The prevalence of strabismus in unilateral coronal synostosis.

BACKGROUND: While there is a clear correlation between unilateral coronal synostosis (UCS) and ocular motility abnormalities, the literature provides little information as to the true epidemiology of strabismus, or the underlying etiology of these paralleled pathologies. The purpose of this study is to investigate the rate of oculomotor abnormalities associated with UCS and its management. METHODS: A retrospective review of all patients identified to have single-suture, nonsyndromic UCS treated by fronto-orbital advancement at a tertiary craniofacial referral center from 1977 to 2013 was perf

Refractive surgery: a treatment for and a cause of strabismus.

PURPOSE OF REVIEW: The purpose of this article is to review refractive surgery as a means of treatment for strabismus and as a potential cause of strabismus and binocular diplopia. RECENT FINDINGS: Numerous studies have reported that refractive surgery is effective in correcting certain types of strabismus such as accommodative and partially accommodative esotropia. Studies on the treatment of exotropia related to anisometropia have demonstrated less favorable outcomes. In patients without manifest strabismus preoperatively, recent studies have shown that strabismus and diplopia can occur aft

Amblyopia and strabismus in Iranian schoolchildren, Mashhad.

PURPOSE: To determine the prevalence of amblyopia and strabismus among the population of schoolchildren Mashhad, Iran. MATERIALS AND METHODS: In a cross-sectional study with cluster sampling, 2510 schoolchildren were selected from schools of district 1 in Mashhad. Uncorrected visual acuity (UCVA) and best corrected visual acuity (BCVA) were recorded for each participant. Amblyopia was distinguished as a reduction of BCVA to 20/30 or less in one eye or 2-line interocular optotype acuity differences in the absence of pathological causes. RESULTS: Of the 2510 selected schoolchildren, 2150 parti

Manifested strabismus in a case of Apert syndrome.

A full-term female baby was diagnosed as having Apert syndrome with craniosynostosis, hypertelorism, syndactyly, polydactyly, and cleft plate. At her first ophthalmic visit at 8 months old, she was noted to have bilateral exophthalmos with epiblepharon on her lower lids, exotropia, and right inferior oblique muscle overaction (IOOA) and manifested right dissociated vertical deviation (DVD). There was no keratopathy or optic neuropathy. Bilateral lateral rectus muscles and right inferior oblique muscle recession were performed with correction of bilateral epiblepharon at 1 year of age. Prevalence of strabismus in patients with pathologic myopia.

BACKGROUND: To determine the prevalence and clinical features of strabismus in patients with pathologic myopia. METHODS: Medical records of a total of 636 highly myopic patients were retrospectively reviewed. Pathologic myopia was defined as spherical equivalent (SE) of at least -8D, or axial length >26.5 mm in patients older than 9 years, <-4D in those younger than 5 years, <-6D in those aged from 6 to 8 years. Myopic refractive degree, axial length measurements, best-corrected visual acuity and eye position were then analyzed. RESULTS: Among 636 patients with pathologic myopia, 520 (81.8%)

The role of botulinum toxin in decompensated strabismus.

BACKGROUND: The surgical management of decompensated strabismus is well described. Conservative treatment may employ the use of lenses and prisms. However, the use of botulinum toxin in decompensated squint is poorly documented. In these patients, with the potential for fusion, botulinum toxin may serve to stabilize the ocular alignment by bringing the images within the fusion range. PURPOSE: The aim of this study was to assess the role of botulinum toxin in the treatment of decompensated squints. METHODS: A retrospective case series involving 92 patients with decompensated squints between D

An investigation into the mechanisms causing antipodean strabismus.

BACKGROUND: Antipodean strabismus is a rare clinical entity in which a patient manifests an esotropia when fixating with one eye and an exotropia when fixating with the other eye. It has been described in the settings of marked uncorrected anisometropia, dissociated strabismus or combinations of paresis, and mechanical restriction of eye movement. METHODS: A retrospective review of four patients with antipodean strabismus. RESULTS: All patients demonstrated a unique and uncommon pattern in which there was esotropia with one eye fixating and exotropia with the other eye fixating. None of the

Paralytic strabismus as a manifestation of lyme borreliosis.

