[The diagnostic and treatment characteristics of spinal fractures in the victims of multiple trauma].

The authors present their experience in the treatment of 107 patients with spinal injuries associated with polytrauma. The topical diagnosis of the spinal lesions in the severely injured patients was rather difficult both at the preclinical stage and during the clinical examination. For making the diagnosis in combined spinal trauma urgent laparoscopy and aimed roentgenography was employed with the patient in the dorsal position. When the latter technique is employed, a special attachment ("cassette holder") is used which allows to make the roentgenograms with mobile X-ray units. The authors h

Production of a reproducible spinal burst fracture for use in biomechanical testing.

We have developed a technique to create a reproducible spinal burst fracture of the 12th vertebral body using 6-8-week-old calf spines with ribs, muscles, and vessels resected. We used the entire thoracolumbar segment of 20 calf spines with a standardized 5-mm-deep slice placed onto the body of T12 and the T11-12 disc. We then delivered a proximal-axial impact to the vertically mounted spine, preflexed to 15 degrees of forward flexion, by dropping a 32-kg weight, guided by a 1.55-m steel rod (potential energy = 487 J). Motion was limited to anterior flexion only, at the T12-13 disc, by splinti

Imaging findings of spinal brown tumors: a rare but important cause of pathologic fracture and spinal cord compression.

Brown tumors rarely develop in the spine, and neurological compromise is exceedingly uncommon. There is a growing body of literature describing brown tumors that involve the spine, but few emphasize the radiographic findings. In the present case, we illustrate the development and progression of biopsy-proven brown tumors leading to neurological compromise through radiographs, computed tomography, magnetic resonance, and nuclear imaging acquired over a 4-year span.

Surgical management of multilevel cervical spinal stenosis and spinal cord injury complicated by cervical spine fracture.

BACKGROUND: There are few reports regarding surgical management of multilevel cervical spinal stenosis with spinal cord injury. Our purpose is to evaluate the safety and feasibility of open-door expansive laminoplasty in combination with transpedicular screw fixation for the treatment of multilevel cervical spinal stenosis and spinal cord injury in the trauma population. METHODS: This was a retrospective study of 21 patients who had multilevel cervical spinal stenosis and spinal cord injury with unstable fracture. An open-door expansive posterior laminoplasty combined with transpedicular scre

Measurement of spinal canal narrowing, interpedicular widening, and vertebral compression in spinal burst fractures: plain radiographs versus multidetector computed tomography.

OBJECTIVE: To assess the reliability of measurements of spinal canal narrowing, vertebral body compression, and interpedicular widening in burst fractures in radiography compared with multidetector computed tomography (MDCT). MATERIALS AND METHODS: Patients who had confirmed acute vertebral burst fractures over an interval of 34 months underwent both MDCT and radiography. Measurements of spinal canal narrowing, vertebral body compression, and interpedicular widening from MDCT and radiography were compared. RESULTS: The 108 patients (30 female, 78 male, aged 16-79 years, mean 39 years) had 12

Correlations of dual-energy X-ray absorptiometry, quantitative computed tomography, and single photon absorptiometry with spinal and non-spinal fractures.

Controversy continues as to which method of measuring bone mineral density (BMD) best detects osteoporosis and best correlates with fractures of the spine, hip and elsewhere. To answer these questions the prevalence of fractures was carefully determined among 90 subjects (70 with osteoporosis, 6 with mild primary hyperparathyroidism, 1 with osteomalacia and 13 normals) and simultaneous measurements were made using spinal computed tomography (QCT), spinal anteroposterior (AP) and supine lateral dual X-ray absorptiometry (DXA), femoral neck and total hip DXA, and distal third radial DXA and sing

A biomechanical analysis of spinal instrumentation systems in thoracolumbar fractures. Comparison of traditional Harrington distraction instrumentation with segmental spinal instrumentation.

A total of 61 biomechanical tests were performed on 25 cadaveric spinal segments to investigate the comparative strengths of three instrumentation systems: 1) conventional Harrington distraction instrumentation (HRI), 2) segmentally wired Harrington distraction rods, and 3) Luque segmental spinal instrumentation (SSI). In type I testing in which axial preload was applied to normal specimens, and then progressive rotation until ultimate failure followed, five of six Harrington systems failed at the bone-metal interface. In contrast, all six Luque SSI vertebral segments disrupted in a location r

Prevalence and Impact of Fractures in Persons with Spinal Cord Injuries: A Population-Based Study Comparing Fracture Rates between Individuals with Traumatic and Nontraumatic Spinal Cord Injury.

BACKGROUND: Musculoskeletal complications are one of the most common reasons for a patient with a spinal cord injury (SCI) to be rehospitalized. Bone loss due to immobilization and changes in metabolic processes because of the SCI lead to an increased risk of fractures. OBJECTIVE: To evaluate the prevalence and demographic characteristics of people living with an SCI who had a secondary fracture. METHODS: We used population health administrative data from Ontario, Canada, in individuals with either traumatic (TSCI) or nontraumatic SCI (NTSCI). Records of duplicate cases, missing unique patie

Spinal-origin abdominal pain secondary to silent osteoporotic compression fractures: A case report.

Abdominal pain in older individuals is frequently attributed to visceral disorders; however, vertebral compression fractures can present with isolated abdominal pain. Herein, we report a diagnostically challenging case that has not been previously published as a preprint. A woman in her early 80s presented with a 2-week history of persistent right upper quadrant dull pain, which had not been relieved by antispasmodic medications. Physical examination revealed mild tenderness in the right upper quadrant. Laboratory tests showed mild anemia (hemoglobin, 91 g/L) and severe osteoporosis. Results o

Posterior treatment of ankylosing spinal diseases with thoracolumbar fractures: a network meta-analysis.

OBJECTIVE: The implementation of diverse surgical techniques plays a crucial role in managing ankylosing spinal diseases (ASDs), serving as invaluable tools. Presently, posterior surgery stands out as the predominant approach owing to its familiarity with anatomical structures; however, it comprises various methodologies that require a thorough comprehension for their suitable application. Henceforth, we performed a network meta-analysis to assess and prioritize the efficacy and safety of surgical interventions for ASDs. METHODS: The databases PubMed, EMBASE, Cochrane Library, and CNKI (China

Risk factors for osteoporotic fractures in persons with spinal cord injuries and disorders.

UNLABELLED: Clinical risk factors for fracture were explored among Veterans with a spinal cord injury. At the end of 11 years of follow-up, the absolute risk of fracture was approximately 20 %. Among the clinical and SCI-related factors explored, a prior history of fracture was strongly associated with incident fracture. INTRODUCTION: Few studies to date have comprehensively addressed clinical risk factors for fracture in persons with spinal cord injury (SCI). The purpose of this study was to identify risk factors for incident osteoporotic fractures in persons with a SCI that can be easily de

Spinal and Epidural Anesthesia in Patients With Recent Stable Fractures of Vertebral Column.

BACKGROUND: The use of central neuraxial block (CNB) in patients with spinal injuries with or without spinal cord injury continues to be a contentious issue due to paucity of evidence supporting or refuting its use. There are only a few case reports reporting the use of the technique in these patients. We performed a retrospective record review of patients who underwent neuraxial blockade for lower limb orthopedic surgery in the presence of coexisting recent spine injury (defined as spine injury within 1 month) to assess the occurrence of postoperative deterioration of spinal cord function or
