Dynamic evaluation of the sternoclavicular and acromioclavicular joints using an upright four-dimensional computed tomography.

In the analysis of the shoulder complex, the sequential changes occurring in the sternoclavicular and acromioclavicular joints during active shoulder motion are challenging to track. This study aimed to investigate the in vivo sternoclavicular and acromioclavicular joint motions during active elevation, including the sequential changes in these joint spaces using upright four-dimensional computed tomography (4DCT). Bilateral shoulders of 12 healthy volunteers upright 4DCT were obtained during active elevation similar to a "hands up" motion. The sternoclavicular and acromioclavicular rotation a

A Current Concepts Review of Clavicle Injuries in Ice Hockey From Sternoclavicular to Acromioclavicular Joint.

Clavicle injuries are common in ice hockey, and a number of high-profile fractures and dislocations have occurred in elite hockey players in recent years. Acromioclavicular joint injuries, clavicle fractures, and sternoclavicular joint injuries are some of the most frequent hockey-related injuries treated by orthopaedic surgeons, and familiarity with the management of these injuries and sport-specific considerations for treatment and recovery are critical. Injuries involving the clavicle can sometimes be life-threatening, and subtle findings on physical examination and radiographic studies can

Escherichia coli bacteremia with secondary seeding in the sternoclavicular joint: A case report and literature review.

INTRODUCTION: Septic arthritis of the sternoclavicular joint (SCJ) is a rare phenomenon in healthy individuals. It is generally described in patients with predisposing risk factors such as diabetes mellitus, intravenous drug use, immunosuppression, chronic liver disease and central venous catheters. CASE REPORT: We describe a case of Escherichia coli related SCJ septic arthritis in an otherwise healthy adult with review of the literature. DISCUSSION: There were only two previous reports of patients with E. coli and to our knowledge, this is the third case of a patient with SCJ septic arthrit

Early Detection of Inflammation and Joint Destruction Revealed by Ultrasound in a Patient with Sternoclavicular Septic Arthritis.

A 53-year-old Japanese man experienced fever/neck pain, and neck magnetic resonance imaging revealed a spinal epidural abscess. The following day, he developed a palpable mass with evident inflammatory signs in the right sternoclavicular joint (SCJ) with severe pain. Ultrasound revealed synovitis with remarkable power Doppler signals in the right SCJ. Blood cultures yielded Streptococcus agalactiae growth. After 12 days, ultrasound showed right distal clavicle bone erosion. His symptoms improved with long-term parenteral antibiotic treatment, but the right SCJ joint destruction progressed for

Sternoclavicular joint septic arthritis with chest wall abscess in a healthy adult: a case report.

BACKGROUND: Septic arthritis of the sternoclavicular joint is rare. It can be associated with serious complications such as osteomyelitis, chest wall abscess, and mediastinitis. In this report, we describe a case of an otherwise healthy adult with septic arthritis of the sternoclavicular joint with chest wall abscess. CASE PRESENTATION: A 68-year-old Japanese man presented to our hospital complaining of pain and erythema near the right sternoclavicular joint. Despite 1 week of oral antibiotics, his symptoms did not improve. Computed tomography revealed an abscess with air around the right pec

[2 cases of pericardial tamponade caused by migration of fracture wires from the sternoclavicular joint].

Two cases of dislocation of the sternoclavicular joint are reported in which migrating nails caused fatal heart tamponade. In both cases death occurred due to fracture nails which had not been bent or secured against migration in some way, and X-ray control had been inadequate. The doctors were charged with manslaughter by negligence since they had not exercised reasonable care as would have been required in those circumstances. As a result of the two attending physicians, trials, it is concluded when that operative osteosynthesis techniques are applied in order to affix dislocations of the st

Traumatic posterior dislocation of the sternoclavicular joint in a 12-year-old boy: a case report.

Sternoclavicular joint (SCJ) dislocation is a rare but serious orthopedic injury, representing fewer than 1% of all fractures or dislocations. Posterior dislocations are particularly concerning due to the SCJ's proximity to vital structures such as the trachea, esophagus, subclavian vessels, and brachial plexus. Clinical suspicion should be heightened when a patient presents with a compression-type injury to the shoulder girdle and pain around the SCJ. We report the case of a 12-year-old boy who presented to the emergency department after being punched in the right shoulder by another child, r

Unusual Sternoclavicular Joint Infection With Pseudomonas in a Young Adult With Hidradenitis Suppurativa: A Case Report.

Sternoclavicular joint infection is a relatively rare and serious condition, and its association with Pseudomonas aeruginosa is uncommon. Moreover, hidradenitis suppurativa is a chronic inflammatory skin disease that may predispose patients to deep-seated infections, as illustrated by this unusual report. We present a case of a 31-year-old male patient with a Pseudomonas aeruginosa sternoclavicular joint infection. The subject presented for a three-month history of swollen, erythematous, and tender right sternoclavicular joint, refractory to non-steroidal anti-inflammatory drugs (NSAIDs) and s

Numerical investigation of the sternoclavicular joint modeling technique for improving the surgical treatment of pectus excavatum.

