Opponensplasty for Severe Carpal Tunnel Syndrome with the Transfer of the Palmaris Longus Tendon to the Rerouted Extensor Pollicis Brevis Tendon.

The authors describe a novel opponensplasty for severe carpal tunnel syndrome that uses the palmaris longus (PL) tendon transferred to the rerouted extensor pollicis brevis (EPB) tendon with pulley reconstruction using a portion of the PL tendon simultaneously with the carpal tunnel release. Like the Camitz opponensplasty, this technique utilises the PL as the motor source, does not require special postoperative treatment and enables fast functional recovery even in older patients. Compared with the Camitz procedure, this technique can easily acquire thumb rotation without tendon bowstringing.

Rupture of the posterior tibial tendon. Evaluation of injury of the spring ligament and clinical assessment of tendon transfer and ligament repair.

Eighteen of twenty-two patients who were having a tendon transfer to treat rupture of the posterior tibial tendon had evidence of injury to the spring ligament. The injury consisted of a longitudinal tear in the ligament in seven patients, a lax ligament without a gross tear in seven, and a complete rupture of the ligament in four. The ruptured posterior tibial tendon was treated with transfer of the flexor digitorum longus in twenty of the twenty-two patients. A variety of methods were used to repair the ligament. It is essential to determine the status of the spring ligament when patients ar

Results of transfer of the flexor digitorum superficialis tendons to the flexor digitorum profundus tendons in adults with acquired spasticity of the hand.

Thirty-one patients who had transfer of the flexor digitorum superficialis tendons to the flexor digitorum profundus tendons en masse in thirty-four non-functional spastic hands were examined at an average of fifty months postoperatively. All of the patients had had a clenched-fist deformity preoperatively, with severe hygienic problems of the palmar skin and no active function of the hand. Postoperatively, all of the hands were in an open position, which allowed for good hygiene of the palmar surface. A minor wound infection developed in three patients. Neurectomy of the motor branch of the u

Dual-Tendon Transfer for Chronic Extensor Pollicis Longus Ruptures: Augmented Extensor Indicis Proprius Transfer with Proximal EPL Stump Lengthening.

BACKGROUND: Extensor indicis proprius (EIP) transfer augmented with proximal extensor pollicis longus (EPL) stump lengthening restores thumb extension and optimizes function in cases of chronic EPL tendon ruptures, which impair hand dexterity and fine motor skills. Traditional EIP-to-EPL transfers often disrupt the natural oblique course of the EPL around the Lister tubercle, leading to functional deficits1-3. This dual-tendon transfer preserves anatomical alignment and improves thumb biomechanics, enhancing extension strength and the adduction moment arm at the carpometacarpal (CMC) joint. D

Endoscopic versus open flexor hallucis longus tendon transfer for reconstruction of chronic achilles tendon rupture: A prospective randomized pilot trial.

BACKGROUND: Chronic Achilles tendon rupture often requires surgical reconstruction. Flexor hallucis longus (FHL) tendon transfer is widely used, and endoscopic techniques may reduce soft-tissue morbidity and facilitate early recovery. METHODS: Thirty patients with chronic Achilles tendon rupture were randomized to undergo open or endoscopic FHL tendon transfer. Functional outcome was assessed using the Foot and Ankle Ability Measure (FAAM) over 18 months. Secondary outcomes included operative time and wound complications. Exploratory outcomes included postoperative pain assessed by visual ana

Combined Achilles Tendon-bone Block Allograft and Flexor Hallucis Longus Tendon Transfer for Long Segment Defects Involving the Insertional Region.

Long-segment disease involving the insertional region of the Achilles tendon can be a challenging problem. These patients often have significant disability and functional problems. Surgical management requires some type of tissue replacement, regional tissue rearrangement or tendon transfer. Various types of allograft tissue as well as synthetic materials have been described. Patients often have residual weakness and functional deficits following surgery. We have utilized an Achilles tendon-bone block allograft in combination with a flexor hallucis longus tendon transfer for long-segment Achil

Endoscopic Flexor Hallucis Longus Tendon Transfer for Reconstruction of the Achilles Tendon Rupture in High-Risk Patients: A Case Series.

The neglected Achilles tendon rupture requires surgical reconstruction for the best functional outcome. According to the current literature, there are many reconstructive options available that demonstrate acceptable functional results in most cases. These procedures require large incisions, leading to potential wound-healing complications. Therefore, these procedures may not be suitable for patients who are at high risk for wound-healing problems. A minimally invasive approach is desirable in this situation to decrease the risk of this potential complication. Endoscopic transfer of the flexor

Single-stage reconstruction of achilles tendon rupture with flexor hallucis longus tendon transfer and simultaneous free radial fasciocutaneous forearm flap.

Reconstruction of the Achilles tendon is challenging but critical for successful ambulation. The goals of Achilles tendon reconstruction are to restore power of plantar flexion, restore normal range of movement of the ankle joint, and ensure durable and pliable soft tissue coverage. We present a dehiscence of the Achilles tendon and partial soft tissue loss secondary to infection. Simultaneous Achilles tendon reconstruction with flexor hallucis longus (FHL) tendon transfer and soft tissue reconstruction with free radial forearm flap was performed as a single-stage procedure. The FHL transfer p

Principles and biomechanics of muscle tendon unit transfer: application in temporalis muscle tendon transposition for smile improvement in facial paralysis.

