The origin of the temporary reduction in hematocrit and hemoglobin occurring after spinal cord injury (SCI) may be related to the neurologic dysfunction and to a change in erythropoietin (Ep) synthesis and/or secretion. Serum Ep, vital capacities, and numerous hematologic profiles were obtained in 12 cervical SCI patients during the first 12 weeks and 13th to 27th post-injury weeks, and in 8 thoracic SCI patients during the first 8 weeks and 9th to 28th weeks. Serum iron, iron binding, and saturation were obtained in 4 cervical SIC patients and blood gases in 10 patients with cervical SCI and

There are many classifications of spinal cord injury. The anatomic definitions help in understanding the pathophysiology but are less useful in calculating recovery. Basic classifications, such as those by Guttman and Frankel, are useful for epidemiological studies, especially if one wishes to compare patients internationally or to compare data bases but this information, except in complete lesions, is too limited to allow more refined calculations as to the effectiveness of treatment modalities, especially in partial lesions. An accurate motor index, such as we have described, or such as the

This sequela may be more common than is recognized. Because syringomyelia can progress to advanced stages without overt symptoms, awareness of this entity is vital for early diagnosis. Unexplained pain, numbness and ascending or descending anesthesia, followed by motor loss, point to the diagnosis. Other signs include neurotrophic arthropathy, Horner's syndrome, autonomic dysreflexia and manifestations of brainstem involvement. Computed tomography helps identify the cysts. Operative procedures to promote drainage of the fluid from the syrinx may cure or halt progression of the lesion.

Four patients having high-level quadriplegia developed elevated serum calcium concentrations (11 to 15.8 mg/100 ml) within three months of injury. All were young males (ages 15 to 19 years) and quadriplegic (C4-C7). Presenting symptoms were nausea, vomiting, polydipsia, polyuria and lethargy. In two patients severe muscle wasting and cachexia with clinical symptoms developed and persisted for several months. Laboratory studies in all patients showed negative calcium balance with hypercalciuria. Reduced renal function was seen in all patients but returned to normal with return of normal serum c

A study was made of the pattern of spinal cord injury in the elderly who are forming an ever-increasing proportion of our population. It was noted that the elderly patients had a higher incidence of cervical injuries than other age-groups with a much lower incidence of dorsal and lumbar injuries. The mortality rate in complete cervical injuries was very high but in incomplete cervical injuries and those at a lower level the majority survived the acute phase. The cause of death in the acute phase was similar to that of other age-groups. The majority of survivors were able to be discharged to th

Spasticity is a hyper-excitable state of the reflex arcs in the spinal cord below the level of injury. Not only is the skeletal motor system affected, but bladder, bowel, blood pressure, and erection reflex mechanisms are also involved. Spasticity gradually emerges from the initial phase of spinal shock one to two months after the injury, and usually reaches a plateau of a mild to moderate degree in 3 to 4 months. Neurophysiological mechanisms indicate an increase in the alpha and gamma reflex systems and that central excitability through the interneurons is also involved in these systems. Exc

Spinal cord injury (SCI) severely disrupts central nervous system (CNS) function by interrupting sensory and motor signal transmission, often resulting in permanent deficits due to the formation of a glial scar. Although the amniotic membrane (AM) is derived from the human placenta and is a promising biomaterial, its efficacy in treating SCI remains unexplored. This study investigates the therapeutic potential of AM fragments in a surgically induced acute SCI model in rats, focusing on preserving tissue integrity and modulating astrocyte distribution and reactivity. SCI was experimentally indu

Here, we present updated nationwide epidemiological and economic data on spinal cord injuries (SCIs) in France, based on the most recent reliable data from health insurance providers for 2023. The incidence rate is approximately 1,500 new cases per year, with an estimated 98,800 people currently living with an SCI. The estimated cost per patient is €8,000, potentially reaching €80,000 in the first year, equating to €790 million annually. Although France's healthcare system helps to reduce costs, spending on SCIs still accounts for a significant proportion of the total healthcare budget (0.4%).

To determine if regional weather patterns could predict the severity of cervical spinal cord injuries (CSCI) across the United States. Non-elective CSCI patients from 2015 to 2020 were identified in the National Inpatient Sample. Linear mixed-effects models explored associations between CSCI features and weather patterns, with random effects for hospitals. Granger causality tests were performed for each region to assess if weather could predict complete CSCI. Cross-correlation analyses examined temporal trends. Logistic mixed-effects models evaluated correlations between CSCI severity and seas

CONTEXT: Clinical Practice Guidelines from the Consortium for Spinal Cord Injury (SCI) Medicine recommend daily self-screening of at-risk skin surfaces, but many Veterans with SCI describe challenges using the standard issue long-handled self-inspection mirror (LSIM).
OBJECTIVE: The objective of this project was to compare the LSIM to a recently developed camera-based self-inspection system (CSIS). User feedback guided iterative engineering to improve and develop the new technology in preparation for transfer to industry.
METHODS: Five Veterans with spinal cord injury (SCI) volunteered to co

Spinal cord injury (SCI) is one of the most complex diseases. After SCI, severe secondary injuries can cause intense inflammatory storms and oxidative stress responses, leading to extensive neuronal apoptosis. Effective regulation of inflammation and oxidative stress after SCI remains an unresolved challenge. In this study, resveratrol-loaded nanoparticles coated with neutrophil membranes (NMR) were prepared using the emulsion-solvent evaporation method and membrane encapsulation technology. Multifunctional biomimetic nanoparticles retain neutrophil membrane-related receptors and possess a str

Persons with spinal cord injury (SCI) are at high risk for developing neurogenic obesity due to muscle paralysis and obligatory sarcopenia, sympathetic blunting, anabolic deficiency, and blunted satiety. Persons with SCI are also at high risk for shoulder, elbow, wrist, and hand injuries, including neuromusculoskeletal pathologies and nociceptive pain, as human upper extremities are poorly designed to facilitate chronic weight-bearing activities, including manual wheelchair propulsion, transfers, self-care, and day-to-day activities. This article reviews current literature on the relationship

Spinal cord injury (SCI) in youth presents with unique manifestations and complications as compared to adult-onset SCI. The primary care clinician must consider the physical, physiological, cognitive, and psychological changes transpiring during childhood and adolescence. Physical changes include increasing size, weight, and bladder volume. Physiologic considerations include decreasing heart rate and increasing blood pressure with age. Cognitive issues include communication, executive functioning, and self-management skills. Lastly, psychological processes involve emotional functioning and est

A 12-year-old male patient with Coffin-Lowry syndrome was scheduled for posterior cervical decompression and fusion for cervical spinal injuries. The patient had features of Coffin-Lowry syndrome including mental retardation, prominent forehead, a short nose with a wide tip, a wide mouth with full lips, short stature, microcephaly, and kyphoscoliosis. We anticipated major troubles related to anesthesia such as difficult ventilation and intubation, communication difficulty during induction and extubation, and difficulty in using a naso-pharyngeal airway. In addition, we had to stabilize neck al

Preclinical studies for spinal cord injury (SCI) have utilized transection and contusion injury paradigms even though human SCIs occur by a spectrum of primary injury mechanisms such as spinal cord contusion from vertebral burst fracture, shearing from fracture-dislocation, and stretching from distraction injuries. We contrasted the neuropathology in animal models mimicking these clinically relevant injuries at an early 3-hour time-point in order to relate patterns of secondary pathology to the primary injury mechanism.