Lyme disease is a multi-system organ disorder caused by Borrelia burgdorferi. Although ocular manifestations have been reported, these remain a rare feature of the disease. This report shows a 49-years old patient that has been bitten by a tick and as consequence of which developed symptoms of the Lyme disease. In 1998 the patient was hospitalized in our Eye Clinic due to operating treatment of the paralytic strabismus (abductal nerve paralysis), as a rare feature of the Lyme disease. Postoperative squint angle was significantly reduced, but without any temporal movement. Diplopia was still pr

Strabismus surgery in children with Möbius syndrome.

Möbius syndrome is a congenital disorder of facial diplegia associated with lateral gaze paralysis. Although palsy of the sixth and seventh cranial nerves is the minimum diagnostic finding for Möbius syndrome, neuropathologic evidence indicates that this is a more complex syndrome.(1) Clinically, it is characterized by a total absence of facial expression and severe esotropia. Other anomalies may be associated with this syndrome, especially other cranial nerve palsies and Poland syndrome. The etiology of this syndrome has not been clearly established. [Vertical and tortional deviations in early strabismus].

The occlusion of one eye may trigger two types of deviation: 1) Heterophorias: the occluded eye deviates towards a horizontal, vertical or torsional abnormal position of rest. Fusion keeps the eyes straight during binocular fixation. 2) Dissociated deviations, horizontal (DHD), vertical (DVD), torsional (DTD): they are found in infantile strabismus. The deviation without fixation is always smaller than the deviation of the occluded eye. The more typical cases are the ones where the position of rest without fixation is an orthoposition. Normal binocular vision is lacking. Most of the time, an a

[Vertical strabismus and methods for its treatment].

The paper analyses results after treatment of 80 children with vertical squint, of them 8 patients had vertical squint alone and the rest--association of vertical and horizontal squint. Most of the children had divergent vertical deviation. For correction of vertical squint, myoplastic operations were used. In case of mixed squint with the angle of 5-7 degrees, the lengthening of horizontal muscles was made at the first stage. In case of more pronounced angles of vertical deviation, one-stage lengthening of horizontal and vertical rectus muscles was made. In case of vertical squint alone, supe

Unusual complication of strabismus and lid surgery.

A patient with longstanding dysthyroid eye disease developed a broad adhesion between the globe and the upper eyelid following simultaneous surgery to recess the levator palpebrae superioris and lateral rectus muscles. This unusual complication may have developed as a consequence of failure to suture the conjuctiva of the upper fornix to the tarsus and also due to the prolonged static contact between the resulting adjacent raw areas due to postoperative eyelid traction and pressure dressings. Periods of such contact may have been further prolonged by the action of diazepam, which reduces the p

Variability of strabismus surgery for acquired esotropia.

We developed a mathematical model to predict the probable success rate of the first surgery to correct acquired esotropia with normal accommodative convergence-accommodation ratio in children. A computer-averaged dose-response curve of the effect of 68 bilateral medial rectus recessions was combined with various assumptions regarding likely errors in surgical performance and likely errors in measurement of the preoperative deviation. Fifteen different sets of reasonable assumptions were tested, giving a range of theoretical success rates from 72% to 96%. The results of this study clearly indic

Treatment of strabismus in adults with botulinum toxin A.

Eighty-five adults with horizontal concomitant strabismus were treated with an injection of a low dose of botulinum toxin A (BTXA) into the lateral or medial rectus muscle. The ocular deviation was reduced by an average of 60% independently of its size and whether or not surgery had previously been performed. The change was temporary, however, except in those cases with binocular functions, when fusion was re-established. Repeated low dose injections can maintain the improvement, but at higher doses, although larger reductions are produced, temporary local side effects--ptosis and vertical str

Concomitant strabismus as a sign of intracranial disease.

A child with a postoperative residual estropia of 16 diopters was suddenly found to have 75 D of esotropia for distance and 65 D of estropia for near. Full versions were noted that would be inconsistent with sixth nerve palsy. Divergence palsy is characterized by a substantially larger convergent strabismus for distance that, in this case, was only 10 D. One is left with the conclusion that concomitant strabismus may indicate the onset of intracranial disease that, in this case, was caused by an arteriovenous malformation.

[Early diagnosis of congenital and infantile strabismus].

Early detection of strabism by the pediatrician is essential for induction of an effective ophthalmologic therapy. Evaluation should not only be done in children with manifest strabism, but also in children prone to develop this disorder like premature-born infants, children with cerebral defects or even children from families with high incidence of strabism.
