Foot and ankle surgery : official journal of the European Society of Foot and Ankle Surgeons2017eng
Fractures of the talus are challenging to manage, with historically poor outcomes and a high rate of complications. The rare nature of this injury limits the number of studies available to guide treatment. Fortunately, a number of advancements have been made in the last decade. There is increased recognition regarding the importance of anatomic reconstruction of the osseous injury. Advanced imaging is used to assess the subtalar joint, where even slight displacement may predispose to arthritis. Increasing use of dual anteromedial and anterolateral approaches, along with plate fixation, has imp

Tumors rarely metastasize to the foot bones; however, a misdiagnosis can lead to a poor outcome. These metastatic tumors can cause foot pain and other symptoms in patients who are asymptomatic for the primary tumor. To accurately diagnose acrometastases, the attending physician must have a high index of suspicion and conduct a thorough examination and appropriate diagnostic testing. We present a rare case of metastatic pulmonary adenocarcinoma of the talus, which was initially misdiagnosed as a benign bone cyst. After pulmonary adenocarcinoma was diagnosed, the patient received multidrug treat

PURPOSE OF THE STUDY Peripheral fractures of the talus (lateral talar process and posterior talar process) are rare injuries, easy to miss on examination due to their location and clinical manifestation. They account for 0.3% to 1.0% of all fractures of the talus. An incorrect or late treatment of talar process fractures can result in permanent pain, impingement syndrome, healing in malposition, pseudoarthrosis development and also, due to joint instability, in a potential development of severe subtalar arthritis. The aim of this retrospective study was to evaluate the results of surgical mana

The purpose of the research was to study the results of surgical treatment for rare and severe injury, such as the total talus dislocation and determine the causes of complications. The study was based on findings from dynamic observations of 15 patients with this type of musculoskeletal pathology, who underwent surgical treatment in three multi-profile medical facilities of Kutaisi during 2005-2013 years. Statistical results were as follows: men - 12, women -3. Patients age range 29-51 years. Trauma was mainly caused by motor vehicle accidents and falls. Three patients had a sports injury. Th

Tuberculosis osteomyelitis of foot bones is uncommon and may evade the diagnosis for long time because of atypical presentation, paucibacillary nature and relatively normal picture on laboratory investigations. As tuberculosis of bone mimic like chronic osteomyelitis, clinical picture with confection by other organism lead to misdiagnosis. A case of tubercular osteomyelitis of talus reported here for its rarity and atypical presentation. Clinical suspicion and histological confirmation of diagnosis is recommended avoiding undue delay and for early chemotherapy.

A 50-year-old male patient sustained a supination-adduction type ankle fracture with an associated sagittal split fracture of the talus. The patient was treated initially in a short leg splint, and upon presentation to an orthopaedic surgeon, an external fixator was applied. After the soft tissue swelling improved enough to permit open reduction and internal fixation, the patient was brought back to the operating room for definitive treatment with removal of the external fixator and open reduction and placement of internal fixation. The patient's postoperative course was uncomplicated. At the

Acute bone bruises of the talus after ankle injury need to be managed differently from osteochondral defects. Bone bruises have a benign course, but there may be persistent edema. A bone bruise should not delay rehabilitation unless symptoms persist or significant edema is close to the subchondral plate. Osteochondral defects have a less predictable prognosis, and rehabilitation should aim at promoting healing of the subchondral fracture. A period of nonweight bearing reduces the cyclical pressure load through the fissure and promotes healing. Surgery should be reserved for chronic symptomatic

It has become clear that contrary to the previously held belief that osteochondral lesions of the talus occurred anterolaterally and posteromedially, there is a much wider spread across the talus. Lesions can now be mapped by their biology and geography, and from this the ideal procedure and the ideal surgical approach can be selected. Familiarization with a range of skills, such as posterior ankle arthroscopy and malleolar osteotomies, are required to be able to perform this surgical plan.

