Serotonergic and dopaminergic modulation of attentional processes.

Disturbances in attentional processes are a common feature of several psychiatric disorders such as schizophrenia, attention deficit/hyperactivity disorder and Huntington's disease. The use of animal models has been useful in defining various candidate neural systems thus enabling us to translate basic laboratory science to the clinic and vice-versa. In this chapter, a comparative and integrated account is provided on the neuroanatomical and neurochemical modulation of basic behavioural operations such as selective attention, vigilance, set-shifting and executive control focusing on the compar

Cognitive Inflexibility Shares Across Schizophrenia and Obsessive-Compulsive Disorder: A Task-Based Functional MRI Study.

BACKGROUND AND HYPOTHESIS: Schizo-obsessive comorbidity (SOC), defined as obsessive-compulsive symptoms (OCS) in schizophrenia (SCZ), is linked to severe psychopathology and poor prognosis. Schizophrenia and obsessive-compulsive disorder (OCD) share cognitive impairments, particularly in inhibition and cognitive flexibility, which may underlie SOC. However, little is known regarding the underlying neural mechanisms of SOC. We aimed to directly compare the inhibition- and cognitive flexibility-related neural activations between patients with SOC, SCZ, OCD, and healthy controls (HCs). STUDY DES

Cognitive flexibility in juvenile anorexia nervosa patients before and after weight recovery.

Deficits in set-shifting abilities have been robustly described in adult patients with anorexia nervosa (AN). These deficits are associated with behavioral traits, such as rigidity and perfectionism, and are independent of starvation. However, little is known about neurocognitive deficits in juvenile patients with AN. The brain circuits that support set shifting are not fully mature in these patients. One possibility is that neuroendocrinological changes, such as elevated cortisol levels, contributing to alterations in cognitive performance in individuals with AN. Set-shifting abilities (Visua

Sound can improve visual search in developmental dyslexia.

We examined whether developmental dyslexic adults suffer from sluggish attentional shifting (SAS; Hari and Renvall in Trends Cogn Sci 5:525-532, 2001) by measuring their shifting of attention in a visual search task with dynamic cluttered displays (Van der Burg et al. in J Exp Psychol Human 34:1053-1065, 2008). Dyslexics were generally slower than normal readers in searching a horizontal or vertical target among oblique distracters. However, the addition of a click sound presented in synchrony with a color change of the target drastically improved their performance up to the level of the norma

Behavioral and ERP evidence for amodal sluggish attentional shifting in developmental dyslexia.

The goal of this study was to examine the claim that amodal deficits in attentional shifting may be the source of reading acquisition disorders in phonological developmental dyslexia (sluggish attentional shifting, SAS, theory, Hari & Renvall, 2001). We investigated automatic attentional shifting in the auditory and visual modalities in 13 dyslexic young adults with a phonological awareness deficit and 13 control participants, matched for cognitive abilities, using both behavioral and ERP measures. We tested automatic attentional shifting using a stream segregation task (perception of rapid su

A Pilot Study to Advance Task-Sharing of Gastroschisis Management in Uganda.

Introduction: Gastroschisis mortality in Africa is high partly due to delays in care. In Uganda, skilled birth attendants (SBAs) are the first point-of-contact for most babies, and with proper training, may be willing to participate in surgical task-sharing. Objective: Empower Ugandan skilled birth attendants with the knowledge and practical skills needed to care for babies with gastroschisis. Methods: Ugandan SBAs completed a one-day gastroschisis course, and resident physicians also requested to participate. A pre- and post-course test was administered to assess gastroschisis knowledge and c

Development of a neuropsychological assessment in smoking.

INTRODUCTION: Smoking is one of the causes leading to the development of disease and mortality worldwide. One of the focuses of interest in this area is the impact of smoking on neuropsychological health. However, few studies provide instruments to assess executive functioning in smokers. The purpose of this study was to examine the viability of the internal structure of a neuropsychological battery for the assessment of executive function in smokers. SUBJECTS AND METHODS: A total of 171 smokers (Mage= 47.44, SDage= 8.48) were assessed. Executive functions were assessed at baseline with measu

Manipulating the decision making process: Influencing a "gut" reaction.

The present series of studies sought to examine how external factors influence behavioral decision making task performance. Utilizing the Iowa Gambling Task (IGT) to assess risky decision making, we examined the influence of a dual task paradigm (Study 1, Study 2), shifting task focus to decision making speed versus accuracy (Study 3), and varied intertrial intervals (Study 4). College student participants completed the IGT and decision making speed and the patterns of IGT selections by deck in the earlier (decision making under ambiguity) and later (decision making under risk) trials were exa

Sustaining lifesaving care: Tactical infrastructural adaptations for clinics in low-resource settings during severe flooding.

We present a number of suggestions that will ensure continuity of medical care services at tropical clinics in situations where isolation is caused by frequent massive floods and lack of electricity. We demonstrate how the move from resource-limited triage will be achieved through the use of manual instruments, heat-stable drugs and community task-shifting in order to address trauma, emergency delivery and sepsis.

Rethinking category selectivity: insights from the macaque inferior temporal cortex.

Ritchie et al. argue that the traditional framework of category selectivity has limited value for understanding the organization of the ventral visual stream and propose shifting focus toward behavioral relevance and examining vision under naturalistic task conditions. While I agree with many of their points, I expand on their discussion of category selectivity, as well as the drivers of ventral stream organization, from a nonhuman primate perspective.

