measured. Factor analysis revealed a 3-component structure. The first component included phonological processing tasks, RAN, and motor balance. The second component included verbal short-term and working memory tasks. Speech perception loaded strongly as a third component,

Spectral ripple discrimination and use of speech context factors in relation to speech perception in noise in single-sided deaf cochlear implant users.

OBJECTIVE: To investigate the relation between spectral ripple discrimination and speech perception in noise in single-sided deaf (SSD) cochlear implant (CI) users, and to investigate the use of speech context in speech perception in noise, for both the normal-hearing (NH) and the CI ear. DESIGN AND STUDY SAMPLE: Twenty adult SSD CI users (40 ears) participated in the study. We used the spectral-temporally modulated ripple test (SMRT) and measured speech reception thresholds (SRTs) with the consonant-vowel-consonant (CVC)-in-noise test, the hearing-in-noise (sentence) test (HINT), and the dig

Engaging the Articulators Enhances Perception of Concordant Visible Speech Movements.

Purpose This study aimed to test whether (and how) somatosensory feedback signals from the vocal tract affect concurrent unimodal visual speech perception. Method Participants discriminated pairs of silent visual utterances of vowels under 3 experimental conditions: (a) normal (baseline) and while holding either (b) a bite block or (c) a lip tube in their mouths. To test the specificity of somatosensory-visual interactions during perception, we assessed discrimination of vowel contrasts optically distinguished based on their mandibular (English /ɛ/-/æ/) or labial (English /u/-French /u/) postu

Focal cranial dystonia.

In the neurologic literature there has been a growing recognition of a syndrome of focal cranial dystonias referred to variously as blepharospasm-oromandibular dystonia, Brueghel's syndrome, or Meige's syndrome. Typically, this syndrome presents as an isolated and idiopathic dystonia of facial or oromandibular muscles occasionally prompting a referral to a speech pathologist. Our findings in a group of 10 patients with this diagnosis are presented, and the negligible effect of an intensive program of speech management for one subject is described.

Outcomes of Cochlear Implantation in Mumps-Induced Single-Sided Deafness: A Retrospective Analysis.

BACKGROUND Mumps, a viral infection from the Paramyxoviridae family, is a rare but significant cause of sensorineural hearing loss (SNHL), especially in children. While typically unilateral, mumps-induced SNHL can result in profound single-sided deafness, for which cochlear implantation may be considered when hearing aids are ineffective. This study aimed to evaluate outcomes of cochlear implantation in patients with single-sided deafness caused by mumps. MATERIAL AND METHODS This retrospective study included 14 patients (10 women, 4 men; mean age 39.4) with single-sided deafness caused by mum

[Manifestations of inactivity in unilateral hearing aid management of patients with a high degree of hearing loss].

Concluding from audiometrical practice using hearing aids unilaterally we could find an inactivity of the unused contralateral ear. We have investigated if it is possible to measure this inactivity by common psychoacoustic methods. We tested 92 persons with advanced combined hearing loss. We had to state that the inactivity is not measurable by mean tone hearing loss according to HAIC (0.5, 1 and 2 kHz), the percentage hearing loss according to Fowler-Sabine, the percentage of hearing loss calculated by speech-audiogram according to Boenninghaus and Röser 1973 and the value of hearing loss for

Longitudinal Speech Outcomes in Cochlear Implant Recipients Are Associated With Neural Factors Identified Using Psychophysics and Functional Brain Imaging.

OBJECTIVES: There remain large variations in cochlear implant auditory-speech perception ability, which are poorly explained by patient history factors. This study aims to better understand how combinations of speech processing limitations, identified using psychophysical and functional brain imaging measures, are associated with long-term speech understanding outcomes of new cochlear implant recipients. DESIGN: Forty-three cochlear implant recipients (44 ears) were recruited to participate in a longitudinal study to evaluate limitations at various locations along the auditory processing path

Dichotic word recognition in noise and the right-ear advantage.

PURPOSE: This study sought to compare dichotic right-ear advantages (REAs) of young adults to older adult data (C. M. Roup, T. L. Wiley, & R. H. Wilson, 2006) after matching for overall levels of recognition performance. Specifically, speech-spectrum noise was introduced in order to reduce dichotic recognition performance of young adults to a level consistent with that of older adults with hearing loss. METHOD: Dichotic word-recognition performance was evaluated in the free-recall response paradigm across 2 conditions: (a) quiet and (b) noise (+11 dB signal-to-noise ratio). Participants inclu

[Progressive noise induced hearing loss caused by hearing AIDS, a dilemma for the worker and the expert alike].

BACKGROUND: Investigating cases of noise induced hearing loss the expert is often confronted with the situation that the hearing loss is progressive although the noise exposure has been reduced to almost non-damaging levels. Other causes such as age, hereditary deafness, head injuries, blasts, internal diseases can be excluded. Hearing aids as sources of damaging noise? By consulting the protocol of the hearing-aid acoustician and by own examinations the expert should obtain the following data: loudness level that yields best discrimination score of speech; level of discomfort for tones and sp

The relationships between self-reported hearing disability and measures of auditory disability.

Although required for many practical purposes, adequate measures of hearing disability are not yet available. In an attempt to identify a set of performance tests for predicting hearing disability in daily life the relationship between self-reported disability scores and measures of auditory disability was examined. The Amsterdam Inventory was completed by 51 respondents aged 30 to 70 years who also performed on various tests. Earlier factor analysis of the inventory scores resulted in the distinction of five aspects of auditory disability. Stepwise multiple regression analysis in the present

Effects of aging and noise on real-time spoken word recognition: evidence from eye movements.

PURPOSE: To use eye tracking to investigate age differences in real-time lexical processing in quiet and in noise in light of the fact that older adults find it more difficult than younger adults to understand conversations in noisy situations. METHOD: Twenty-four younger and 24 older adults followed spoken instructions referring to depicted objects, for example, "Look at the candle." Eye movements captured listeners' ability to differentiate the target noun (candle) from a similar-sounding phonological competitor (e.g., candy or sandal). Manipulations included the presence/absence of noise,

Computer-based auditory training (CBAT): benefits for children with language- and reading-related learning difficulties.

This article reviews the evidence for computer-based auditory training (CBAT) in children with language, reading, and related learning difficulties, and evaluates the extent it can benefit children with auditory processing disorder (APD). Searches were confined to studies published between 2000 and 2008, and they are rated according to the level of evidence hierarchy proposed by the American Speech-Language Hearing Association (ASHA) in 2004. We identified 16 studies of two commercially available CBAT programs (13 studies of Fast ForWord (FFW) and three studies of Earobics) and five further ou
