They're Dying. Now What? The Impact of a 2-Hour End-of-Life Educational Intervention on Bedside Nurse Self-efficacy, Knowledge, and Clinical Practice.

The palliative care team identified a lack of knowledge and confidence for nurses providing end-of-life care in a hospital. The team completed a quality improvement project establishing significant improvement in knowledge and self-efficacy after a 2-hour educational intervention. The next step was to study the impact on practice. Clinical management of pain, dyspnea, secretions, and agitation was compared for a 3-month period before and after intervention. Thirty-six patients were identified in the preintervention group, and 46 patients were in the postintervention group. A 2-sample proportio

Management of Physical Symptoms in Patients with Advanced Cancer during the Last Weeks and Days of Life.

Patients with advanced cancer are faced with many devastating symptoms in the last weeks and days of life, such as pain, delirium, dyspnea, bronchial hypersecretions (death rattle) and intractable seizures. Symptom management in the last weeks of life can be particularly challenging because of the high prevalence of delirium complicating symptom assessment, high symptom expression secondary to psychosocial and spiritual factors, limited life-expectancy requiring special considerations for prognosis-based decision-making, and distressed caregivers. There is a paucity of research involving patie

Place of Death Trends and Utilization of Outpatient Palliative Care at the End of Life—Analysis of Death Certificates (2001, 2011, 2017) and Pseudonymized Data From Selected Palliative Medicine Consultation Services (2017) in Westphalia, Germany.

BACKGROUND: The largest German study on place of death was performed for deaths in selected regions of Westphalia in the years 2001 and 2011. In the period thereafter, provision of palliative care was regionally expanded. This upgrade included the establishment of palliative medicine consultation services (PMCS), combining general and specialized palliative care on an outpatient basis. A follow-up place of death survey took place in 2017. The aim was to depict the trends in place of death between 2001 and 2017. A second goal was to determine how often outpatient PMCS were used by persons who d

End of Life in Prison.

What a good end of life means is a particularly relevant question in the context of confinement and prison. Most of the questions and issues raised by end of life for those living in liberty also apply to the correctional setting. However, the institutional particularities and logics of the prison create unique barriers and make it difficult in practice to reconcile concerns in regard to end of life-like care and comfort-with the mandate of corrections-confinement and punishment. At present, the literature on end of life in prison is dominated by U.S. contributions. We have therefore invited r

Psychiatry and the dying prisoner.

Due to the growing number of ageing prisoners in the American correctional system, penal institutions are increasingly caring for patients with chronic and potentially terminal medical conditions. To address this problem states have attempted sentencing reform initiatives and adopted compassionate release programmes; however, these efforts have failed to significantly reduce the number of elderly or seriously ill inmates. Correctional mental health services are now called upon to aid in the care of prisoners at the end of life. This article presents the common elements of prison hospice progra

Anticholinergics in palliative medicine: an update.

Anticholinergics, or antimuscarinic drugs, are drugs that competitively inhibit the action of acetylcholine at muscarinic receptors, leading to a blockade of the actions of the parasympathetic nervous system at sites where overactivity can lead to increased symptom burden. Successful blockade of the parasympathetic nervous system ultimately leads to decreased production of secretions in the salivary, bronchial, and gastrointestinal tracts. These effects are often used for several symptoms that originate due to parasympathetic nervous system overactivity, such as the "death rattle" and malignan

An optimisation framework for resource allocation in palliative and end-of-life care.

End-of-life care for frail and elderly patients is frequently characterised by high healthcare utilisation, fragmented service delivery, and limited coordination, resulting in variable quality and excess cost. This study presents a proof-of-concept framework, tested using synthetic data to illustrate potential applications in strategic planning. Few planning approaches integrate patient-level pathways into operational models that balance efficiency with patient-centred outcomes. Optimisation models were developed to support strategic resource planning for frail, elderly, and palliative patient

Blood Transfusion in End-of-Life Cancer Care: Clinical Evidence, Patient Blood Management and Goal-Concordant Practice.

Blood transfusion near the end of life for patients with cancer may relieve symptoms attributed to anaemia or control bleeding, but its palliative value depends on whether benefit occurs within the patient's expected survival and outweighs the clinical and practical burden of treatment. This structured narrative review examines evidence concerning red-cell and platelet transfusion, transfusion-sparing patient blood management, hospice access and alternative delivery models in adults receiving palliative or end-of-life care. Structured searches of MEDLINE, Embase, Scopus and the Cochrane Librar

Prognostication in advanced cancer: foreseeing from global insights and foretelling in the Asian context.

