The nerve root sedimentation sign in diagnosis of lumbar spinal stenosis.

BACKGROUND: The nerve root sedimentation sign is a magnetic resonance (MR) sign, shown to be present in central lumbar spinal stenosis. The lack of sedimentation of the nerve roots to the dorsal part of the dural sac is consistent with the positive nerve root sedimentation sign. PURPOSE: To validate the reliability of the nerve root sedimentation sign in diagnosis of different grades of lumbar spinal canal stenosis. MATERIAL AND METHODS: This study was a retrospective review of 101 consecutive MR imaging (MRI) studies obtained on patients with clinically suspected lumbar canal stenosis. Base

Spinal accessory nerve injury: eight cases and review of the literature.

The spinal accessory nerve (SAN) is the major motor supply to the trapezius and sternocleidomastoid muscles. The superficial course of the SAN in the posterior cervical triangle makes it vulnerable to injuries. The most common reason is an iatrogenic injury during surgery but other causes such as stretch or traction injury have also been reported. Five iatrogenic SAN injuries were occurred after radical neck dissection (RND) for laryngeal carcinoma in two patients, lymphadenectomy for oral mucosal tumor in one patient, surgery for lipoma in one patient, and lymph node biopsy for tuberculosis i

Temporal changes in MrgC expression after spinal nerve injury.

Mas-related G-protein-coupled receptor subtype C (MrgC) may play an important role in pain sensation. However, the distribution of MrgC receptors in different subpopulations of rodent dorsal root ganglion (DRG) neurons has not been clearly demonstrated owing to a lack of MrgC-selective antibody. It is also unclear whether peripheral nerve injury induces different time-dependent changes in MrgC expression in injured and uninjured DRG neurons. Here we showed that MrgC immunoreactivity is distributed in both IB4-positive (non-peptidergic) and calcitonin gene-related peptide-positive (peptidergic)

Unusual insidious spinal accessory nerve palsy: a case report.

INTRODUCTION: Isolated spinal accessory nerve dysfunction has a major detrimental impact on the functional performance of the shoulder girdle, and is a well-documented complication of surgical procedures in the posterior triangle of the neck. To the best of our knowledge, the natural course and the most effective way of handling spontaneous spinal accessory nerve palsy has been described in only a few instances in the literature. CASE PRESENTATION: We report the case of a 36-year-old Caucasian, Greek man with spontaneous unilateral trapezius palsy with an insidious course. To the best of our

Classification and clinical anatomy of the first spinal nerve: surgical implications.

OBJECT: Data regarding the first cervical nerve are scanty and conflicting, however this nerve may need to be identified for neurosurgical procedures such as rhizotomy for torticollis and suboccipital pain syndromes. The authors performed the present study to elucidate further the detailed anatomy of the first cervical nerve and review its clinical relevance. METHODS: Forty adult cadavers (80 sides) were used in this study. Dissection was performed at the craniocervical junction with special attention to the formation and presence of the components of the C-1 spinal nerve. Additionally, conne

The spinal root origins and clinical implications of the lower subscapular nerve.

The purpose of this study was to define the spinal root origins of the lower subscapular nerve and the amounts of participating nerve fibers from each spinal root and to discuss the clinical implications. Using a method of separating the nerve fascicles that traces the particular nerve fibers at the intrafascicular level, the spinal root origins of the lower subscapular nerve appeared as two types. The first type comprised 76.9% and was composed of the C5, C6, and C7 roots; the second type comprised 23.1% and was composed of the C6 and C7 roots. The number of nerve fibers was 357.2 +/- 139.7 (

Spinal myoclonus following a peripheral nerve injury: a case report.

Spinal myoclonus is a rare disorder characterized by myoclonic movements in muscles that originate from several segments of the spinal cord and usually associated with laminectomy, spinal cord injury, post-operative, lumbosacral radiculopathy, spinal extradural block, myelopathy due to demyelination, cervical spondylosis and many other diseases. On rare occasions, it can originate from the peripheral nerve lesions and be mistaken for peripheral myoclonus. Careful history taking and electrophysiological evaluation is important in differential diagnosis. The aim of this report is to evaluate the

Spinal accessory nerve meningioma in a paediatric patient: case report.

BACKGROUND: Mabry syndrome is a very rare condition characterized by cognitive impairment, distinctive facial features, hypotonia, elevated alkaline phosphatase (ALP) levels in the blood (hyperphosphatasia), and recurrent generalized tonic-clonic seizures. It can be accompanied by gastrointestinal anomalies such as Hirschsprung's disease (HD). In this study, we aim to present a case of hyperphosphatasia with impaired intellectual development syndrome 2 (HPMRS2) from Türkiye, featuring a novel homozygous p.Arg445Pro variant in the PIGO gene, occurring in association with HD. Hes1 and Hes5 regulate the development of the cranial and spinal nerve systems.

