Results of acute zone III extensor tendon injuries treated with dynamic extension splinting.

Motivated by the favorable results of dynamic splinting in the treatment of more proximal extensor tendon injuries, we explored the use of dynamic splinting in the treatment of zone III extensor tendon lacerations. Twenty-two tendon repairs in 19 patients were available for review. On the basis of degree of extension lag and pulp-to-palm distance, 14 repairs were rated as excellent, 6 as good, and 2 as fair. Using total active motion at 10 weeks, the proximal interphalangeal and distal interphalangeal joints averaged 170 degrees. There was one mild boutonniere deformity. The results of this pr

Tendon versus nerve transfers - balancing hand function in upper extremity high nerve injuries.

IntroductionHigh peripheral nerve injuries disrupt the balance between flexor and extensor muscle groups and result in significant impairment of strength, coordination and dexterity. Primary nerve repair is widely accepted as the optimal treatment following transection injury, but outcomes after high-level nerve reconstruction remain unpredictable. Prolonged denervation commonly leads to irreversible muscle atrophy, limiting the potential for meaningful distal functional recovery despite technically successful nerve repair.Reconstructive strategies:Different surgical strategies are available t

[Assessment of ligament and tendon injuries of the elbow using magnetic resonance imaging].

An overview about the normal anatomy and frequent pathologies of tendons and ligaments of the elbow using MRI will be provided. The unique joint configuration and articulation, as well as passive ligamentous and active muscle structures as well as the unique configuration of the articulating components of the elbow contribute to joint stabilization. MRI is an essential imaging modality in patients with ligamentous injuries including the sequelae of joint dislocation as well as chronic pathologies such as long-standing and refractory tendinopathies. Ideally, when reporting MRI of the elbow, the

Tibialis posterior tendon and deltoid and spring ligament injuries in the elite athlete.

The tibialis posterior tendon and the spring and deltoid ligament complexes combine to provide dynamic and passive stabilization on the medial side of the ankle and hindfoot. Some of the injuries will involve acute injury to previous healthy structures, but many will develop insidiously. The clinician must be aware of new treatment strategies and the level of accompanying scientific evidence regarding injuries sustained by athletes in these areas, while acknowledging that more traditional management applied to nonathletic patients is still likely to be appropriate in the setting of treatment f

Diagnosis and treatment of finger deformities following injuries to the extensor tendon mechanism.

Injuries to the finger extensor apparatus are very common and may produce chronic deformity and loss of function. Diagnosis is contingent on an understanding of the complex anatomy of this region as well as the ability to perform a careful physical examination. Immobilization is usually the most effective treatment of acute problems. Surgery is often necessary for chronic conditions, but the results are much less predictably corrective.

Early controlled active mobilization with dynamic splintage for treatment of extensor tendon injuries.

Early controlled active mobilization with a dynamic splint for treatment of repaired extensor tendon injuries was studied prospectively. Postoperative dynamic splintage started on the third postoperative day and involved elastic band extension and active flexion of the digits. Mobilization of the digits within the splint was done immediately under supervision. Range of motion was stepped up progressively and splintage was stopped after 5 to 6 weeks. Thirty-eight patients with 48 digits were reviewed. The average follow-up was 6.7 months (range, 4 to 10 months). Injuries involving the fingers s

PURPOSE: To evaluate the use of monopolar radiofrequency energy (MRFE) to shorten stretched dorsal extensor tendon apparatus (DETA) tissues in a canine model. METHODS: Eleven adult canine forelimbs were used in this in vitro investigation. The DETA tissue was isolated between the metacarpophalangeal and proximal interphalangeal joints in the third and fourth digits of each limb. Isolated tissue was stretched in all but 2 of the digits (control group). After tissue stretching, monopular radiofrequency energy (MRFE) was applied to 18 of the digits at 1 of 3 temperatures: 50 degrees C, 60 degree

The Independent Effect of Nerve Injury and Tendon Injury on Rotator Cuff Fatty Infiltration.

BACKGROUND: Muscular fatty infiltration (FI), atrophy, and fibrosis commonly occur after chronic rotator cuff tear and are barriers to successful rotator cuff repair. It is unclear if these phenomena result from tendon injury alone, neurological injury, or a combination. PURPOSE: To determine the role of suprascapular nerve mediated FI with and without chronic rotator cuff tear in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 22 rabbits underwent bilateral shoulder procedures. Extremities were randomized to surgical procedures: suprascapular neurotomy, supra

Clinical Updates on Superficial Digital Flexor Tendon Injuries: Insights on Diagnostic and Therapeutic Advancements.

Superficial digital flexor tendinopathy is one of the most common orthopedic soft tissue injuries in horses. The veterinary challenge for this condition is less related to identifying the presence of the injury (as it is usually readily evident clinically) but more related to improving the healing process so that the healed tendon is more like normal tendon in biology and function in order to reduce reinjury, which is the key adverse consequence of the injury. Diagnostic imaging has improved in recent years to aid with treatment choices and monitoring the healing process.

Flexor digitorum profundus tendon injuries in Zone 2 repaired with a modified Mantero technique.

From January 2010 to January 2017, 81 complete flexor digitorum profundus tendon disruptions in Zones 2B and 2C were treated using the modified Mantero technique. The patients were re-examined at a mean of 62 months (range 30-96) after operation. We analysed outcomes against ages, gender, pulley integrity, associated injuries and follow-up times. The median total range of motion of both interphalangeal joints, distal interphalangeal joint alone and Quick DASH scores were significantly better for the group with pulley vented versus no pulley vented. According to the Strickland and Glogovac crit

Early active wrist mobilization in extensor tendon injuries in zones 5, 6, or 7.

Historically, flexor tendon injuries have been the focus of multiple treatment techniques, splinting approaches, and protocols. However, extensor tendon injuries can also have significant problems, such as extensor lag and wrist tenodesis. Because these deficits seem to have less of an effect on our daily functions, when compared to the flexors, less emphasis is seen in the literature. These authors have set up a splinting and exercise program that could help elevate our dealing with problems associated with healing extensor tendons.