PURPOSE OF THE STUDY: To present a retrospective evaluation of the results of our method of open reduction and internal fixation.
MATERIAL AND METHODS: A total of 35 patients were surgically treated for talar fractures at our department between 2004 and 2008. There were 27 men and eight women, with an average age of 31 years (range, 21 to 65). Talar neck fractures were recorded in 21 and talar body fractures in 14 patients. The most frequent cause of injury was a fall from height (77%); motorcar accidents were less frequent (14%). Open fractures were found in 8.5% of the patients, and talar f

OBJECTIVE: To explore the operative methods of malunited or nonunited talus fractures.
METHODS: Twenty-two patients of malunions or nonunions after displaced talar fractures were treated from January 2000 to January 2008. There were 17 males and 5 females with an average age of 34 years (ranged from 15 to 52 years). According to classification of posttraumatic talar deformities (Zwipp 2003), there were 10 cases of type I (malunion and/or joint displacement), 8 cases of type II (nonunion with joint displacement), 4 cases of type III (type I/II with partial AVN). The surgical treatments include

Fifty-seven year old patient with open medial dislocation of the left talus, with suprasyndesmotic fracture of the fibula, due to indirect traumatism. The following were carried out: washing, cleaning the talus and the soft parts, Friedrich, reduction, fixing with Kirschner needles, suture of the deltoid ligament, and osteosynthesis of the fibula. There were neither surface nor deep post-operational infections. The patient underwent six weeks of immobilisation and spent three months on discharge. Two years later there were no radiological signs of avascular necrosis. In the osseous gammagraphy

Total dislocation of the talonavicular joint is rare. Detection of a major tarsal bone extrusion is simple. However, its management and treatment are not. We present a case of a total extrusion of the talus and navicular of an open dislocation and the reconstruction options that subsequently followed. In spite of a well-perfused foot, our patient decided to undergo a below the knee amputation after an infection.

Total extrusion of the talus is an unusual injury, and the obvious risks of reimplantation of the extruded bone include infection and avascular necrosis. In this article, the authors present the case of a 34-year-old man who sustained an open ankle injury with complete extrusion of the talus. The talus was recovered at the scene of the accident, and subsequently reimplanted along with ankle stabilization with pins and an external fixator. At 6 weeks following the osseous surgery, final soft tissue reconstruction with a suralis flap was performed. At 3 years after the injury, radiographs reveal

OBJECTIVE: To determine the prognostic reliability, sensitivity, and specificity of the Hawkins sign. The Hawkins sign is a subchondral radiolucent band in the talar dome that is indicative of viability at 6 to 8 weeks after a talus fracture. It is visible in the anterior-posterior view, but seldom appears on lateral radiographs.
DESIGN: Retrospective study.
SETTING: University hospital.
PATIENTS AND METHODS: Between January 1995 and December 2000, a total of 41 patients (13 female, 28 male) with displaced talar fractures were operated on in our hospital. Thirty-four patients with a mean ag

Talar head and body injuries are not recognized easily and can create significant long-term disability when missed. Careful investigation of any injury about the ankle requires clinical and radiographic examination. A CT scan is extremely helpful in diagnosing and treating these injuries. Displaced fractures require open reduction of the major joint surfaces and internal fixation. Prolonged nonweight bearing and immobilization is the norm. Despite aggressive management, complications that involve avascular necrosis and posttraumatic arthritis to the subtalar and ankle joints occur frequently.

AVN of the talus is a challenging disease process with respect to patho-physiology and treatment. We believe that our algorithm is a legitimate approach to aid the orthopedic surgeon in initiating a promising treatment. It is divided into different levels and allows to change between some. This is not the only way to proceed but it seems promising, especially if the long-term results with the vascularized bone grafts show revascularization of the talus. As always in medicine, the treatment needs to be individualized. Arthrodesis always should be the last option and is a challenging procedure.

This article addresses very large osteochondral lesions of the talus with an osteonecrosis overlap. Six cases are discussed where lesions were deemed too large for osteochondral autograft transfer (OATS) or autologous chondrocyte transplant (ACT). These cases were treated with bulk fresh or fresh frozen matched talus allograft transplantation. The associated literature is reviewed.

Diagnosis of OLTs requires a high index of suspicion because these lesions are rare and the symptoms can be falsely attributed to acute or chronic ankle sprains. When no abnormality is present on plain radiographs, a bone scan or MRI can reliably identify the presence of an OLT. CT scanning can provide even better detail of the location and size of the fragment and help stage these lesions and guide treatment. Arthroscopic staging is believed to be the best method to determine treatment. In a patient without an obvious loose body, initial nonoperative treatment is warranted. When nonoperative
