Endourological evaluation and management of leukoplakia of the renal pelvis.

Since August 1989, we have seen 4 patients with leukoplakia of the renal pelvis associated with a longstanding renal stone. In 2 of them, excretory or retrograde pyelography revealed multiple filling defects in the left renal pelvis as well as a renal stone, although urine cytological examination was negative. One of the other 2 patients underwent extracorporeal shock wave lithotripsy (ESWL) for the renal stone, but this was not followed by the passage of stone fragments. The renal stone in the remaining patient was associated with staghorn calculi. For stone extraction as well as endoscopic e

Magnetic resonance imaging of the kidneys.

A study of the magnetic resonance imaging (MRI) appearances of the kidneys in six normal volunteers and 52 patients is reported. Corticomedullary differentiation was seen with the inversion-recovery (IR 1400/400) sequence in the normal volunteers and in patients with functioning transplanted kidneys and acute tubular necrosis. Partial or total loss of corticomedullary differentiation was seen in glomerulonephritis, acute and chronic renal failure, renal artery stenosis, and transplant rejection. The T1 of the kidneys was increased in glomerulonephritis with nephrotic syndrome, but the T1 was w

Squamous cell carcinoma of the renal pelvis.

Primary squamous cell carcinoma (SCC) of the renal pelvis is rare because SCC represents only 0.5% to 0.8% of malignant renal tumors. Chronic irritation, inflammation, and infection induce squamous metaplasia of the renal collecting system, which may progress to dysplasia and carcinoma in most of affected individuals. Nephrolithiasis, especially formation of staghorn calculi, is the most common risk factor for SCC, which usually occurs in older adults (age 50-70 years) with no sex predilection. Clinical features include flank or abdominal mass, weight loss, hematuria, or paraneoplastic syndrom

A feasibility study of a novel irrigation technique for reducing intrapelvic pressure during ureterorenoscopy using conventional devices.

PURPOSE: Ureterorenoscopy (URS) is a widely accepted approach for managing upper urinary tract stones, including renal calculi measuring > 20 mm. However, an elevated intrapelvic pressure (IPP) during URS can lead to infectious and procedural complications. We developed a simple irrigation technique and evaluated its efficiency in reducing IPP during URS. METHODS: In this technique, a 14/16-Fr ureteral access sheath (UAS) was advanced to the ureteropelvic junction or distal to the stone. A 4.7-Fr single-J ureteral stent was inserted through the UAS, with the tip placed in the renal pelvis or u

Application of warning biopsies in percutaneous nephrolithotomy.

BACKGROUND: At present, few articles on percutaneous nephrolithotomy (PCNL) for renal calculi and renal pelvic tumors detected by intraoperative biopsy exist, which has provided limited guidance for clinical practice. In this article, we aimed to further study the relationship between renal calculi and renal pelvic tumors. METHODS: We retrospectively analyzed the medical records of patients with abnormal mucosal biopsy results who underwent PCNL for kidney stones in the Urology Department of Peking University People's Hospital from January 2011 to November 2021. RESULTS: In total, 2,801 pati

Open stone surgery in the treatment of bilateral complex renal stones with left infected hydronephrosis: A case report.

INTRODUCTION AND IMPORTANCE: With the strong development of percutaneous nephrolithotomy as well as other less invasive procedures, the indication of open surgery for the treatment of nephrolithiasis has been significantly reduced and is only applied in selective cases. CASE PRESENTATION: A 55-year-old male was admitted to the Department of Urology due to fever and left flank pain. Clinical examination and imaged studies reveal staghorn calculi in the right kidney and infected hydronephrosis with multiple stones on the left side. The stone removal surgery was performed in 2 sessions, left fir

[Current status of extracorporeal shock wave lithotripsy in the treatment of urolithiasis].