Muscle tendon unit (MTU) transfer is a common procedure performed to restore hand function after peripheral nerve or spinal cord injury. The principles of MTU transfer established for hand surgery can be adopted to optimize the dynamic excursion of the temporalis tendon transfer procedure for facial reanimation. Additionally, the force generating ability of a transferred MTU depends on the ideal length-tension relationship of the donor muscle. There are unclear guideline for selecting the ideal tension at which a transferred MTU will generate maximum force and excursion and current practice of

[Intramuscular lengthening and range of motion after local tendon transfer for repair of retracted supraspinatus tendon defects. A biomechanical study].

BACKGROUND: The current cadaver study was performed to evaluate and compare infraspinatus (ISP) vs. subscapularis (SSC) tendon transfer used for the repair of a large supraspinatus (SSP) rotator cuff defect. The main outcome measures were the range of motion and intramuscular length changes during motion. METHODS: Thirteen fresh-frozen cadaver shoulders were used in the study. A supraspinatus defect extending to the apex of the humeral head (Patte size II) was created. Transosseous repair was then attempted with the ISP and with the SSC in all cases; intramuscular length changes were measured

Gout tophus in transferred tendons after 33 years: A case report.

Surgical site gout is an extremely rare complication that is difficult to diagnose, particularly in patients without a history of gout. A 57-year-old male patient was admitted with no previous history of gout, complaining of surgical site gout located at the junction where flexor carpi ulnaris tendon was transferred to extensor digitorum communis tendon after 33 years of the initial surgery. The patient was presented with a progressive swelling over the last three months which was located on the dorsoulnar side of the right wrist joint. Magnetic resonance imaging revealed an iso/hypointense ma

[Tendon transfers around the shoulder for patient with brachial plexus injury sequels].

Brachial plexus lesions can significantly affect patient function. The type of management will depend on the severity of the injury as well as the anatomical location. Tendon transfers around the shoulder have emerged as an effective treatment alternative, and therefore we should consider them when faced with patients who might require it. There are various transfer techniques, within which the most frequently used are upper trapezium, latissimus dorsi and lower trapezium transfers, each with its specific indications and objectives depending on the type of patient. These surgeries aim to decre

The surgical anatomy of the dorsal scapular nerve: a triple-tendon transfer perspective.

BACKGROUND: Iatrogenic or traumatic injury to the spinal accessory nerve is a rare but debilitating injury. An effective treatment, known as the Eden-Lange modification triple-tendon transfer procedure, involves the transfer of the rhomboid major (RM), rhomboid minor (Rm), and levator scapulae (LS). Careful detachment of their insertions is necessary to avoid injury of the dorsal scapular nerve (DSN). This study evaluated the surgical anatomy and safety of the DSN relative to this procedure. METHODS: The study used 12 cadavers (22 shoulders). The RM, Rm, and LS were detached from their insert

Surgical Planning for Flexor Digitorum Longus Tendon Transfers: An Anatomic Study.

Posterior tibial tendon dysfunction is often coupled with various degrees of hindfoot valgus and equinus. Preoperative planning is essential to appropriate procedure choice and surgical efficiency. The purpose of the present study was to assess the anatomy at the harvest site for flexor digitorum longus tendon transfer, specifically at the master knot of Henry. Thirty fresh-frozen below-the-knee cadavers were used for dissection. A standard anatomic approach was performed for posterior tibial tendon debridement and flexor digitorum longus tendon transfer. The flexor digitorum longus tendon was

Outcome of Lateral Transfer of the FHL or FDL for Concomitant Peroneal Tendon Tears.

BACKGROUND: Concomitant tears of the peroneus longus and brevis tendons are rare injuries, with literature limited to case reports and small patient series. Only 1 recent study directly compared the results of single-stage lateral deep flexor transfer, and no previous series objectively evaluated power and balance following transfer. The purpose of this study was to evaluate clinical outcomes, patient satisfaction, and objective power and balance data following single-stage flexor hallucis longus (FHL) and flexor digitorum longus (FDL) tendon transfers for treatment of concomitant peroneus lon

Plantar pressures following anterior tibialis tendon transfers in children with clubfoot.

BACKGROUND: Relapses following nonoperative treatment for clubfoot occur in 29% to 37% of feet after initial correction. One common gait abnormality is supination and inversion of the foot caused by an imbalance of the anterior tibialis tendon muscle. The purpose of this study was to determine if plantar pressures are normalized following an anterior tibialis tendon transfer (ATTT). METHODS: Thirty children (37 clubfeet) who underwent an ATTT, were seen for plantar pressure testing preoperatively and postoperatively. Each foot was subdivided into 7 regions: medial/lateral hindfoot and midfoot

Complications after tendon transfers in the foot and ankle using bioabsorbable screws.

BACKGROUND: Tendon transfers are commonly used for correction of pathology or deformity of the foot and ankle. Bioabsorbable implants have been developed as an alternative to metal interference screws. The purpose of this study was to document complications following tendon transfers of the foot and ankle using bioabsorbable poly-L-lactide interference screws. METHODS: A retrospective chart review was used to identify patients in whom either of the 2 senior authors had performed a tendon transfer of the foot and ankle using a bioabsorbable interference screw between 1999 and 2005. Tendon transfers for peroneal nerve injuries in the multiple ligament injured knee.

Although knee dislocations are relatively rare injuries; associated drop foot complication as a consequence of common peroneal nerve palsy (CPN) is substantially high. Even after successful ligament constructions; unresolved CPN palsy is a factor contributing to bad outcome after knee dislocations. CPN palsy is seen more after open dislocations, rotatory dislocations, and especially in patients with posterolateral corner injuries. CPN palsy can readily be diagnosed clinically, although high index of suspicion is needed. Surgical exploration in the acute setting is still debatable. Conservative
