Lateral extraarticular tenodesis improves stability in non-anatomic ACL reconstructed knees: in vivo kinematic analysis.

PURPOSE: To carry out an in vivo kinematic analysis to determine whether adding a lateral extraarticular tenodesis (LET) for those patients with subjective instability and objective residual laxity after a transtibial (TT) anterior cruciate ligament reconstruction (ACLR) reduces anteroposterior and rotational laxity and to evaluate the 2-year follow-up clinical outcomes to analyze whether biomechanical changes determine clinical improvement or not. METHODS: A total of 19 patients with residual knee instability after TT ACLR who underwent a modified Lemaire LET were prospectively evaluated for

Shoulder Arthroscopic Rotator Cuff Repair With Biceps Tenodesis and Acromioplasty Using a Single Working Portal.

Rotator cuff repair, acromioplasty, and biceps tenodesis operations have become some of the most common shoulder surgical procedures, evolving from open techniques to minimally invasive arthroscopic techniques. The use of many arthroscopic portals has been associated with surgical risks to many surrounding anatomic structures. We present an arthroscopic technique using a single anterolateral working portal for rotator cuff repair, acromioplasty, distal clavicle excision, and long head of the biceps tenodesis; this technique decreases the risk of injury to the surrounding neurovascular and musc

Treatment of chronic scapholunate instability: Results with three-ligament tenodesis vs. scapholunate and intercarpal ligamentoplasty.

In this retrospective case control/comparison study, we compared the clinical and radiological outcomes in patients with chronic scapholunate dissociation treated with three-ligament tenodesis (3 LT) versus scapholunate and intercarpal ligamentoplasty (SLICL). Twenty patients with a mean age of 43 years were treated with the 3 LT procedure and 26 patients with a mean age of 44 years with the SLICL procedure. All patients had chronic reducible scapholunate dissociation without chondral lesions. The two groups of patients were operated on by senior surgeons, at the same facility, over two differ

Arthroscopic ligamentoplasty (bone-tendon-tenodesis). A new surgical technique for scapholunate instability: preliminary cadaver study.

We present an anatomical study and description of a new surgical technique for arthroscopic treatment of scapholunate ligament injuries. Five cadaver specimens were used to perform the technique. After arthroscopic surgery, anatomic dissection was performed to measure the distances to critical wrist structures such as the posterior interosseous nerve and the radial artery, and the size and position of the plasty. This arthroscopic technique offers three advantages: soft tissue damage is reduced (avoiding an extensive approach and injury to the secondary stabilizers and reducing scar tissue); i

[New arthroscopic portal for performing tenotomy/tenodesis procedures on the long head of the biceps brachii tendon].

BACKGROUND: Shoulder arthroscopy is the standard technique for performing procedures involving the intertubercular groove. Current techniques continue to produce excessive soft tissue manipulation and neurovascular injury. MATERIALS AND METHODS: A cross-sectional, observational and descriptive study was conducted on a cohort of 24 shoulders following the standard surgical protocol and using punch dissection. The neurovascular structures with risk of damage by the standard lateral portal were evaluated during the study to establish a secure area for a new arthroscopic portal. Finally, the safe

Proximal humeral fractures treated with hemiarthroplasty: does tenodesis of the long head of the biceps improve results?

INTRODUCTION: Pathology of the long head of the biceps (LHB) may be the cause of anterior shoulder pain after hemiarthroplasty for treatment of fractures of the proximal humerus. The currently available literature lacks adequate randomised trials examining whether tenodesis of the LHB improves results. The purpose of this study was to evaluate the effects of tenodesis of the LHB on the clinical outcome following hemiarthroplasty for fractures of the proximal humerus. METHODS: This prospective randomised study included 37 patients treated with hemiarthroplasty for four-part fractures, fracture

Treatment of distal biceps tendon ruptures using a single-incision technique and a Bio-Tenodesis screw.

No consensus exists in the literature on the optimal operative treatment method for distal biceps tendon ruptures. It is our hypothesis that a single-incision technique with a poly-L-lactide Bio-Tenodesis screw (Arthrex, Inc, Naples, Florida) is a safe and effective method for operative management of distal biceps tendon ruptures, with success and complication rates comparable to previous reports in the literature. This article describes a prospective case series of 29 consecutive patients (30 operations) managed by the same surgeon over 34 months. Average follow-up was 19.6 months. Two patien

Extensor tenodesis for plexic hands with C7 to T1 or C8, T1 root avulsions: a new technique.

In C7 to T1 or C8, T1 root avulsion palsies, restoration of finger active extension is not possible. Only tenodesis may restore hand opening in active wrist flexion. Many techniques have been described to restore this motion. In routine techniques, extensor tendons are fixed on radius or sutured on dorsal retinaculum. However, in these procedures, progressive tendon lengthening or ruptures may occur and salvage procedure may be difficult to perform. Therefore, we proposed a new extensor tenodesis technique. The extensor digitorum communis tendons are sutured on the paralyzed flexor digitorum s

Combined Anterior Cruciate Ligament Reconstruction and Lateral Extra-articular Tenodesis With Augmented Continuous Iliotibial Band Autograft.

The purpose of this technical note is to describe a reproducible surgical technique for anterior cruciate ligament reconstruction combined with a lateral tenodesis using a continuous iliotibial band autograft that allows self-augmentation of graft diameter without the need for additional tendon harvesting. The key modification involves creating an augmented intra-articular graft by inverting a detached distal iliotibial band strip over the central portion of the graft, thereby increasing its diameter while preserving native continuity.

Onlay Fixation With an All-Suture Anchor Provides Sufficient Biomechanical Stability for Lateral Extra-articular Tenodesis.

BACKGROUND: Spiked ligament staples (SLSs) and biocomposite suture anchors (BSAs) are commonly used onlay fixation devices for modified Lemaire lateral extra-articular tenodesis (LET). However, hardware prominence, tunnel penetration, footprint size, and irritation are documented issues. Knotless all-suture anchors (ASAs) are a promising alternative that mitigates these issues. PURPOSE: To compare the biomechanical behavior of LET using ASAs with SLSs and BSAs under simulated anterolateral forces. STUDY DESIGN: Controlled laboratory study. Scapulothoracic tenodesis using hamstring tendon graft for treatment of problematic scapula winging: A new surgical technique.

INTRODUCTION: Winging of the scapula occurs due to dysfunction of its stabilising muscles, most commonly serratus anterior and/or trapezius, for example in facioscapulohumeral muscular dystrophy. Resultant loss of scapular control and abnormal kinematics can decrease shoulder function due to glenohumeral joint instability, loss of range of motion and pain. Previously described treatment for cases resistant to physiotherapy includes scapulothoracic arthrodesis which involves risk of non-union and metalwork failure, as well as reduced respiratory function due to immobilisation of a segment of th
