Acute de Quervain's Tenosynovitis Caused by Calcium Pyrophosphate Dihydrate Deposition.

A patient presented with acute de Quervain's tenosynovitis with no history of trauma or overuse. Infection was ruled out. Aspiration of the involved tendon sheath yielded a significant quantity of tenosynovial fluid closely resembling viscous synovial fluid, which showed intracellular calcium pyrophosphate dihydrate crystals. X-rays showed flecks of linear calcification in the region of the tendon sheath. This is a rare clinical presentation of calcium pyrophosphate dihydrate disease.

Granulomatous tophaceous gout mimicking tuberculous tenosynovitis: report of two cases.

Granulomatous inflammation in a tissue specimen raises concern about infection with Mycobacterium tuberculosis, atypical mycobacteria, certain fungi, Brucella species, and other infectious agents. Inflammatory disorders, such as sarcoidosis, crystal-associated arthritis, or foreign body reactions also are considered when granulomatous changes are seen on histological examination of a tissue specimen. We describe two cases of granulomatous tenosynovitis due to tophaceous deposits in patients with gout. In one case, tuberculous synovitis was considered the primary diagnosis until the diagnosis o

Tenosynovitis With Psammomatous Calcifications: A Distinctive Trauma-Associated Subtype of Idiopathic Calcifying Tenosynovitis With a Predilection for the Distal extremities of Middle-Aged Women-A Report of 23 Cases.

The term "idiopathic calcifying tenosynovitis" (ICT) refers to a clinically and radiologically defined syndrome of pain and tendinous calcifications, most often involving the shoulder joint. A distinctive subset of ICT cases, termed "tenosynovitis with psammomatous calcifications" (TPC), occurs in the distal extremities and shows characteristic morphology, in particular psammomatous calcifications. As only 14 cases have been reported to date, TPC remains poorly recognized by both pathologists and clinicians. Twenty-three well-characterized cases of TPC along with all available radiologic and c

Soil to Sheath: Nocardia Flexor Tenosynovitis in an Immunocompetent Pottery Artist.

Stenosing flexing tenosynovitis (trigger finger) is a chronic, noninfectious condition of the flexor tendon sheath, whereas infectious flexor tenosynovitis is an acute hand infection that requires urgent recognition and treatment. Infectious flexor tenosynovitis involves bacterial inoculation of the flexor tendon sheath and can rapidly progress to tendon necrosis, stiffness, and even amputation if diagnosis and surgical management are delayed. It is classically associated with Kanavel's four cardinal signs and is considered a surgical emergency in the hand.

Optimal management of pyogenic flexor tenosynovitis of the hand: a cohort study.

BACKGROUND: Pyogenic flexor tenosynovitis (PFT) is a common hand infection associated with significant morbidity. Management varies due to lack of evidence-based guidelines, typically involving surgery and antibiotics. We conducted a cohort study, comparing outcomes between different surgical methods to see if one is optimal. METHODS: Retrospective data collection of patients who underwent surgery for PFT in a single tertiary plastic surgery unit between 21 August 2013 and 4 October 2023 was undertaken. Demographics, injury mechanism, operative details and antibiotic use were recorded. Cases

A Rare Manifestation of Extrapulmonary Tuberculosis: Tenosynovitis of Wrist and Multiple Fingers.

Tuberculous tenosynovitis is a rare extrapulmonary manifestation of tuberculosis and often presents with nonspecific symptoms that mimic more common inflammatory or degenerative conditions. We report a unique case from Eastern Morocco involving a 39-year-old male with a two-year history of progressive swelling and limited flexion of the wrist and the second, third, and fifth fingers. Imaging revealed tenosynovitis of the flexor tendons, and histopathological examination confirmed tuberculosis through the presence of granulomatous inflammation with caseous necrosis. The patient underwent a surg

Navigating the Diagnostic Maze: A Case of Post-COVID-19 Tenosynovitis.

Post-COVID-19 sequelae can include various complications, including musculoskeletal manifestations. Unvaccinated against COVID-19, a 21-year-old woman initially experienced severe COVID-19 and subsequently recovered. She later developed mild COVID-19, which was followed by severe myalgia and joint pain in her upper extremities. After a third COVID-19 infection, she experienced intense pain in multiple hand joints and lethargy. The blood tests were normal, and the ultrasound report revealed tenosynovitis of the wrist joint. Her pain improved with combined treatment approaches.  To our knowledge

Histoplasma capsulatum tenosynovitis: An unusual presentation in a kidney transplant recipient.

