Chest radiography is the most common method to confirm a diagnosis of pneumonia among patients with an appropriate clinical presentation. Radiography and computed tomography demonstrate different imaging patterns in pneumonia, depending on the distribution of lung injury. Lung ultrasound can also identify pneumonia consolidation and interstitial edema. Ultrasound guidance may aid diagnostic thoracentesis.

A patient presented to the emergency department with tachypnea, fever, a right pleural effusion, and lung consolidation. The computed tomographic guided thoracentesis yielded a greenish fluid with bilirubin. The ultrasound examination demonstrated a distended gallbladder with stones, positioned on the ventral face of the liver and a free communication between the fundus and the pleural cavity. The cholecystopleural fistula was confirmed at operation.

Chylothorax is an uncommon cause of pleural effusion and is most frequently associated with trauma or malignancy. Its occurrence in Behçet's disease (BD) is exceptionally rare and is usually related to extensive central venous thrombosis causing obstruction of the thoracic duct. We report the case of a 35-year-old woman with a five-year history of BD who presented with progressive dyspnea, dry cough, and mild hemoptysis. Chest computed tomography (CT) revealed extensive thrombosis of the superior vena cava extending to both brachiocephalic veins and the right subclavian vein, associated with l

INTRODUCTION AND IMPORTANCE: Pancreatic pseudocysts (PPCs) cause two-thirds of pancreatic cystic diseases; pancreaticopleural fistula (PPF), a rare (0.4% in pancreatic diseases) life-threatening complication, occurs when mediastinal pseudocysts (60% from PPCs) rupture into the thorax, with 4.5% risk in PPC patients (highest in middle-aged males with alcohol-related chronic pancreatitis). Its nonspecific symptoms (dyspnea, mild/absent abdominal pain) delay diagnosis.
CASE PRESENTATION: A 44-year-old male with a 3-year recurrent acute pancreatitis and hyperlipidemia was admitted for sudden dysp

Chylothorax is an uncommon cause of pleural effusion resulting from the disruption or obstruction of the thoracic duct or its tributaries. Malignancy is a major non-traumatic cause, most commonly lymphoma, while involvement of gynecologic malignancies is exceedingly uncommon and largely limited to isolated case reports. We report the case of a 61-year-old woman with a prior history of poorly differentiated endometrial carcinoma treated in 2022 with total abdominal hysterectomy, bilateral salpingo-oophorectomy, and adjuvant brachytherapy, who presented three years later with progressive dyspnea

OBJECTIVE: To investigate the etiology of pleural effusions (PE) in adults and the accuracy of pleural fluid (PF) cytology and cultures in malignant and infectious PE, respectively.
PATIENTS AND METHODS: Retrospective analysis of all consecutive patients with PE undergoing diagnostic thoracentesis during the last 19 years in a university hospital.
RESULTS: The leading causes of PE among the 3,077 patients were: cancer (27%), heart failure (21%), pneumonia (19%), tuberculosis (9%), abdominal surgery (4%), pericardial diseases (4%) and cirrhosis (3%). Tuberculosis was the most common etiology

Attenuation values in computed tomography (CT) are used as a diagnostic aid in certain clinical conditions. In our study, we investigated the effectiveness of attenuation values, obtained through the noninvasive method of CT, in determining the type of pleural effusions in the emergency department. Patients who presented to the emergency department with a diagnosis of pleural effusion and underwent thoracentesis within 48 hours between January 1, 2023, and January 1, 2024, were included in the study. Exclusion criteria were patients under 18 years of age, those without CT imaging, those presen

Empyema evolves through exudative, fibrinopurulent and organized phases throughout 3 to 6 weeks. Clinical symptoms, a chest skiagram followed by thoracentesis, are enough for diagnosis. Most patients improved with systemic antibiotics, thoracentesis and chest tube drainage. The present case represents a four-year-old girl who had a fever, cough, chest pain and breathing difficulty for two weeks. Right-sided empyema was diagnosed after admission to our facility, which had not responded to appropriate antibiotics and closed chest tube drainage. Re-evaluation of the patient indicates a possible c

BACKGROUND: In patients with pleural effusion, specific ultrasound characteristics are associated with pleural malignancy.
OBJECTIVES: This study aimed to evaluate the added value of an additional, up-front, systematic thoracic ultrasound (TUS) to standard imaging in patients with unilateral pleural effusion of unknown cause in a clinical setting.
METHODS: In a prospective observational pilot study, patients referred for workup and thoracentesis of a unilateral pleural effusion received up-front TUS following a set protocol in addition to available imaging and US guiding the thoracentesis or

