Achilles and patellar tendinopathies in athletes: pathogenesis and surgical treatment.

In the period from 1985 to 1992, 136 athletes underwent 148 surgical exploration of Achilles (98 operations in 90 patients) and patellar (50 operations in 46 patients) tendons for persistent pain. The morphology and the biochemistry of tendon samples taken at operation were investigated. A marked increase of glycosaminoglycans, especially chondroitin sulphate, was present. The complete alleviation of pain and the return to full activity were obtained in 79% of the athletes, while the remaining 21% were not able to return to their sports activities. Generally, successful outcome after surgery w

Efficacy of memantine prodrug microemulsion in a Preclinical model of tendinopathy.

Tendinopathy is a complex musculoskeletal pathology characterized by pain and dysfunction in the tendon. Tendinopathy management includes pharmacological treatments, which, however, have numerous side effects and low efficacy. New pharmacological approaches are essential to overcome these drawbacks. This study aimed to test the activity of the newly synthesized memantine-prodrug Memit against chronic tendon pain, which releases H2S and memantine at the therapeutic site, formulated in a microemulsion for peri-tendon administration. Memit (1 % w/w) was successfully formulated in a microemulsion

Upregulation of FABP4 induced inflammation in the pathogenesis of chronic tendinopathy.

OBJECTIVES: Excessive inflammation contributes to the pathogenesis of tendinopathy. Fatty acid binding protein 4 (FABP4) is a pro-inflammatory adipokine mediating various metabolic and inflammatory diseases. This study aimed to examine the expression of FABP4 and its association with the expressions of inflammatory cytokines in tendinopathy. The effects of a single injection of FABP4 on tendon pathology and inflammation were examined. The effect of FABP4 on the expressions of inflammatory cytokines and the effect of IL-1β on the expression of FABP4 in tendon-derived stem/progenitor cells (TDSC

Tendinopathies and Pain Sensitisation: A Meta-Analysis with Meta-Regression.

The presence of pain sensitisation has been documented and reported as being a possible cause of treatment failure and pain chronicity in several musculoskeletal conditions, such as tendinopathies. The aim of the present study is to analyse existing evidence on pain sensitisation in tendinopathies comparing the local and distant pain thresholds of healthy and affected subjects with distinct analysis for different tendinopathies. PubMed, Cochrane Central Register, Scopus, and Web Of Science were systematically searched after registration on PROSPERO (CRD42020164124). Level I to level IV studies

Expression of alarmins in a murine rotator cuff tendinopathy model.

The aim of this study was to investigate the presence of alarmins in a novel murine rotator cuff tendinopathy model. Alarmins have been described as essential early activators of an immune response to tissue damage. Subacromial impingement was induced in both shoulders of 37 male C57Bl/6 mice by placement of a small metal clip in the subacromial space. Animals were allocated to different time points up to 6 weeks. The morphology and cellularity of the supraspinatus tendon were evaluated by hematoxylin-eosin staining, alcian blue, and picrosirius red. The expression and localization of alarmins

Percutaneous Zadek osteotomy for the treatment of insertional Achilles tendinopathy.

BACKGROUND: Insertional Achilles tendinopathy (IAT) is a challenging common lower extremity disorder, despite several treatment options described in literature. Open dorsal closing wedge calcaneal osteotomy or Zadek Osteotomy (ZO), for the treatment of the IAT has good clinical results but a high rate of postoperative complications. The purpose of this study is to describe percutaneous ZO for the treatment of the IAT and to evaluate its impact on the clinical and functional postoperative outcomes. METHODS: Twenty-six consecutive patients presenting with unilateral IAT refractory to nonoperati

Proximal to Distal Exostectomy for the Treatment of Insertional Achilles Tendinopathy.

UNLABELLED: When insertional Achilles tendinopathy is addressed surgically via a central-Achilles splitting approach, the calcaneal osteotomy has classically been performed from distal to proximal. We describe a simple proximal to distal technique that allows optimal resection of both the calcaneal exostosis and Achilles enthesophyte, minimizes risk to the soft tissues and skin, provides a bony attachment surface parallel to the axis of the Achilles tendon, and avoids the risk of osteotomy extension into the subtalar joint. LEVELS OF EVIDENCE: Level V: Technique tip.

Clinical Measures and Their Contribution to Dysfunction in Individuals With Patellar Tendinopathy.

CONTEXT: Patellar tendinopathy (PT) is prevalent in physically active populations, and it affects their quality of living, performance of activity, and may contribute to the early cessation of their athletic careers. A number of previous studies have identified contributing factors for PT; however, their contributions to self-reported dysfunction remain unclear. OBJECTIVE: The purpose of this investigation was to determine if strength, flexibility, and various lower-extremity static alignments contributed to self-reported function and influence the severity of PT. DESIGN: Cross-sectional res

Outcome of surgery for chronic patellar tendinopathy: A systematic review.

There is no consensus on how to best surgically treat chronic patellar tendinopathy. This systematic review investigates the surgical treatment for chronic patellar tendinopathy, and the outcomes. A database search was performed to identify all relevant articles, to which exclusion criteria were then applied. Data was extracted from 24 studies, and the outcomes were then systematically reviewed. The results showed that the success rate of surgery for patellar tendinopathy is generally over 77%. Return to sports rates are better for proximal tendon patients who undergo bony procedures. Patients