It is now common to perform the Nuss procedure as a surgical treatment for pectus excavatum. As several types of detailed surgical methods exist as part of the Nuss procedure, studies are currently being conducted to verify their relative superiority via computerized biomechanical methods. However, no studies have considered the influence of sternoclavicular joints on the simulations of the Nuss procedure. Accordingly, this study aims to demonstrate the influence of these joints by comparing the clinical data with the finite element analysis data. Scenarios were set by classifying the movement

Successful treatment in a child with enthesitis-related arthritis involving the sternoclavicular joint: a case report.

BACKGROUND: Although the sternoclavicular joint (SCJ) may be involved in ankylosing spondylitis, rheumatic arthritis, and Behçet's disease and participates in the systemic inflammatory process of arthritis, it is often neglected during routine rheumatologic clinical examinations. To the best of our knowledge, this is the first study to report etanercept treatment in juvenile idiopathic arthritis (JIA) with SCJ involvement. CASE PRESENTATION: In this study, we describe an unusual case of a child with juvenile idiopathic arthritis with an initial presentation of sternoclavicular mass. The patie

[Acute posterior dislocation of the sternoclavicular joint: Reduction, fixation by endobutton system and alternative procedures].

Acute posterior dislocation of the sternoclavicular (SC) joint is rare but can lead to life-threatening vascular injuries of the mediastinum; however, diagnosis is difficult and the injury can be initially overlooked so that surgical treatment is delayed. Although a variety of different treatment modalities have been published, the ideal fixation technique has not yet been identified. We report the case of a patient suffering from a locked posterior SC joint dislocation caused by a skiing accident. The injury was treated by transarticular endobutton fixation. This article describes the techniq

Osteoradionecrosis of the head and neck: a case of a clavicular-tracheal fistula secondary to osteoradionecrosis of the sternoclavicular joint.

Radiation therapy is an integral part of treatment for head and neck cancer, but its use is not without complications. We describe the first reported sternoclavicular-tracheal fistula resulting from osteoradionecrosis (ORN) at the medial clavicle. This ORN resulted from definitive radiation therapy for a primary pyriform sinus squamous cell carcinoma. The diagnosis of ORN was made by fiberoptic bronchoscopy. The physiologic damage of ORN is based on a compromised blood supply and altered metabolism of bone formation secondary to effects of ionizing radiation. Treatment requires meticulous hygi

Escherichia coli associated hematogenous sternoclavicular joint osteomyelitis: A rare condition with a rare causative pathogen.

Escherichia coli is the most common microorganism that causes urinary tract infections (UTIs), including acute prostatitis. However, E. coli osteomyelitis, especially ones that involve the sternoclavicular joint, are rare hematogenous complications. We present a case of an immunocompetent man who presented with symptoms of UTI and right shoulder pain. Urine cultures and blood cultures grew E. coli. There was radiographic evidence of prostatic abscess and a right sternoclavicular joint osteomyelitis. This case is unique given the rare occurrence of non-traumatic clavicular bacterial osteomyelit

Acute Chest Wall Infections: Surgical Site Infections, Necrotizing Soft Tissue Infections, and Sternoclavicular Joint Infection.

Acute chest wall infections are uncommon and share similar risk factors for infection at other surgical sites. Smoking cessation has been shown to decrease the risk of surgical site infection. Depending on the depth of infection and/or involvement of the organ space, adequate therapy involves antibiotics and drainage. Early diagnosis and debridement of necrotizing soft tissue infections is essential to reduce mortality. Sternoclavicular joint infections require surgical debridement, en bloc resection, and antibiotic therapy. A standard approach to wound closure after resection has yet to be es

Chronic posterior sternoclavicular joint fracture dislocations in children and young adults: results of surgical management.

BACKGROUND: Posterior sternoclavicular joint (SCJ) fracture dislocations in children and young adults can be missed on initial examination or could fail closed treatment. We hypothesize that surgical interventions (open reduction and ligamentous reconstruction or medial clavicle resection depending on the clinical scenario) are safe treatment options in patients with symptomatic chronic posteriorly displaced SCJ injuries. METHODS: A retrospective review was conducted of all patients younger than 25 years of age who underwent surgical treatment for chronic posteriorly displaced SCJ injuries at

[Short-term results of conservative treatment without reduction maneuver of the anterior sternoclavicular joint dislocation].

BACKGROUND: To evaluate the results of the conservative treatment without a reduction maneuver in patients with anterior sternoclavicular joint (SCJ) dislocation. METHODS: Four cases with anterior dislocation of the SCJ were evaluated retrospectively. Three cases were unconscious and were followed in the intensive care unit. All of the cases were treated conservatively without any reduction maneuver, as they are either unconscious or refused the reduction in one case. Average follow-up period was 16 months (10-21 months). RESULTS: Good clinical results were achieved in three cases with only

A case of hip joint septic arthritis due to haematogenous infection, which was misdiagnosed sternoclavicular joint septic arthritis as SAPHO syndrome.

Anterior chest pain associated with sternoclavicular arthritis has been considered a symptom specific to SAPHO syndrome. Differentiating aseptic arthritis of the SAPHO syndrome from infectious sternoclavicular joint (SCJ) arthritis is often difficult. We reported a 55-year-old woman with left hip joint and right SCJ pain. Diagnosis and treatment were delayed because she was misdiagnosed with SAPHO syndrome, and haematogenous infection spread to the hip joint septic arthritis. She was diagnosed promptly after presenting to our hospital and underwent early debridement, and her hip joint and SCJ