We aimed to (1) evaluate the incidence of tuberculous pleural effusion (TPE) over 25 years in our centre; (2) measure the yield of different diagnostic techniques; (3) compare TPE features between immigrant and native patients. Retrospective study of patients who underwent diagnostic thoracentesis and pleural biopsy in our hospital between 1995 and 2020. TPE was diagnosed in 71 patients (65% natives, 35% immigrants). Onset was acute in 35%, subacute in 26% and prolonged in 39%. Radiological features were atypical in 42%. Thoracentesis specimens were lymphocyte-predominant in 84.5% of patients,

Objectives: Pleural effusion is a common reason for hospital admission with thoracentesis often required to diagnose an underlying cause. This study aimed to determine if the imaging characteristics of TUS effectively differentiates between transudative and exudative pleural fluid. Methods: Patients undergoing TUS with pleural fluid analysis were retrospectively identified at a single center between July 2016 and March 2018. TUS images were interpreted and characterized by established criteria. We determined diagnostic performance characteristics of image criteria to distinguish transudative f

Urinothorax [UT], the accumulation of urine in the pleural space, is an uncommon cause of pleural effusions resulting from trauma, obstruction, or iatrogenic causes. Thoracentesis with pleural fluid analysis and evaluation of biochemical characteristics, such as pleural fluid creatinine (PCr) to serum creatinine ratio (Scr), is necessary to establish this diagnosis. This case illustrates a 93 year old man with a complicated past medical history including chronic kidney disease stage 4, adenocarcinoma of the prostate status post brachytherapy complicated by proctitis, high grade transitional ce

Light's criteria are the cornerstone to differentiate exudates from transudates. The traditional literature states that malignant pleural effusions are rarely transudative; therefore, cytology tends to be low yield and not a cost-effective decision. This case describes an 82-year-old female who developed a transudative pleural effusion despite having an underlying malignancy, highlighting the importance of integrating clinical judgment into pursuing thoracentesis with the cytological examination.

Octreotide is a somatostatin analogue used for treating congenital chylothorax and congenital hyperinsulinism in infants. By increasing splanchnic arteriolar resistance and decreasing gastrointestinal blood flow, octreotide indirectly reduces lymphatic flow in chylous effusions.Splanchnic ischaemia following octreotide predisposes infants to necrotising enterocolitis (NEC). Although NEC occurrence in infants treated with octreotide for hyperinsulinaemic hypoglycaemia has been reported widely, its incidence in infants with chylothroax is low. We describe a case of congenital chylothorax in a pr

PURPOSE OF REVIEW: Malignancy accounts for a substantial proportion of pleural effusions, and cytologic examination of pleural fluid is the easiest way to establish the diagnosis. This review summarizes relevant advances in the diagnostic cytopathology of pleural effusions for the practicing pulmonologist.
RECENT FINDINGS: Cytologic analysis using a stained smear and cell block preparation is able to provide the diagnosis in about 55% of malignant effusions. Processing a maximum of two separate specimens (provided the initial examination is negative) with an approximate volume of 40 ml each i

BACKGROUND: Pleural effusion is common in routine medical practice and can be due to many different underlying diseases. Precise differential diagnostic categorization is essential, as the treatment and prognosis of pleural effusion largely depend on its cause.
METHODS: This review is based on pertinent publications retrieved by a selective search in PubMed and on the authors' personal experience.
RESULTS: The most common causes of pleural effusion are congestive heart failure, cancer, pneumonia, and pulmonary embolism. Pleural fluid puncture (pleural tap) enables the differentiation of a tr

We report the use of point-of-care ultrasound to identify a hydropneumothorax in a 6-year-old previously healthy girl arriving at the emergency department in severe respiratory distress. The use of point-of-care ultrasound in this instance allowed for the expedient management and stabilization of the hydropneumothorax with emergent thoracentesis. The patient was ultimately found to have Streptococcus pyogenes bacteremia and necrotizing pneumonia.

The authors report a case of pseudochylothorax revealed by pleural tuberculosis. Classically, pseudochylothorax is a late complication of chronical and calcified pleurisy especially sequellae of tuberculosis treated by induced pneumothorax. Treatment consist on thoracentesis or aspiration with frequent remove. Surgical decortication can be useful. Specific chemotherapy is only necessary in the patients in whom tuberculosis is present or never treated with antibiotics.

Chylothorax is defined as a pleural effusion with triglyceride levels greater than 110 mg/dL and/or chylomicrons present in the pleural fluid. A chylothorax may be classified as traumatic or nontraumatic, with malignancy being the most common cause of atraumatic chylothoraces. Herein, we present the case of a 63-year-old woman with a past medical history of a mediastinal teratoma and stage III colon adenocarcinoma who presented to the emergency room with new-onset shortness of breath. A week prior to presentation, she was diagnosed with metastatic renal cell carcinoma after a retrocrural lymph