Reimagining the role of the nursing workforce in Uganda after more than a decade of ART scale-up.

BACKGROUND: The expanding roles and increasing importance of the nursing workforce in health services delivery in resource-limited settings is not adequately documented and sufficiently recognized in the current literature. Drawing upon the theme of 2020 as the International Year of the Nurse and the Midwife, we set out to describe how the role of nurses expanded tremendously in health facilities in Uganda during the era of anti-retroviral therapy (ART) scale-up that commenced in June 2004. METHODS: We employed a mixed-methods sequential explanatory research design. Phase I entailed a cross-s

Genital self-sampling for HPV-based cervical cancer screening: a qualitative study of preferences and barriers in rural Ethiopia.

BACKGROUND: In the context of WHO's "task shifting" project and growing global consensus on primary HPV-based cervical cancer screening, self-sampling is a promising new tool to expand screening access, uptake and coverage for women worldwide. We aimed to explore perceptions and acceptability of HPV self-sampling-based cervical cancer screening among community members and health professionals in rural northwest Ethiopia and to identify preferences and socio-cultural barriers regarding self-sampling in order to design a suitable high-coverage screening intervention for a rural African setting.

Safety of tubal ligation by minilaparotomy provided by clinical officers versus assistant medical officers: study protocol for a noninferiority randomized controlled trial in Tanzanian women.

BACKGROUND: Female sterilization by tubal ligation is a safe, extremely effective, and permanent way to limit childbearing. It is the most popular modern contraceptive method worldwide. The simplest way to provide tubal ligation is by a procedure called minilaparotomy, generally performed with the client under local anesthesia with systemic sedation and analgesia. In Tanzania, unmet need for family planning is high and has declined little in the past decade. Access to tubal ligation is limited throughout the country, in large part because of a lack of trained providers. Clinical officers (COs)

Role of the posterior parietal association cortex in the processing of spatial event information.

Humans and monkeys with damage to the parietal association cortex (PC) exhibit deficits on visual-spatial tasks that are often dependent upon the attentional requirements of the task at hand. In order to test whether there is a correspondence between humans and rats in terms of the function of PC in the processing of spatial information, separate groups of rats were trained in two list-learning tasks, a win-stay task and a win-shift task, on an eight-arm radial maze. It is assumed that these two tasks vary in degree of required attention effort, because the win-shift rule is considerably easie

The reality of neonatal nursing work in Kenya and implications for quality and safety: Direct observation of tasks and time utilisation.

BACKGROUND: Little is known about how nurses working in care settings affected by remarkably high workloads and workforce constraints manage their work and time to deliver needed care. OBJECTIVE: This study aimed to characterise nursing workflows and examine time allocated to care in high mortality, intermediate care Kenyan neonatal units. DESIGN AND METHODS: Using 'shadowing' as a time and motion technique, we directly observed 1-2 nurses per 12-hour shift in 8 Kenyan county hospitals. We used an Activity Log Sheet to document their activities during the shift and thematic content analysis

Western-style diet induces insulin insensitivity and hyperactivity in adolescent male rats.

The prevalence of obesity in children and adolescents has increased rapidly over the past 30 years, as has the incidence of attention deficit hyperactivity disorder (ADHD). In 2012, it was found that overweight children have a twofold higher chance of developing ADHD than their normal weight counterparts. Previous work has documented learning and memory impairments linked to consumption of an energy-dense diet in rats, but the relationship between diet and ADHD-like behaviors has yet to be explored using animal models. Therefore, the purpose of this study was to explore the role of diet in the

Verbal and nonverbal fluency in children with heavy prenatal alcohol exposure.

OBJECTIVE: Executive function deficits, including verbal fluency, have been documented in children with histories of prenatal alcohol exposure. Whereas nonverbal fluency impairments have been reported in adults with such exposure, these abilities have not been tested in children. Deficits in both verbal and nonverbal fluency were predicted and assessed in children and adolescents with histories of heavy prenatal alcohol exposure. METHOD: There was a total of 28 (54% female) subjects; children with heavy prenatal alcohol exposure with (n = 10) and without (n = 8) fetal alcohol syndrome (FAS) w

Functional overload and adaptive failure during dual-tasking in Ménière's disease.

BackgroundMénière's disease (MD) impairs cognitive function, yet the dynamics of cognitive-motor interference are not well understood. This study investigated dual-task performance in MD, hypothesizing a greater performance cost for patients compared to healthy controls under combined cognitive and postural loads.MethodTwenty-nine patients with MD and 29 healthy controls participated in a dual-task paradigm that combined computerized dynamic posturography (CDP) with a cognitive battery assessing visuospatial, executive, and working memory functions. Performance was measured across Sensory Orga

Neuropathic Pain Creates an Enduring Prefrontal Cortex Dysfunction Corrected by the Type II Diabetic Drug Metformin But Not by Gabapentin.

Chronic pain patients suffer from pain-related cognitive deficits, even when taking commonly prescribed analgesics. These deficits are likely related to pain-related maladaptive plasticity in the frontal cortex. We sought to model cognitive deficits in mice with neuropathic pain to examine maladaptive morphological plasticity in the mPFC and to assess the effects of several therapeutics. We used an attentional set-shifting task in mice with spared nerve injury (SNI) who received either a single intrathecal injection of an analgesic dose of clonidine, 7 d of 100 mg/kg gabapentin, or 7 d of 200
