The Relationship between the Frontal Branch of the Superficial Temporal Artery and the Temporal Hairline in Bicoronal Incisions: Cadaveric and Clinical Study.

INTRODUCTION: Classic bicoronal skin incisions for bifrontal craniotomy are usually performed near the course of the superficial temporal artery (STA). This frequently results in injuries to the frontal branch (fSTA) or even the STA's main trunk. We investigate the usual course of the fSTA and evaluate a previously proposed modification to bicoronal scalp incisions for its rate of STA preservation. MATERIALS AND METHODS: Sixteen sides of cadaveric heads were dissected. We investigated the location of the fSTA in relation to the temporal peak of the hairline. We also performed a retrospective

Interrupted Versus Running Sutures for Superficial Temporal Artery to Middle Cerebral Artery Cranial Bypass.

BACKGROUND AND OBJECTIVES: Superficial temporal artery to middle cerebral artery (STA-MCA) bypass is the workhorse for flow augmentation surgery. Although either interrupted or running sutures can be used to complete the anastomosis with high intraoperative patency rates, no previous study in the cranial bypass literature has compared long-term patency and maturity of end-to-side STA-MCA anastomoses. We compared STA-MCA anastomoses performed with running vs interrupted sutures by evaluating bypass flow and anastomotic maturation on follow-up vascular imaging. METHODS: Ninety-six STA-MCA anast

How I do it: superficial temporal artery to middle cerebral artery bypass for moyamoya disease.

BACKGROUND: Moyamoya disease (MMD) is a chronic progressive disease leading to recurrent stroke due to occlusion of the terminal internal carotid arteries. Surgical revascularization for symptomatic MMD is an effective treatment for preventing further stroke. However, direct revascularization of advanced MMD remains challenging due to small caliber of the recipient vessel and perforators. METHODS: We introduce our technique of superficial temporal artery (STA) to middle cerebral artery (MCA) bypass with figures and video to illustrate the procedure. CONCLUSION: Surgical nuances of STA to MCA

Frontotemporal Approach for Infectious Aneurysm Trapping and Superficial Temporal Artery-Middle Cerebral Artery Bypass.

Although neurotropic, the varicella-zoster virus (VZV) is a rare cause of mycotic cerebral aneurysms. As with other mycotic aneurysms, medical management can provide complete resolution. Surgery for refractory aneurysms can be complicated by vessel friability and complex morphologies requiring excision and revascularization. In Video 1, we present key steps in the surgical management of a previously ruptured and growing fusiform mycotic cerebral aneurysm. A 58-year-old woman with a history of neuromyelitis optica resulting in lower-extremity paraplegia and chronic immunosuppression presented e

Surgical anatomy of the superficial temporal artery to prevent facial nerve injury during arterial biopsy.

To investigate the topographical relationship between the frontal branch of the superficial temporal artery (FSTA) and the temporal branch of the facial nerve (TFN) with the aim of preventing nerve injury during FSTA biopsy. Fifty-seven hemifaces of 33 cadavers were dissected. Vertical lines drawn to the lateral orbital margin (LOM) and the superior root of the helix were used as the anterior and posterior reference positions, respectively. Horizontal lines drawn through the supraorbital margin and lateral canthus were used as the superior and inferior reference points, respectively. The depth

Anterior temporal artery to posterior cerebral artery bypass for revascularization of the posterior circulation: An anatomical study.

We describe a novel intracranial-to-intracranial bypass technique between the anterior temporal artery and the posterior cerebral artery for revascularization of the posterior circulation. Four formalin-fixed human heads were examined to demonstrate the detailed anatomy of the middle cerebral artery and the posterior cerebral artery, and to illustrate the step-by-step bypass procedure. The anterior temporal artery, a branch of the middle cerebral artery, can be anastomosed to the P2 segment of the posterior cerebral artery as an alternative to extracranial bypass donor segments for treatment o

Percutaneous superficial temporal artery access for carotid artery stenting in patients with a hostile aortic arch.

PURPOSE: To describe an entirely percutaneous treatment for carotid artery stenting in a type IIa aortic arch via the superficial temporal artery (STA) for through-and-through guidewire access. TECHNIQUE: The technique is demonstrated in an 83-year-old man status post left carotid endarterectomy 3 months prior who presented with 2.5 hours of confusion and dysphasia following diagnostic carotid arteriography. The patient was known to have a type IIa bovine aortic arch and a severe 70% stenosis of the left internal carotid artery. The patient was referred for carotid artery stenting by his vasc

New pull-through technique using the superficial temporal artery for transbrachial carotid artery stenting.

