To study the efficacy of electrical stimulation in treating spasticity of six spinal cord injured patients, transcutaneous electrical nerve stimulation (TENS) was applied to the dermatomes belonging to the same spinal cord level as the selected spastic muscle group. Spasticity was assessed in knee extensors by a pendulum test in which the knee joint angle of a swinging lower leg was recorded with an electrogoniometer. TENS was found to produce a noticeable decrease of spasticity in three of the patients, but had little effect on the others.

This report presents observations in 160 patients undergoing chronic spinal cord stimulation for various disorders of the motor system and compares the results obtained using older conventional two-electrode bipolar stimulation with a newly developed four-electrode multiple level system. Improvement was noted in 84% of the 75 patients with cerebral palsy, 67% of the 42 patients with dystonia, 62% of the 21 patients with torticollis and 73% of the 22 patients with post-traumatic neurologic loss. Significant improvements were noted when comparing the two-electrode system with the new multiple le

The effect of percutaneous epidural spinal cord stimulation on neurogenic bladder has been evaluated on the basis of objective clinical and urodynamic criteria. Seven patients suffering from stable bladder and sphincter dysfunction due to spinal cord diseases of different causes of non-evolutive nature were examined. In some of them chronic pain or substantially improved micturition in six of our seven patients. Complete or almost complete relief of bladder spasticity, marked increase of bladder capacity, and reduction or abolition of residual urine were recorded. The beneficial effect on blad

PURPOSE OF REVIEW: Spinal cord stimulation (SCS) has been used for decades to alleviate chronic pain. A growing body of literature suggests that the procedure is not without risks. Understanding the risks of SCS implantation can help treating physicians formulate individualized care plans that promote patient safety and minimize risks.
RECENT FINDINGS: The overall complication rate associated with SCS has been reported to be 31.9 to 43%. The most common complication in the literature appears to be electrode migration. Other complications ranging in rates of occurrence include hematoma formati

Spinal cord stimulator trials are indicated for the treatment of postlaminectomy syndrome with persistent severe back and limb pain which has failed conservative treatment options and where no further surgery is indicated. They are also indicated for refractory complex regional pain syndrome. This article details patient positioning and set up, step-by-step instructions for the procedure and postoperative management. Pearls and pitfalls are also discussed. In addition, an instructional procedure video, Supplemental Digital Content 1 (http://links.lww.com/CLINSPINE/A121) accompanies this paper.

Cervical spinal cord injury causes severe functional impairment with limited spontaneous recovery, and while spinal cord stimulation has emerged as a promising neuromodulatory strategy, evidence for cervical applications remains fragmented. To address this gap, we conducted a systematic review synthesizing preclinical and clinical evidence on cervical spinal cord stimulation for functional rehabilitation following spinal cord injury. The review was registered on PROSPERO (CRD420251088804) and conducted in accordance with PRISMA guidelines, with PubMed, Embase, IEEE Xplore, and Web of Science s

BACKGROUND: Spinal cord injury (SCI) can lead to severe impairments in motor, sensory, and autonomic functions, significantly affecting patients' functional independence and quality of life. Transcutaneous spinal cord stimulation (tSCS), as an emerging non-invasive neuromodulation technique, has shown potential in improving motor function. However, existing studies demonstrate substantial heterogeneity, and its clinical efficacy remains unclear.
OBJECTIVE: To systematically evaluate the effects of tSCS on motor function and functional independence in individuals with SCI, and to explore the p

Spinal cord stimulation (SCS) is an effective therapeutic option for refractory neuropathic pain, but infectious complications, particularly epidural abscess, are rare and carry high morbidity. Evidence describing the inpatient rehabilitation course following urgent surgical decompression in patients with radiographic cord compression but largely preserved neurologic function remains limited, and standardized functional outcomes are seldom reported. We present the case of an 84-year-old woman with severe dextroscoliosis, kyphosis, and chronic axial back pain who underwent an outpatient SCS tri

Humans use their arms in complex ways that often demand two-handed coordination. Neurological conditions limit this impressive feature of the human motor system. Understanding how neuromodulatory techniques may alter neural mechanisms of bimanual coordination is a vital step towards designing efficient rehabilitation interventions. By non-invasively activating the spinal cord, transcutaneous spinal cord stimulation (tSCS) promotes recovery of motor function after spinal cord injury. A multitude of research studies have attempted to capture the underlying neural mechanisms of these effects usin