Extracorporeal shock wave lithotripsy became a main treatment method for the upper urinary tract calculi. About 500 ESWL machines of 28 types are currently used in Japan and ESWL therapy occupies more than 90% of the surgical intervention for upper tract stones. The doctors who are engaging in the ESWL therapy are strongly advised to know the features of their own ESWL machines, since the treatment strategy may differ considerably depending on the type of machines. The strategy has changed from the in-patient ESWL under anesthesia by original Dornier HM-3 to out-patient anesthesia-free treatme

Case of the diffuse form of xanthogranulomatous pyelonephritis.

Xanthogranulomatous pyelonephritis (XGP) is a rare entity and constitutes less than 1% of chronic pyelonephritis. A 71-year-old male was introduced to our department with general malaise and abnormal findings of computed tomography (CT). Abnormal findings of complete blood count and laboratory examination included an elevated WBC count and C-reactive protein. Urinalysis showed combined hematuria and pyuria, and Escherichia coli was detected in urine culture. Abdominal CT revealed left hydronephrosis with staghorn renal calculi and thin cortex of the left kidney. Because of poor condition, the

Retroperitoneal vs transperitoneal laparoscopic pyelolithotomy; a single surgeon's experience.

OBJECTIVE: To compare the outcomes of transperitoneal and retroperitoneal laparoscopic pyelolithotomy procedures. METHODS: A total of 104 consecutive laparoscopic pyelolithotomy surgeries performed by a single surgeon on patients with staghorn or renal pelvic calculi larger than 20 mm were evaluated. Intraoperative and postoperative clinical parameters from two groups, transperitoneal (TLPL) (N = 55) and retroperitoneal (RLPL) (N = 49), were compared. The surgeon performed TLPL for the first five years and then switched to the RLPL approach for the next five years. RESULTS: There were no sig

[Free peritoneal patch grafts in surgery on renal pelvis and ureter (author's transl)].

36 operations of the renal pelvis or ureter were carried out in 35 patients. In all these cases a split stenosis or a tissue defect were covered by free peritoneal patches. Urographic controls of 30 patients showed 29 good or very good results. The following statements can be made: 1. In cases of a secondary scarred stenosis, of renal pelvic or ureteral wall defects, plastic operations of intrarenal pelvis of transsinus operations of staghorn calculi the splitting with splinting and covering by autologous tissue seems to represent a suitable operation. -- 2. As material the renal capsule, the

Incidentally detected primary squamous cell carcinoma of the kidney: Case series with review of the literature.

Squamous cell carcinoma of the kidney is a rare malignancy, usually not suspected clinically because of the lack of any pathognomic sign and ambiguous clinical and radiological features, and thus, patients present at advanced stages resulting in poor prognosis. We report here four cases of incidentally diagnosed primary renal squamous cell carcinoma that were treated at our hospital. The mean age of the patients (two females and one male aged above 60 years, except one male whose age was 25 years) was 60 years. Both the males and one of the females had a history of multiple renal calculi, and

[Staghorn renal lithiasis treated with shock waves. Bacteriologic aspects].

Struvite renal stones are caused by infection of the urine with bacteria that synthesize the enzyme urease. Ammonium is released by the breakdown of urea by urease, the urine becomes highly alkaline, and magnesium ammonium phosphate (struvite) and carbonate apatite crystallize. Incorporation of the infecting bacteria within the developing stone, results in a focus of infection that is resistant to conventional antimicrobial therapy, and which is manifested clinically by repeated urinary tract infection caused by persistent bacteriuria. Extracorporeal shock wave lithotripsy (ESWL) currently is

Access to the upper calyx in supine position: breaking a myth.

The feasibility and safety of upper calyx access in supine percutaneous nephrolithotomy (PCNL) remains controversial. We aimed to detail our technique and describe the outcomes in effectivity as well as safety in a large cohort. The authors analyzed patients with renal and proximal ureteral stones who underwent supine PCNL using upper calyx access at Rio Hortega University Hospital Valladolid (Spain) between 2013 and 2023. Patients were placed in the Galdakao-modified supine Valdivia position. Transpapillary percutaneous access to the upper calyx was guided by a combination of ultrasound (US)
