Use of Transfer Learning for the Automated Segmentation and Detection of Swallows via Digital Cervical Auscultation in Children.

Digital cervical auscultation (CA) has high diagnostic test accuracy in the detection of aspiration in children. However, the clinical application of digital CA is limited because swallow sound recordings require manual segmentation by trained experts, which is time consuming and not feasible in clinical practice. The automated detection of swallowing sounds in adults from sound recordings have reported accuracies between 76 and 95%. No equivalent literature exists for the automated detection of swallowing sounds in children. This study aimed to establish whether automated machine learning usi

Multiple rapid swallows and rapid drink challenge in patients with esophagogastric junction outflow obstruction on high-resolution manometry.

BACKGROUND: Esophagogastric junction outflow obstruction (EGJOO) as defined by Chicago Classification of esophageal motility disorders (CCv3.0) encompasses a broad range of diagnoses, thus posing clinical challenges. Our aims were to evaluate multiple rapid swallow (MRS) and rapid drink challenge (RDC) during high-resolution manometry (HRM) to aid identifying clinically relevant EGJOO. METHODS: Patients with a HRM diagnosis of EGJOO based on CCv3.0 that also completed MRS and RDC during HRM and barium esophagram were retrospectively identified. Radiographic EGJOO (RAD-EGJOO) was defined by ei

Optimal number of multiple rapid swallows needed during high-resolution esophageal manometry for accurate prediction of contraction reserve.

BACKGROUND: Multiple rapid swallows (MRS) is a provocative test for assessment of contraction reserve, however reproducibility on repetitive MRS is incompletely understood. Our aim was to determine the optimal number of MRS sequences for consistent assessment of contraction reserve. METHODS: One hundred and fifty-nine consecutive patients (79 IEM and 80 normal motility) who underwent high-resolution manometers were enrolled. Ten single swallows (SS) and 10 MRS were performed. Gold standard for evaluation of the contraction reserve was the ratio between the mean DCI of 10 MRS and the mean DCI

Vigor of peristalsis during multiple rapid swallows is inversely correlated with acid exposure time in patients with NERD.

BACKGROUND: Multiple rapid swallowing (MRS) during high-resolution manometry (HRM) is increasingly utilized as provocative test to assess esophageal peristaltic reserve. The aim of this study was to evaluate the correlation between MRS response and impedance and pH (MII-pH) parameters in endoscopy negative heartburn (ENH) patients. METHODS: We enrolled consecutive ENH patients, who underwent HRM and MII-pH study, with a selected MII-pH profile: abnormal MII-pH (pH+/MII+); normal MII-pH (pH-/MII-). HRM was performed with 10 wet swallows (WS) and one MRS. Mean distal contractile integral (DCI)

Conjunctivitis, rhinitis, and sinusitis in cliff swallows (Petrochelidon pyrrhonota) found in association with Mycoplasma sturni infection and cryptosporidiosis.

Fledgling cliff swallows were cared for at a rehabilitation facility when clinical signs of ocular disease, characterized by conjunctivitis, epiphora, and hyperaemia of palpebrae and nictitans, were recognized. Treatment consisted of topical and oral antibiotic therapy and one topical steroid administration. However, one cliff swallow died and three were killed due to poor therapeutic response. Conjunctival swabs were obtained ante-mortem from the three cliff swallows and were submitted for mycoplasma culture and molecular diagnostics. Heads of the three birds were fixed in 10% neutral buffere

Differences in response of the proximal esophagus to wet swallows in patients of Chagas' disease and idiopathic achalasia.

Chagas' disease and idiopathic achalasia have similar esophageal manifestations such as absent or incomplete lower esophageal sphincter relaxation and aperistalsis in the esophageal body (alterations seen mainly in the distal esophageal body). Our aim in this paper was to study the response of the proximal esophageal body to wet swallows in patients with Chagas' disease and patients with idiopathic achalasia. We retrospectively analyzed the time interval between the onset of the pharyngeal contractions 1 cm proximal to the upper esophageal sphincter, as well as 5 cm distal to the pharyngeal me

Application of early swallowing function training combined with ice stimulation intervention in patients with stroke swallowing disorders.

Stroke incidence has risen in recent years, with swallowing disorders affecting over half of patients and leading to complications such as aspiration pneumonia and malnutrition. Swallowing function training and ice stimulation have shown promise individually. This study evaluates their combined effect on stroke-related dysphagia. This study aimed to investigate the clinical value of early swallowing function training combined with ice stimulation intervention in stroke patients with swallowing disorders. From March 2019 to March 2023, 92 patients were randomly assigned to either a control grou

New Swallowing Method to Improve Pharyngeal Passage of a Bolus by Creating Negative Pressure in the Esophagus-Vacuum Swallowing.

Herein, we present a case of a patient with Wallenberg syndrome with severe bulbar dysphagia who discovered a unique swallowing method: creating strong negative pressure in the esophagus to improve pharyngeal passage of a bolus. A 47-yr-old man presented with a subarachnoid hemorrhage secondary to a ruptured aneurysm in the right vertebral artery. After coil embolization, he experienced severe dysphagia due to Wallenberg syndrome and required tube feeding. Eighty-one days after the onset of the stroke, a videofluoroscopic swallowing evaluation revealed that the bolus was rapidly sucked into th

Evaluation of Swallowing in Poststroke Individuals Using the Swallowing Proficiency for Eating and Drinking (SPEAD) Protocol: A Pilot Study.

BACKGROUND AND OBJECTIVES: Poststroke dysphagia affects 37-78% of stroke survivors, leading to complications such as malnutrition, dehydration, and psychological distress. Current tools for assessing swallowing, such as the Timed Water Swallow Test and Test of Mastication and Swallowing Solids, do not evaluate intermediate consistencies. The Swallowing Proficiency for Eating and Drinking (SPEAD) protocol incorporates thin liquids, solids, and moderately thick liquids, providing a comprehensive evaluation of swallowing function. This study assessed SPEAD's utility in a stroke population and its

Validation of the Swedish Dynamic Imaging Grade of Swallowing Toxicity for Flexible Endoscopic Evaluation of Swallowing (DIGEST-FEES).

In the head and neck cancer (HNC) population around 45% suffer from chronic swallowing difficulties after cancer treatment. Previously a measure for flexible endoscopic evaluation of swallowing (FEES) where swallowing efficiency, safety and overall ability is evaluated within the same framework has been lacking. The Dynamic Imaging Grade of Swallowing Toxicity for FEES (DIGEST-FEES) was developed in 2021 and provides such a measure for patients with HNC. The aim of this study was to translate and validate the DIGEST-FEES into Swedish (Sw-DIGEST-FEES). A translation of the protocol to Swedish w
