Dynamics of subdural hygroma following decompressive craniectomy: a comparative study.

OBJECT: This retrospective comparative cohort study was aimed at discovering the risk factors associated with subdural hygroma (SDG) following decompressive craniectomy (DC) to relieve intracranial hypertension in severe head injury. METHODS: Sixty-eight of 104 patients who had undergone DC during a 48-month period and survived > 30 days were eligible for this study. To assess the dynamics of subdural fluid collections, the authors compared CT scanning data from and the characteristics of 39 patients who had SDGs with the data in 29 patients who did not have hygromas. Variables significant in

Arachnoid cysts associated with post-traumatic and spontaneous rupture into the subdural space.

Post-traumatic or spontaneous rupture of an arachnoid cyst resulting in a subdural haematoma is rare. Much more rarely, a ruptured arachnoid cyst may be present with a subdural CSF collection without evidence of haemorrhage. These are most commonly seen in the middle cranial fossa, where arachnoid cysts occur most frequently. In this paper, five teenage patients (four male, one female) are reported with post-traumatic or spontaneous arachnoid cyst ruptures, resulting in subdural haematomas in four patients and a subdural CSF collection without haemorrhage in one patient. Possible pathogenesis

[Liliequist membranotomy for patients with ruptured intracranial aneurysms (author's transl)].

In order to reconstruct the blocked CSF pathway, we attempted to excise the Liliequist membrane in 22 patients with subarachnoid hemorrhage resulted from ruptured intracranial aneurysms. After clipping the aneurysmal neck, the Liliequist membrane was reached through the space between the optic nerve and the internal carotid artery within the same operative field. As a result, the incidence of postoperative ventricular dilatation was remarkably reduced in comparison with control cases without Liliequist membranotomy. The necessity for the shunt operation for post-SAH hydrocephalus was also redu

Roles of tissue plasminogen activator in chronic subdural hematoma, independent of the fibrinolytic system.

Inflammation and angiogenesis are critical processes contributing to the progression of chronic subdural hematoma (CSDH). Tissue plasminogen activator (tPA), which is highly abundant in CSDH effusion, has been implicated not only in hematoma liquefaction but also in progressive hematoma enlargement. Notably, tPA expression has also been reported in intracranial neoplasms, where it contributes to the development of peritumoral edema, suggesting roles beyond fibrinolysis. We therefore hypothesized that tPA exerts non-fibrinolytic effects in the pathophysiology of CSDH. Twenty CSDH fluid specimen

Pathological contrast enhancement of the oculomotor and trigeminal nerves caused by intracranial hypotension syndrome.

The typical symptom of intracranial hypotension syndrome is orthostatic headache. The headache may also be accompanied by neck pain and stiffness, low backache, radicular symptoms, quadriplegia, interscapular pain, nausea/vomiting, and cranial nerve involvement symptoms (hearing and visual problems, face pain and numbness, hypogeusia). Radiologically, on cranial magnetic resonance imaging, intracranial hypotension syndrome is characterized by dural thickening and contrast enhancement, subdural effusion, engorgement of the venous structures, sagging or downward displacement of the brain, and pi

Giant calvarial metastasis from secondary Ewing sarcoma with chronic subdural hygroma in a childhood hematological cancer survivor - An unusual case report.

We report an unusual case of a 9-year-old boy with Hodgkin lymphoma post chemoradiotherapy, who later developed Ewing sarcoma involving the eighth rib, which had metastasised to the cranial vault with meningeal and subdural involvement. Presenting with seizures, headache and hemiparesis, he underwent craniotomy and gross total tumour resection, with histopathology confirming metastatic Ewing sarcoma. Despite initial improvement, the patient presented with further cranial metastasis on follow-up. This case contributes to the limited literature on Ewing sarcoma metastasis to the CNS in patients

Cerebro-venous hypertension: a frequent cause of so-called "external hydrocephalus" in infants.

INTRODUCTION: External hydrocephalus (eHC) is commonly defined as a subtype of infant "hydrocephalus" consisting of macrocepahly associated with enlarged subarachnoid space and no or mild ventriculomegaly. This status is thought to be related to impaired CSF absorption because of arachnoid villi immaturity. However, other factors like the venous system might be involved in the development of the clinical picture. METHODS: All patients diagnosed with eHC received prospectively contrast-enhanced 3D MR phlebography. Venous sis abnormalities were graded depending on the number of affected sinus s

Update on intracranial hypertension and hypotension.

PURPOSE OF REVIEW: The aim of this article is to review recent findings on the pathophysiology of idiopathic changes in intracranial pressure. The review will focus on idiopathic intracranial hypertension (IIH) and spontaneous intracranial hypotension (SIH). RECENT FINDINGS: Substantial evidence indicates that IIH is associated with delayed absorption of cerebrospinal fluid (CSF). Stenoses of the transverse sinus are common in IIH, but their clinical significance has not been entirely clarified. Despite the observed efficacy of endovascular treatment in some IIH patients, a correlation betwee

Intracranial hypotension caused by cervical cerebrospinal fluid leak: treatment with epidural blood patch.

UNLABELLED: This report describes treatment with cervical epidural blood patch of low cerebrospinal fluid (CSF) pressure headache resulting from spontaneous CSF leak via a tear in a cervical dural cuff. The leak was diagnosed by a dynamic computed tomography (CT)-myelography study followed by gadolinium enhanced magnetic resonance imaging(MRI)-scan. The epidural needle was inserted with the aid of image intensifier and CT-scan to guide the needle to the precise site of the CSF leak. Blood mixed with gadolinium was injected, and subsequent MRI scanning provided the first description of spread o

Cerebrospinal fluid leak treated by aspiration and epidural blood patch under computed tomography guidance.