Carotid artery stenting (CAS) is an effective and less invasive alternative to carotid endarterectomy for internal carotid artery (ICA) stenosis, but the guiding catheter is often technically difficult to introduce into the common carotid artery (CCA) in patients with concomitant atherosclerotic disease in the peripheral vessels or aorta. A new pull-through technique between the superficial temporal artery (STA) and the brachial artery was used to deliver the guiding catheter into the CCA safely and steadily. An 83-year-old male presented with repeated transient left hemiparesis caused by seve

Superficial temporal artery to superior cerebellar artery anastomosis via the presigmoid retrolabyrinthine transtentorial approach.

BACKGROUND: The conventional method for performing superficial temporal artery to superior cerebellar artery (STA-SCA) anastomosis is applied to ischemic lesions in the posterior circulation via the temporobasal approach. However, this approach can be difficult and presents limitations due to the deep and narrow surgical field. METHODS AND RESULTS: STA-SCA anastomosis using the presigmoid retrolabyrinthine transtentorial approach was performed, in which a large skin incision was required for the isolation of the STA, compared to that associated with the petrosal approach as described in previ

Superficial Temporal Artery to Middle Cerebral Artery (STA-MCA) bypass: How I do it.

The superficial temporal artery-middle cerebral artery (STA-MCA) bypass remains a key extracranial-intracranial revascularization technique for moyamoya disease, complex aneurysms, and symptomatic steno-occlusive pathology not amenable to endovascular treatment. Effective bypass requires detailed knowledge of STA and MCA anatomy, comprehensive preoperative imaging, and meticulous microsurgical planning. We describe a stepwise operative strategy including donor and recipient vessel preparation, microanastomosis, and intraoperative patency assessment. Technical nuances, complication avoidance, a

Replaced posterior cerebral artery with early branching temporal artery diagnosed by magnetic resonance angiography.

PURPOSE: To describe a case of replaced posterior cerebral artery (PCA) in which all branches of the PCA arose from the anterior choroidal artery (AChA) with an early branching temporal artery. METHODS: An 83-year-old man with cerebral infarctions underwent cranial magnetic resonance (MR) imaging and MR angiography using a 3-Tesla scanner. MR angiography was performed using a standard 3-dimensional time-of-flight technique. RESULTS: A large anomalous artery arose from the supraclinoid segment of the right internal carotid artery (ICA) and supplied all branches of the right PCA, mimicking fet

Anatomic Variation of the Superficial Temporal Artery and Posterior Auricular Artery in a Pediatric Moyamoya Disease Population.

BACKGROUND AND PURPOSE: In certain cases of pediatric patients with Moyamoya disease undergoing encephaloduroarteriosynangiosis (EDAS) treatment, the posterior auricular artery can be used as an alternative when the parietal branch of the superficial temporal artery is unavailable. In this study, anatomic variations of the superficial temporal and posterior auricular arteries in pediatric patients with Moyamoya disease and postoperative outcomes of posterior auricular artery-EDAS are explored. MATERIALS AND METHODS: Medical records of 572 patients with Moyamoya disease who underwent surgical

Revascularization of the anterior cerebral artery using a free superficial temporal artery graft: a case report.

BACKGROUND: Various bypass procedures have been used to treat occlusive cerebrovascular diseases. Most procedures were performed to prevent further ischemia in the territory of the middle cerebral artery, while only a few of the papers discuss the anterior cerebral artery (ACA) territory. METHOD AND RESULTS: A patient who initially presented with several episodes of transient monoparesis in the left leg was found to have severe stenosis of the right A2-A3 junction. A free superficial temporal artery (STA) graft was anastomosed between the right A2 and A3 portion intracranially. Transient isch

Temporal artery biopsy and temporal artery ultrasound inter-rater agreement for the diagnosis of giant cell arteritis: an ancillary analysis from the multicentre prospective ECHORTON study.

Temporal artery biopsy (TAB) and, more recently, temporal artery ultrasound are recommended for the diagnosis of giant cell arteritis (GCA). The inter-rater agreement for TAB is poorly reported, and agreement for ultrasound is variable. A prospective study, the ECHORTON study, evaluated a diagnostic strategy for GCA that used temporal artery ultrasound as the first-line diagnostic test and TAB for ultrasound-negative patients. Clinical expertise served as the reference method. We propose assessing the inter-rater agreement in interpreting TAB and ultrasound images collected in this study. From
