Optimising the dose of oral midazolam sedation for dental procedures in children: a prospective, randomised, and controlled study.

BACKGROUND: Midazolam sedation poses a significant dilemma in paediatric dentistry, which is to find out the optimal dosing with minimal undesirable adverse events. In this study, we aimed to compare the effect of three doses of oral midazolam (0.5, 0.75, and 1 mg/kg) on the sedative state and cooperative behaviour of children during dental treatment. We further compared completion rates, parent satisfaction, and all adverse events. DESIGN: Ninety children aged 3-10 years were randomised to three equal groups. Groups A, B, and C received 0.5, 0.75, and 1 mg/kg of oral midazolam, respectively.

Designing clinically useful systems: examples from medicine and dentistry.

Despite promising results in medical informatics research and the development of a large number of different systems, few systems get beyond a prototype state and are really used in practice. Among other factors, the lack of explicit user focus is one main reason. The research projects presented in this paper follow a user-centered system development approach based on extensive work analyses in interdisciplinary working groups, taking into account human cognitive performance. Different medical and health-care specialists, together with researchers in human-computer interaction and medical info

On the Future Design of Bio-Inspired Polyetheretherketone Dental Implants.

Polyetheretherketone (PEEK) is a promising implant material because of its excellent mechanical characteristics. Although this polymer is a standard material in spinal applications, PEEK is not in use in the manufacturing of dental implants, where titanium is still the most-used material. This may be caused by its relative bio-inertness. By the use of various surface modification techniques, efforts have been made to enhance its osseointegrative characteristics to enable the polymer to be used in dentistry. In this feature paper, the state-of-the-art for dental implants is given and different

Methoxyflurane: a review with emphasis on its role in dental practice.

Methoxyflurane was developed as an anaesthetic agent and introduced into clinical practice in 1960. It soon became evident that it possessed analgesic properties that other drugs did not. Due to toxicity concerns, it lost favour in general anaesthesia and had been largely abandoned by the late 1970s. The manufacturer withdrew it in 1999, and the Food and Drug Administration in the United States did not renew its licence in 2005. It has also been withdrawn by the European Union. However, it continues to be used in Australasia, primarily as an inhaled self-administered analgesic by emergency ser

Dental age estimation in the Chhattisgarhi population: A comparative study of Demirjian and Willems methods.

BACKGROUND: Forensic identification, pediatric dentistry, and medico-legal investigations all often utilize dental age estimation. Demirjian and Willems methods are frequently used when estimating dental maturity; however, their accuracy varies across different groups, pointing to the need for population specific validation. AIM: Aim of the current study was to compare and assess the reliability of the Demirjian and Willems for estimating dental age in the children from the Indian state of Chhattisgarh. METHODS: A retrospective cross-sectional study was conducted using 142 orthopantomograms

US/ROS-responsive ALN-modified polymer nanoparticles enhance the efficacy of infiltration anesthesia through improved bone targeting and permeation.

Complex maxillofacial bone anatomy compromises the efficacy of local anesthetics. This study develops an ultrasound (US)/reactive oxygen species (ROS) cascade-responsive polylactic-coglycolic acid (PLGA) nanoparticle system for enhanced dental infiltration anesthesia and perioperative pain management. The nanoparticles, which were surface-modified with alendronate (ALN) via an ROS-cleavable thioketal (TK) linker and coloaded with bupivacaine (BUPI) and perfluorohexane (PFH), enabled enhanced bone targeting and penetration. Under US stimulation, PFH undergoes a liquid-gas phase transition, gene

Hydroxyapatite Dental Inserts for Tooth Restoration: Stress and Displacement Analysis.

Hydroxyapatite (HAP) inserts minimize restoration contraction by constituting a major part of the restoration; however, their effect on the relaxation of tooth tissues has not been previously tested. Finite element analysis was employed to estimate stress and displacement when HAP inserts with a thickness of 1.7 mm or 4.7 mm and a diameter of 4.7 mm were used to substitute for dentin. The volumetric contraction of the composite during polymerization, simulated through steady-state heat transfer analysis, yielded a contraction rate of 3.7%. Descriptive statistics revealed that the incorporation

Too Much of a Good Thing: A Case of Acute Fluoride Ingestion.

A protective effect of fluoride for the prevention of dental caries was observed in communities that consumed naturally fluoridated water compared to non-fluoridated water. Subsequently, fluoride began to be added to the drinking water in various countries as a means of caries prophylaxis. Dentistry began to develop the use of fluoride as an additive to toothpastes and mouth rinses as well as in the form of sodium fluoride powder. Fluoride gels, tablets, and powders can be used safely but can lead to acute ingestions. Here we present the case of a 36-year-old male patient with a history of dep

Antimicrobial management of third molars: survey results for military dentists.

In a survey of military dentists to examine use of antimicrobial agents in the management of third molars, questions addressed use of antibiotics and an antimicrobial rinse in treating pericoronitis and third molar extractions. Results were compared with information from a literature review. According to the survey, a majority of clinicians use antibiotics to treat pericoronitis but not surgical extraction of asymptomatic dental impactions. About 60 percent of respondents use a preoperative rinse with chlorhexidine in treating the third molar conditions discussed. A postoperative rinse with ch

Behavior management of children in the dental office.

One of the first conclusions offered by this article was that behavior management has been a pertinent theme in literature addressing clinical pediatric dentistry. This has been true for much of the twentieth century. Another conclusion stated that this literature generally focuses on the preschool child after the child has acquired sufficient development and maturity that he or she can be communicated to in conversation. It was also pointed out that most general approaches to the subject of behavior management deal with the normal child, which for the purposes of this article can be construed