Histoplasmosis is an expected endemic mycosis in solid organ transplant recipients and occurs as a primary infection, reactivation, or, rarely, acquired from an infected allograft. Reactivation is favored by maintenance immunosuppression or anti-rejection therapy, which facilitates the appearance of disseminated forms as well as unusual presentations. We present the case of a 66-year-old woman with isolated tenosynovitis due to Histoplasma capsulatum 25 years after a kidney transplant.

The value of ultrasound-defined tenosynovitis and synovitis in the prediction of persistent arthritis.

OBJECTIVES: The value of US-defined tenosynovitis in predicting the persistence of inflammatory arthritis is not well described. In particular, the predictive utility of US-defined tenosynovitis of larger tendons is yet to be reported. We assessed the value of US-defined tenosynovitis alongside US-defined synovitis and clinical and serological variables in predicting persistent arthritis in an inception cohort of DMARD-naïve patients with early arthritis. METHODS: One hundred and fifty DMARD-naïve patients with clinically apparent synovitis of one or more joints and a symptom duration of ≤3 m

Histoplasmosis tenosynovitis of the forearm and wrist: imaging, surgical and pathologic findings.

Histoplasmosis is a disease endemic to several parts of the world, including South America. It progresses in a mostly asymptomatic and self-limiting manner but has the potential to cause disseminated pulmonary infection, especially in immunocompromised patients. Rare reports from the literature describe musculoskeletal manifestations related to the two varieties of the pathogen that cause histoplasmosis, namely, the capsulatum or duboissi variety. In this report, we describe the case of a previously healthy, middle-aged man, with slowly progressing pain in the right forearm, wrist, and fingers

Are Plain X-Rays Necessary in the Diagnosis of De Quervain's Tenosynovitis?

Background  The routine use of plain radiography represents a significant expenditure and has been proven unnecessary in several orthopedic conditions. The utility of plain radiographs in the diagnosis of De Quervain's tenosynovitis (DeQ) is not clear. Questions/Purpose  This study evaluates whether plain radiographic findings routinely predict the need for surgery or alter treatment courses in the initial diagnosis of DeQ. Patients and Methods  A total of 200 patients who received wrist X-rays and had a diagnosis of DeQ were retrospectively selected at random. Their images were evaluated for

Ultrasonography for diagnosis, monitoring and treatment of tenosynovitis in patients with rheumatoid arthritis.

Rheumatod arthritis is a chronic systemic autoimmune disease, characterized by inflammation in joints and tendon sheaths, which frequently leads to permanent and serious disability due to joint destruction, but also tendon and ligament ruptures. Clinical management of rheumatoid arthritis has traditionally been supported by biochemical and radiographic findings. However, imaging modalities like ultrasound and magnetic resonance imaging (MRI) have improved the possibility for better management of rheumatoid arthritis patients, due to higher sensitivity and specificity for detecting ongoing infl

Gonococcal Tenosynovitis Diagnosed with the Aid of Emergency Department Bedside Ultrasound.

BACKGROUND: Gonorrhea is the second most common sexually transmitted infection. Disseminated gonococcal infection (DGI) consists of gonococcal infection plus one or more of the triad of arthritis, tenosynovitis, and dermatitis. Diagnosis in the emergency department (ED) must be suspected clinically, as confirmatory tests are often not available. Point-of-care ultrasound (POCUS) can aid in diagnosis and appropriate management by identifying tenosynovitis and excluding arthritis. CASE REPORT: A 26-year-old man with multiple recent sex partners presented to the ED with slowly progressing right w

[Isolated tuberculous tenosynovitis of the Achilles tendon: a report of two cases].

Tuberculous tenosynovitis involving the tendons of the feet is very rare. Isolated primary tuberculous tenosynovitis of the Achilles tendon was detected in two women aged 19 and 53 years, respectively. The younger patient had a swollen and painful Achilles tendon in the left foot. Complete excision of the lesion followed by antituberculous chemotherapy for six months resulted in complete improvement. Magnetic resonance imaging showed normal findings at the end of six months and no recurrence after 27 months of follow-up. The older patient had diabetes and was on cytostatic treatment for rheuma
