Artificial Intelligence in Speech and Language Therapy: A Qualitative Comparative Analysis of Clinical Applications and Outcomes.

Large language models (LLMs) such as ChatGPT are entering clinical practice, yet how their clinical reasoning compares with speech-language therapists (SLTs) is not well understood. This comparative multi-case qualitative study used 3 hypothetical vignettes. Ten experienced SLTs (≥10 years) participated in semi-structured interviews, providing assessment, diagnosis and therapy plans for each vignette. ChatGPT-4o was presented with identical, standardized Turkish prompts over five consecutive days to evaluate the model's temporal consistency in clinical reasoning. All outputs were analyzed with

Using Generative Artificial Intelligence to Classify Primary Progressive Aphasia from Connected Speech.

Neurodegenerative dementia syndromes, such as Primary Progressive Aphasias (PPA), have traditionally been diagnosed based in part on verbal and nonverbal cognitive profiles. Debate continues about whether PPA is best subdivided into three variants and also regarding the most distinctive linguistic features for classifying PPA variants. In this study, we harnessed the capabilities of artificial intelligence (AI) and natural language processing (NLP) to first perform unsupervised classification of concise, connected speech samples from 78 PPA patients. Large Language Models discerned three disti

Leveraging speech and artificial intelligence to screen for early Alzheimer's disease and amyloid beta positivity.

Early detection of Alzheimer's disease is required to identify patients suitable for disease-modifying medications and to improve access to non-pharmacological preventative interventions. Prior research shows detectable changes in speech in Alzheimer's dementia and its clinical precursors. The current study assesses whether a fully automated speech-based artificial intelligence system can detect cognitive impairment and amyloid beta positivity, which characterize early stages of Alzheimer's disease. Two hundred participants (age 54-85, mean 70.6; 114 female, 86 male) from sister studies in the

An Ecosystem of Intelligent ICT Tools for Speech-Language Therapy Based on a Formal Knowledge Model.

The language and communication constitute the development mainstays of several intellectual and cognitive skills in humans. However, there are millions of people around the world who suffer from several disabilities and disorders related with language and communication, while most of the countries present a lack of corresponding services related with health care and rehabilitation. On these grounds, we are working to develop an ecosystem of intelligent ICT tools to support speech and language pathologists, doctors, students, patients and their relatives. This ecosystem has several layers and c

Artificial intelligence-based speech error detection to differentiate primary progressive aphasia variants.

Artificial intelligence-based approaches to speech analysis have the potential to assist with objective speech error analysis in aphasia but off-the shelf tools often fail to detect speech errors due to prioritizing 'fluent transcription'. Speech production errors (dysfluencies) are hallmark diagnostic features of the nonfluent and logopenic variants of primary progressive aphasia, yet they can be challenging to detect and characterize even by expert clinicians. This study aimed to evaluate whether the novel automated lightweight Scalable Speech Dysfluency Modeling system, specifically designe

Triphasic Stimulation in Cochlear Implants: A Comparative Evaluation of Programming Levels, Speech Intelligibility, and Spectral-Temporal Resolution in Users With and Without Inner Ear Malformations.

OBJECTIVES: Nonauditory or facial-nerve sensations limit the electrical dynamic range of some cochlear implant (CI) users with inner ear malformations (IEMs). Triphasic pulses, which redistribute charge over three phases, can raise comfortable current levels without provoking side effects, but their effect on everyday listening remains unclear. We asked whether triphasic stimulation improves speech perception and spectral-temporal processing in CI users with IEM and whether the same change benefits-or degrades-performance in users with normal inner ear anatomy (NIE). We hypothesized that triph

[Articulatory capacity and intelligibility of speech of patients with carcinomas of the mouth cavity. A comparison of pre- and postoperative results of various reconstruction techniques].

Radical orofacial tumor resections often result in speech and swallowing dysfunctions. Thus, both communication ability and life quality can be markedly impaired in patients. Dysfunction is mostly related to the amount of tissue removed and the reconstruction technique used. In this prospective study, pre- and postoperative speech intelligibility following reconstruction with either local grafts, free jejunal flaps or myocutaneous/fasciocutaneous flaps was examined by subjective assessment of monosyllables chosen from the Freiburg audiometry test ("reverse speech intelligibility test"). It was

Rasch analysis of word identification and magnitude estimation scaling responses in measuring naive listeners' judgments of speech intelligibility of children with severe-to-profound hearing impairments.

PURPOSE: Speech intelligibility research typically relies on traditional evidence of reliability and validity. This investigation used Rasch analysis to enhance understanding of the functioning and meaning of scores obtained with 2 commonly used procedures: word identification (WI) and magnitude estimation scaling (MES). METHOD: Narrative samples of children with hearing impairments were used to collect data from listeners with no previous experience listening to or judging intelligibility of speech. WI data were analyzed with the Rasch rating scale model. MES data were examined with Rasch pa

Parametric Cognitive Load Reveals Hidden Costs in the Neural Processing of Perfectly Intelligible Degraded Speech.

Speech is often degraded by environmental noise or hearing impairment. People can compensate for degradation, but this requires cognitive effort. Previous research has identified frontotemporal networks involved in effortful perception, but materials in these works were also less intelligible, and so it is not clear whether activity reflected effort or intelligibility differences. We used functional magnetic resonance imaging to assess the degree to which spoken sentences were processed under distraction and whether this depended on speech quality even when intelligibility of degraded speech w

Is the spontaneous speech of 7-year-old cochlear implanted children as intelligible as that of their normally hearing peers?

OBJECTIVES: Studies on speech intelligibility usually focus on either individual words, sentences or longer sequences of speech. Since these different kinds of speech samples can be judged using different methodologies, a difference in the reported intelligibility scores can either be due to the difference in the type of sample or methodology. The present study compares the speech intelligibility of seven-year-old children with a cochlear implant (CI) with that of their normally hearing (NH) peers. The first aim is to compare the intelligibility of short sentences and longer sequences of speec

Age-Related Decline in Neural Phase-Locking to Envelope and Temporal Fine Structure Revealed by Frequency Following Responses: A Potential Signature of Cochlear Synaptopathy Impairing Speech Intelligibility.

PURPOSE: Assessing the contribution of cochlear synaptopathy (CS) to the variability in speech-in-noise intelligibility among individuals remains a challenge. While several studies have proposed biomarkers for CS based on neural phase-locking to the temporal envelope (ENV), fewer have investigated how CS affects the coding of temporal fine structure (TFS), despite its crucial role in speech-in-noise perception. In this study, we specifically examined whether TFS-based markers of CS could be derived from electrophysiological responses and psychophysical detection thresholds of spectral modulati

An Intelligent Mobile-Enabled System for Diagnosing Parkinson Disease: Development and Validation of a Speech Impairment Detection System.

BACKGROUND: Parkinson disease (PD) is one of the most common neurological diseases. At present, because the exact cause is still unclear, accurate diagnosis and progression monitoring remain challenging. In recent years, exploring the relationship between PD and speech impairment has attracted widespread attention in the academic world. Most of the studies successfully validated the effectiveness of some vocal features. Moreover, the noninvasive nature of speech signal-based testing has pioneered a new way for telediagnosis and telemonitoring. In particular, there is an increasing demand for a