BACKGROUND AND OBJECTIVES: Cerebrospinal fluid (CSF) leakage secondary to surgery of the spine is usually treated by drainage of CSF through a subarachnoid catheter or surgical repair of the dural tear. We present 2 cases in which the pseudomeningocele was treated by aspiration of the leaked CSF and blood patch under computed tomography (CT) guidance. CASE REPORT: Two patients had headache after spine surgery. Physical examination showed a bulging accumulation of fluid at the laminectomy site. Aspiration of the fluid followed by injection of the patients' blood was performed aseptically under

[Common complaint, forgotten diagnosis: spontaneous intracranial hypotension syndrome].

Violation of the balance between the production and absorption rate of the cerebrospinal fluid (CSF) might result in hydrocephalus or the spontaneous intracranial hypotension syndrome (SIH), which is usually presented with a unique type of headache. Several hypotheses have been suggested regarding the pathogenesis of this syndrome, including spinal CSF leakage due to located damage to the spinal dura. Another theory suggests increased CSF absorption from the spinal epidural space caused by declined inferior vena cava pressure. SIH is characterized by typical radiological findings, including th

Neurological Complications in Young Infants With Acute Bacterial Meningitis.

We aimed to evaluate the occurrence, treatment, and outcomes of neurological complications after bacterial meningitis in young infants. A case series study from a retrospective cohort from two tertiary-level medical centers in Taiwan between 2007 and 2016 was conducted. Eighty-five young infants aged < 90 days with bacterial meningitis were identified. 25 (29.4%) were born at preterm. Group B Streptococcus (GBS) and Escherichia coli caused 74.1% of identified cases. Despite the majority (90.6%) initially received microbiologically appropriate antibiotics, 65 (76.5%) had experienced at least on

A rare complication of autologous cranioplasty: symptomatic epidural fluid collection causing mass effect requiring removal of bone flap.

INTRODUCTION AND IMPORTANCE: Cranial reconstruction after decompressive craniectomy is done usually at 20-25 per million people per year. The extra axial fluid collection is around 6% for subdural effusion and 6.1% for CSF leak and fistula. However, in Nepal, the overall complication of cranioplasty was 9% with collection of 4.1% and no any bone removal was done for epidural collection. Here I will be presenting a case that has rare complication of symptomatic epidural fluid collection after cranioplasty requiring removal of bone-flap. CASE SUMMARY: 45 years male underwent right Frontotemporo

Ultrasonographic study of children suspected of hydrocephalus at the Queen Elizabeth Central Hospital in Blantyre, Malawi.

Eighty Malawian children aged below 15 months who presented with rapidly enlarging head and were suspected of having hydrocephalus were investigated with anterior fontanelle ultrasonography at the Queen Elizabeth Central Hospital (QECH), Blantyre. Of the 68 (85%) who were found to have hydrocephalus, 35 (51.5%) were post infective, 14 (20.6%) were associated with lumbosacral myelomeningocele, 13(19.1%) were post haemorrhagic and the rest probably congenital or idiopathic. The remaining 12 patients with macrocephaly but without hydrocephalus comprised six children with hydranencephaly, five wit

Posthaemorrhagic hydrocephalus. Diagnosis, differential diagnosis, treatment, and long-term results.

The clinical features, investigations, and results of treatment are described in a series of 47 infants with posthaemorrhagic hydrocephalus. 7 were unfit for treatment; 3 had medical (isosorbide) treatment alone, 2 of whom made a good recovery; the remaining 37 were surgically treated. 32 (68%) survived for 18 months to 16 years, but 12 of them are severely affected mentally and/or physically. The results can be considered satisfactory in 19 (40%), though there are only 9 (19%) without detectable sequelae. The most serious adverse prognostic features on admission were acute illness with active

Severe fever with thrombocytopenia syndrome-associated encephalitis and intracranial hypotension: A case report.

Severe fever with thrombocytopenia syndrome (SFTS) is a natural infectious disease caused by severe fever with thrombocytopenia syndrome virus (SFTSV). Central nervous system (CNS) involvement is not rare in SFTS. In this work, we report a case of SFTS with CNS symptom onset and the metagenomic next-generation sequencing of cerebrospinal fluid (CSF), confirming invasion of SFTSV into the CNS. The case's lumbar puncture indicated an exceptionally low CSF opening pressure-an uncommon finding in other CNS infections and in case reports of SFTS. A spontaneous subdural effusion was observed on the

A case report of cardiac tamponade after a road accident: think beyond trauma.

BACKGROUND: Cardiac tamponade is a life-threatening compression of the heart caused by the accumulation of fluid in the pericardial sac. Although central venous catheters (CVCs) are essential in modern medicine, they carry a certain risk of complications including cardiac tamponade. CASE SUMMARY: A 12-year-old female was involved in a road accident reporting multiple severe traumatic injuries, including a left humerus fracture and subdural haemorrhage. After 2 days in the intensive care unit, she suddenly developed hypotension and cardiac tamponade was diagnosed. Analysis of the pericardial f

Cerebral Salt-wasting Syndrome in a Critically Ill Patient: An Easily Neglected Syndrome in Intensive Care Unit (ICU).

BACKGROUND: Cerebral salt-wasting syndrome (CSWS), which usually secondary to cerebral diseases, is characterized by hyponatremia and hypovolemia. In clinical practice, it is quite difficult to distinguish CSWS from other hyponatremia syndrome, especially in Intensive Care Unit (ICU) where the conditions of patients are more complicated. Nonetheless, it is crucial because treatments might be fundamentally different. CASE PRESENTATION: We discuss a case of patient who presented with refractory hyponatremia and hypovolemia after traumatic brain injury, finally was diagnosed with CSWS, and succe