The basic helix-loop-helix genes Hes1 and Hes5, known Notch effectors, regulate the maintenance of neural stem cells and the development of the central nervous system (CNS). In the absence of Hes1 and Hes5, the size, shape and cytoarchitecture of the CNS are severely disorganized, but the development of the peripheral nervous system remains to be analyzed. Here, we found that in Hes1;Hes5 double-mutant mice, the cranial and spinal nerve systems are also severely disorganized. In these mutant mice, axonal projections from the mesencephalic neurons to the trigeminal (V) ganglion become aberrant

Variations in the surface anatomy of the spinal accessory nerve in the posterior triangle.

Iatrogenic injury to the spinal accessory nerve has been widely documented and can have medico-legal implications. The resulting syndrome of pain, paralysis and winging of the scapula are often the source of considerable morbidity. This paper researches the degree of accuracy achievable in mapping the surface anatomy of the spinal accessory nerve in the region of the posterior triangle with a view to creating a cartographical aid to surgical procedures. The necks of 25 adult cadavers were dissected bilaterally to expose the spinal accessory nerve. Variations in the course and distribution of t

Sacral spinal nerve stimulation for faecal incontinence: multicentre study.

BACKGROUND: In patients with faecal incontinence in whom conservative treatment fails, options are limited for those with a functionally deficient but morphologically intact sphincter. We investigated the effect of sacral nerve stimulation on continence and quality of life. METHODS: In this multicentre prospective trial, 37 patients underwent a test stimulation period, followed by implantation of a neurostimulator for chronic stimulation in 34. Effect on continence was assessed by daily bowel-habit diaries over a 3-week period and on quality of life by the disease-specific American Society of

Peripheral and spinal motor reorganization after nerve injury and repair.

Functional recovery after peripheral nerve injury depends on the amount as well as on the accuracy of reinnervation by regenerative axons. In this study, the rat sciatic nerve was subjected to crush injury or complete transection repaired by either (1) straight nerve suture, (2) crossed nerve suture of tibial and peroneal fascicles, or (3) silicone tubulization leaving a gap of 4 mm. The compound muscle action potentials (CMAP) of gastrocnemius, tibialis anterior and plantar muscles were recorded 90 days post operation to assess functional reinnervation and Fast Blue, Fluoro Gold and DiI were

Anomalous relationship of the spinal accessory nerve to the internal jugular vein.

A patient with an early cancer of the tongue and undergoing a staging neck dissection was noted to have an anomalous relationship of the internal jugular vein and spinal accessory nerve. At the upper end of the dissection (level II, Memorial Sloan-Kettering classification), the internal jugular vein was observed to divide above and reconstitute below the spinal accessory nerve. Although apparently not previously described in the literature, this finding may be encountered by other surgeons who operate in this area, and it is important that these anatomical variations are borne in mind to preve

Local application of disc-related cytokines on spinal nerve roots.

STUDY DESIGN: To analyze the effects of tumor necrosis factor-alpha, interleukin-1beta, and interferon-gamma on cauda equina function and to define if any of these cytokines could induce nerve root dysfunction comparable with the situation with application of nucleus pulposus. SUMMARY OF BACKGROUND DATA: Proinflammatory cytokines derived from the intervertebral disc have been suggested to mediate the nucleus pulposus-induced nerve root injury following local application of nucleus pulposus. However, it is not known if such cytokines may induce similar injury if applied separately. METHODS: A

Surgical approach to the spinal accessory nerve for brachial plexus reconstruction.

An easy, safe, and less invasive surgical approach to the spinal accessory nerve for brachial plexus reconstruction is described. The technique avoids a longitudinal unsightly scar over the neck and preserves the branches innervating the upper part of the trapezius.

The location of the spinal nerve root on plain radiographs of the cervical spine.

Twelve cervical spines from C2 to T1 were harvested from embalmed cadavers to study the location of the nerve root on plain radiographs. After removal of the soft tissue, the spinal nerves just lateral to the transverse processes were exposed and injected with lead oxide. Plain radiographs including anteroposterior (AP) and left and right oblique views were taken. Angular and linear measurements were performed directly on the radiographs. Results showed that the average frontal angle of the nerve root for all levels was 155 degrees on the AP view, 108 degrees on the foraminal side of the obliq

Glutamate receptors in spinal motoneurons after sciatic nerve transection.

Severing the axon of a neuron triggers profound changes in its soma, beginning within a few days and becoming maximal within a few weeks. Unravelling these changes bears directly on our understanding of degeneration and regeneration after injury. Classically described chromatolysis arises from reorganization of rough endoplasmic reticulum, associated with biosynthetic changes in response to injury. Since motoneurons, in contrast with other central neurons, are able to regenerate their axons, their response to axotomy is of special interest. For successful regeneration, a neuron must shift its

Thoracic spinal nerve and root conduction: a magnetic stimulation study.

We describe a technique of magnetic coil (MC) stimulation of the thoracic spinal nerves and roots in 12 normal subjects and a patient with diabetes mellitus. We kept the MC flat against the vertebral column in the midline over T-7, T-8, and T-9 spinous processes and obtained compound muscle action potentials from the upper rectus abdominis, external oblique, and intercostal muscles. We obtained mean latencies to these muscles after stimulation in the posterior axillary line. We noted that the onset latencies remained fixed despite increasing the intensity of stimulation from 30% to 100% and on