The case of a 37-year-old woman who suffered from spinal cord infarction (SI), resulting in a complete spinal cord injury (AIS A, neurological level T10), and autonomic dysfunction is presented. This study aimed to assess the effect of transcutaneous Spinal Cord Electrical Stimulation (tSCS) on improving motor, sensory, and autonomic function after SI. During the first 8 months, tSCS was applied alone, then, physical therapy (PT) was included in the sessions (tSCS+PT), until completion of 20 months. Compared to baseline, at 20 months, an increase in ISNCSCI motor (50 vs. 57) and sensory scores

INTRODUCTION: Spinal cord stimulation is a widespread treatment of chronic neuropathic pain from different conditions. Several novel and improving technologies have been recently developed to increase the effect of neuromodulation in patients refractory to pharmacological therapy.
RESEARCH QUESTION: To explore spinal cord stimulation's mechanisms of action, indications, and management.
MATERIAL AND METHODS: The paper initially explores the mechanism of action of this procedure based on the generation of an electric field between electrodes placed on the posterior dural surface of the spinal

Transcutaneous spinal cord stimulation (tSCS) is an emerging therapeutic strategy to target spinal autonomic circuitry to normalize and stabilize blood pressure (BP) in hypotensive persons living with chronic spinal cord injury (SCI). Our aim is to describe our current methodological approach to identify individual tSCS parameters that result in the maintenance of seated systolic blood pressure (SBP) within a pre-defined target range. The parent study is a prospective, randomized clinical trial in which eligible participants will undergo multiple mapping sessions to optimize tSCS parameter set

Regaining control of autonomic functions such as those of the cardiovascular system, lower urinary tract and bowel, rank among the most important health priorities for individuals living with spinal cord injury (SCI). Recently our research provided evidence that epidural spinal cord stimulation (ESCS) could acutely modulate autonomic circuits responsible for cardiovascular function after SCI. This finding raised the question of whether ESCS can be used to modulate autonomic circuits involved in lower urinary tract and bowel control after SCI. We present the case of a 32-year-old man with a chr

BACKGROUND: Spinal cord stimulation (SCS) is used to treat chronic pain but carries a rare risk of spinal epidural hematoma (SEH), which can cause serious neurologic injury. Urgent surgical decompression is often considered when neurologic deficits develop.
CASE REPORT: A 73-year-old man developed severe back pain during an SCS trial and was found to have a thoracolumbar SEH after trial lead removal. He remained neurologically stable during hospitalization and was managed with close observation. His pain improved, and follow-up imaging showed a reduction in the hematoma without surgery.

We hypothesized that epidural spinal cord stimulation (ES) and quipazine (a serotonergic agonist) modulates the excitability of flexor and extensor related intraspinal neural networks in qualitatively unique, but complementary, ways to facilitate locomotion in spinal cord-injured rats. To test this hypothesis, we stimulated (40 Hz) the S(1) spinal segment before and after quipazine administration (0.3 mg/kg, ip) in bipedally step-trained and nontrained, adult, complete spinal (mid-thoracic) rats. The stepping pattern of these rats was compared with control rats. At the stimulation levels used,

STUDY DESIGN: Qualitative, semi structured interviews.
OBJECTIVES: Spinal cord epidural stimulation (scES) has shown promise for improving autonomic and motor function in individuals with spinal cord injury (SCI). Participation in translational studies often requires relocation, uprooting the individual and caregivers from their community. Recognizing this burden, this study aimed to explore the motivations and expectations driving the decision to participate in a scES study.

Spinal cord injury (SCI) interrupts signal transmission between the brain and muscles, often leading to permanent motor impairments. Improving hand function is the highest priority for people with tetraplegia. Electrically engaging spinal circuits using spinal cord stimulation has been demonstrated to improve hand function in people with paralysis post-SCI. Here, we used a noninvasive intervention, transcutaneous spinal cord stimulation (tSCS), to facilitate voluntary hand function after SCI. We used a multi-cathode tSCS array to study recruitment patterns across various upper-limb muscles, in
