Role of surgeon-performed ultrasound on further management of patients with acute abdominal pain: a randomised controlled clinical trial.

BACKGROUND: The use of bedside ultrasound performed by the surgeon or emergency physician on duty at the emergency department is increasing. A study was performed which is the first randomised study to evaluate the benefits of surgeon-performed ultrasound for the management of the acute abdomen. METHODS: 800 patients who attended the emergency department at Stockholm South General Hospital, Sweden for abdominal pain were randomised either to receive or not receive surgeon-performed ultrasound as a complement to routine management. Outcome measures included the number of complementary investig

Impact of lockdown for SARS-CoV-2 (COVID-19) on surgical site infection rates: a monocentric observational cohort study.

Surgical site infections are the most common in-hospital acquired infections. The aim of this study and the primary endpoint is to evaluate how the measures to reduce the SARS-CoV-2 spreading affected the superficial and deep SSI rate. A total of 541 patients were included. Of those, 198 from March to April 2018, 220 from March till April 2019 and 123 in the COVID-19 era from March to April 2020. The primary endpoint occurred in 39 over 541 patients. In COVID-19 era, we reported a lower rate of global SSIs (3.3% vs. 8.4%; p 0.035), few patients developed a superficial SSIs (0.8% vs. 3.4%; p 0.

[Patients with diagnosed colonic diverticulitis admitted to a department of surgery].

In 1995-96, 148 patients with the diagnosis diverticulitis of the colon were admitted a total of 176 times to the Department of Gastroenterologic Surgery at Ullevaal Hospital in Oslo. Data on treatment and outcome were recorded retrospectively from patient files. 92% of the hospitalisations were emergency cases of abdominal pain. In 113 hospitalisations, patients were initially treated with intravenous antibiotics, aspiration of the stomach, later oral antibiotics. No deaths were recorded. In 8 out of 64 patients (13%) hospitalised with diverticulitis for the first time, the diagnosis could no

Mortality related to drug-resistant organisms in surgical sepsis-3: an 8-year time trend study using sequential organ failure assessment scores.

The difference in sequential organ failure assessment (SOFA) scores from the baseline to sepsis is a known predictor of sepsis-3 outcome, but the prognostic value of drug-resistant organisms for mortality is unexplained. We employed sepsis stewardship and herein report an observational study. Study subjects were patients admitted to the Departments of Surgery/Chest Surgery from 2011 through 2018 with a diagnosis of sepsis and a SOFA score of 2 or more. Our sepsis stewardship methods included antimicrobial and diagnostic stewardship and infection control. We determined the primary endpoint as i

Multi-disciplinary team approach in a teaching hospital: Challenges and opportunities.

Conducting regular Multidisciplinary Team (MDT) meetings in a tertiary care unit without an oncology department, has posed significant challenges. In addition to that, the dearth of advanced technological applications poses a greater strain on the scarce resources. The Department of Surgery at the Services Hospital, Lahore, has been performing oncological surgeries for some time, but since there was no oncology department, it lacked the MDT approach, which is an essential component in the management of onco surgeries. Realising the importance of MDT meetings, the Department of Surgery has been

Evaluation of point-of-care activated partial thromboplastin time testing by comparison to laboratory-based assay for control of intravenous heparin.

INTRODUCTION: Patients on intravenous heparin require regular activated partial thromboplastin time monitoring. Laboratory-based activated partial thromboplastin time assays necessitate a delay between blood sampling and dose adjustment. Point-of-care testing could permit immediate dose adjustments, potentially enabling tighter control of anticoagulation. AIM: To assess equivalence of activated partial thromboplastin time measured by conventional laboratory assay and by a novel proprietary point-of-care testing system (Hemochron Response, ITC, Thoratec Corporation, Edison, NJ) among surgical

Antibiotic prophylaxis in biliary tract surgery--current practice in The Netherlands.

A questionnaire about the current practice of antibiotic prophylaxis in biliary tract surgery was sent to the chairmen of the departments of surgery of 175 hospitals in The Netherlands. The Dutch hospitals are classified into three categories depending on the type of residency in general surgery. Overall, 80 per cent replied. Antibiotic prophylaxis in elective cholecystectomy is given in 76 per cent of the clinics (100/132), and single-dose prophylaxis is employed in 28 per cent of the clinics (28/100). In patients with acute cholecystitis, emergency surgery is the treatment of choice in 108 h

Update of the Adult and Pediatric Liver Transplant Program at the University of Michigan.

Significant technical innovations and improvements in immunosuppression have been introduced into our liver transplant program since its inception in 1985. The indications for transplantation have been extended to younger and older patients, and simultaneously more patients with comorbidities have been accepted for transplant. The net impact of these changes has been a continuing trend toward improved survival. Overall, patients with hepatitis B or malignancy have had poor survival rates. The introduction of prophylactic anti-hepatitis B immunoglobulin and lamivudine, and better selection of p

[Medial femoral neck fractures: possible reasons for delayed surgery. Part 2: Results of data from external inpatient quality assurance within the framework of secondary data evaluation].

PROBLEM: In Germany a disproportionate number of patients with a femoral neck fracture still experience a delay in surgery. In 2008 delays of more than 48 h after admission occurred for 15.4 % of patients. This leads to increases in surgical and general complications as well as pressure sores. OBJECTIVE: Possible reasons that lead to delayed operations were investigated in a nationwide study. DATA AND METHODS: Using nationwide data from the German inpatient external quality assurance program from the year 2008 the reasons for performing operations later than 48 h after hospital admission wer