Accurate prognostic information is crucial for guiding end-of-life (EOL) decision-making in advanced cancer care. Although the European Society for Medical Oncology (ESMO) recommends using clinicians' prediction of survival (CPS) as an initial reference, CPS alone often lacks precision. This review synthesizes current prognostic models and the dialog surrounding EOL survival prediction. For patients with an expected survival of months, several validated prognostic tools are available, including measures such as the Eastern Cooperative Oncology Group Performance Status, the modified Glasgow Pro

Advance care planning.

Advance care planning (ACP) is done in anticipation of something adverse happening and the likelihood of losing the capacity to be involved in future decision making. ACP encourages people to think about what might happen in serious illness scenarios and to consider their needs or wishes. As long-term conditions, multimorbidity, frailty and end-of-life care become more dominant health challenges, planning for future problems and giving patients and their carers the tools to self-manage becomes more imperative. ACP is part of this philosophy of care, anticipating and planning for future health

How to distinguish opioid toxicity from natural dying in patients with advanced illness and how to manage opioid toxicity?

Patients with advanced, life-limiting illness might develop pain or breathlessness, requiring opioids. Opioid neurotoxicities, like sedation and delirium, overlap with signs of natural dying. Understanding natural dying is a core clinical skill for all healthcare professionals. It is important that clinicians accurately assess patients to distinguish opioid toxicity from natural dying. This is vital to ensure appropriate use of opioids and ensure patient comfort. Patients with opioid toxicity and no pain can usually be managed by reducing the opioid dose. In patients with opioid toxicity and p

A Review on the Management of Symptoms in Patients with Incurable Cancer.

Palliative care aims to alleviate suffering and prioritize the quality of life of patients facing serious and fatal diseases, such as cancer. Cancer patients, especially in advanced stages, often have many difficult-to-control symptoms, such as pain, fatigue, dyspnea, anxiety, and depression, requiring the attention of a multidisciplinary team highly trained in palliative care and end-of-life management. Pain, dyspnea, nausea, and vomiting are the focus of symptomatic assessment in palliative care, but patients experience other equally important symptoms that do not receive as much attention a

Serious health-related suffering and palliative care in South Asian countries.

PURPOSE OF REVIEW: South Asia, with a population of 1.79 billion, has close to 9.9 million individuals experiencing serious health-related suffering (SHS). South Asia accounts for 10.23% of the world's cancer incidence. The prevalence of cancer mortality in the region is 68.44%, significantly higher than the world average of 59.74%. This poor outcome highlights the need to focus on SHS in the region. This review was undertaken to assess the current status of palliative care inequities in the region and to guide future action. RECENT FINDINGS: Using morphine-equivalent doses of opioid analgesi

[The effects of cancer treatments at the end of life: a clinical, ethical, and organizational issue].

The use of cancer treatments in the terminal stages of life is an increasingly important clinical, ethical, and organizational issue. Despite recommendations from major scientific organizations to reduce active treatments for patients with a limited life expectancy, significant evidence shows that these therapies continue to be used even during the final days or weeks of life. This trend raises important questions regarding the quality of care, the appropriate balance between risks and benefits, the effective use of healthcare resources, communication between physicians and patients, and the i

Palliative Care Aspects in Multiple Sclerosis.

CONTEXT: Multiple sclerosis (MS) is an inflammatory, chronic, demyelinating, and neurodegenerative disorder of central nervous system, determined by an auto-immune dysfunction. Severe disability generally occurs in patients with progressive forms of MS that typically develop either after an earlier relapsing phase or less commonly from disease onset. Despite advances in research to slow the progression of MS, this condition remains a life-limiting disease with symptoms impacting negatively the lives of patients and caregivers. OBJECTIVES: To analyze the difefrent aspects of palliative cae in

A wedding in an intensive care unit.

Humanization in critical care has been increasingly discussed, raising awareness to the wide range of end-of-life measures which have been recently revised. Nonetheless, in the absence of consensus on what adequate humanization in critical care should theoretically encompass, different perceptions, either clinical or non-clinical, hinder the implementation of site-specific measures. To our knowledge, there has been no previous report of a wedding taking place in an ICU, specifically, as an integrative part of palliative care.
